Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Cíl: Cílem práce je zhodnotit endovaskulární léčbu iatrogenních poranění renálních tepen. Metodika: Od listopadu 2005 do listopadu 2013 jsme na našem pracovišti léčili jedenáct pacientů s iatrogenním poraněním renálních tepen. Jednalo se o pět žen a šest mužů ve věku od 45 do 71 let, věkový průměr byl 57, 8 roků. K iatrogennímu poranění došlo u šesti pacientů při perkutánní extrakci konkrementu, u dvou nemocných při parciální nefrektomii, u jedné nemocné při perkutánní nefrostomii a u dvou pacientů při biopsii transplantované ledviny. U sedmi pacientů bylo příčinou hematurie pseudoaneuryzma, u tří nemocných pseudoaneuryzma s arteriovenózní pištěli a u jednoho nemocného arteriovenózní pištěl. Vždy byly poraněny periferní větve renální tepny. U všech nemocných byly k embolizaci použity spirály. Výsledky: Technická úspěšnost v našem souboru byla 100%. U všech nemocných došlo k zástavě krvácení a u nemocných s transplantovanou ledvinou se zlepšila funkce. Nedošlo k žádné závažné komplikaci. Závěr: Endovaskulární léčba iatrogenních poranění renálních tepen je jednoduchá, bezpečná a velmi účinná., Aim: To evaluate the outcome of endovascular treatment of iatrogenic injury of renal arteries. Method: From November 2005 to November 2013 we treated 11 patients with iatrogenic injury of renal arteries. There were 5 women and 6 men, aged from 45 to 71; the average age was 57.8 years. Cause of injury was percutaneous extraction of concrements in 6 patients, partial nephrectomy in 2 patients and biopsy of transplant kidney in 2 patients. All patients had hematuria. Source of hematuria was pseudoaneurysm in 7 patients, pseudoaenurysm and arteriovenous fistula in 3 patients and arteriovenous fistula in one patient. Peripheral branches were injured in all patients. Renal injury was managed by coil embolization in all cases. Results: Technical success was achieved in 100%. Clinical success (stop hematuria, hemodynamic stability, improved function of transplant kidneys) was 100%. No major complication was found. Cíl. Referujeme tři vybrané kazuistiky pacientů s ischemií ruky vzniklou v souvislosti s dialyzačním a-v zkratem, která byla léčena embolizací distální a.radialis. Metoda. Na našem pracovišti jsme za roky 2009-2012 provedli embolizaci a. radialis distálně od anastomózy dialyzačního zkratu celkem u čtyř pacientů se steal syndromem. Jednalo se o tři ženy a jednoho muže, průměrný věk byl 69 let. Všem pacientům jsme implantovali embolizační spirály do a. radialis distálně od anastomózy A-V zkratu. Výsledky. U všech pacientů došlo k vymizení příznaků steal syndromu, tři pacienti jsou od té doby bez příznaků. U jedné pacientky došlo po 14 dnech od embolizace ke spontánní rekanalizaci embolizované části tepny a po 5 měsících k reci divě klinických příznaků steal syndromu, po reembolizaci Amplatz plugem došlo k trvalému vymizení klinických příznaků. Závěr. Embolizace a.radialis distálně od a-v anastomózy dialyzačního zkratu je účinný způsob léčby steal syndromu způsobených přítomností zkratu. Je ale nutné si uvědomit, že se jedná většinou o trvalý uzávěr a.radialis, a hrozí tedy riziko ischémie ruky v případě uzávěru ostatních tepen zásobujících ruku., Aim. We present three cases of patients with hand ischemia which was associated with dialysis arteriovenous fistula and which was treated by embolization of distal radial artery. Methods. During years 2009-2012 in our department we performed embolization of radial artery distal to the anastomosis of dialysis fistula in 4 patients with steal syndrome. These were three women and one man, average age 69 years. In all cases we implanted embolization spirals to radial artery distal to the anastomosis of arteriovenous fistula. Results. In all patients the symptoms of steal syndrome disappeared, three patients are symptom-free until now. In one patient a spontaneous recanalization of the treated part of the artery occurred 14 days after the embolization and clinical symptoms of steal syndrome returned after 5 months. Permanent result in this case with no clinical symptoms was achieved by re-embolization by Amplatz plug. Conclusion. The embolization of radial artery distal to the arteriovenous anastomosis of dialysis fistula is an effective way to treat steal syndrome caused by the presence of the shunt.
