Autoři popisují jednotlivé bioptické metody, které jsou dnes součástí úplné mamární diagnostiky, jejich indikace, techniku provedení, kontraindikace a komplikace, spolehlivost pro stanovení histologické diagnózy a význam pro následnou léčbu u maligních nádorů prsu. Jednotlivé bioptické techniky jsou podány v historickém přehledu. Nejdéle užívaná aspirace tenkou jehlou je dnes ve většině screeningových zemí užívána pro biopsii solidních lézí velmi omezeně a její hlavní význam je v aspiraci tekutinových útvarů v prsu. Core cut biopsie (CCB) je standardní metodou pro získání histologické diagnózy u solidních lézí prsní žlázy a její zavedení v devadesátých letech 20. století znamenalo zásadní změnu pro následnou chirurgickou a onkologickou léčbu karcinomu prsu. Jsou referovány nejnovější bioptické metody, které využívají k odběru většího množství tkáně z prsu vakua. Jedná se o vakuovou biopsii, která stanoví velmi přesnou histologickou diagnózu u lézí, kde core biopsie může podhodnotit histologickou diagnózu. Je to zejména v případě histologicky hraničních lézí, jako jsou in situ karcinomy a atypické hyperplazie. Intact BLES biopsie (radiofrekvenční ablace) je vysoce přesná bioptická metoda, která může odebrat ložisko v jednom vzorku tkáně. Jako jediná bioptická metoda je schopná zachovat pro patologa hodnotitelný okraj ložiska, a může tak při další technické inovaci uspět i jako terapeutická metoda u minimálních maligních lézí prsu., The authors describe the different biopsy methods, which are now a part of the complete mamodiagnostics, their indications, technique, contraindications and complications, its reliability for histological diagnosis and the importance of subsequent treatment in malignant breast tumors. Individual biopsy techniques are presented in the historical overview The use of the oldest biopsy method the fine needle aspiration biopsy for solid lesions is nowadays very limited in almost all countries provided breast screening and the main importance of this biopsy method is the aspiration of fluid formations in the breast. The core cut biopsy is the standard method for obtaining histological diagnosis in solid lesions and its introduction in the 90s of the last century marked a major change for the post-surgical and oncological treatment of breast cancer. The latest vacuum-assisted biopsy methods used to receive a larger amount of tissue from the breast. It is a vacuum-assisted biopsy, which provides very accurate histological diagnosis for lesions where the core biopsy may underestimate the histological diagnosis. It is especially in the case of histologically borderline lesions such as carcinoma in situ and atypical hyper-plasia. The Intact BLES biopsy (radiofrequency ablation) is highly accurate biopsy method enables to remove the lesion in a single tissue sample. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Conclusion: According to results, not only pain reduction and improving/maintaining of functional status is the priority of care in elderly with RA, but also reduction of anxiety and depression is important, mainly in older, with longer duration of disease, co-morbidity and also for elderly living alone., Mária Sováriová Soósová, Renáta Suchanová, Libuša Tirpáková, and Literatura
Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. Background and purpose: The prognosis of inoperable carcinomas of esophagus is poor, and therapeutic efforts are generally limited to palliation. The aim of this study is to retrospectively evaluate the effectiveness of intraluminal high dose rate brachytherapy in the palliative treatment of tumorous esophageal stenoses. Patients and methods: Between February 1996 and June 2011 intraluminal brachytherapy was performed in 41 patients with esophageal carcinoma (squamous cell carcinoma in 19 cases and adenocarcinoma in 22 cases). All patients had dysphagia at presentation. Brachytherapy was performed using high dose rate afterloading system. Results: Dysphagia was improved in majority of patients. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Cíl: Cílem práce bylo shromáždit dostupná klinická, radiologická a patologická data o kalcifikujícím pseudotumoru nervové osy (CAPNON). Soubor: Soubor je tvořen kazuistikou a současně přehledem všech pacientů publikovaných v anglické literatuře, vyhledaných v databázi PubMed pomocí klíčových slov: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi a fibro‑osseous lesion. Zahrnuty jsou taktéž všechny citace případů v nalezených publikacích. Výsledky: Včetně naší pacientky jsme v literatuře nalezli celkem 72 pacientů. Intrakraniálně bylo lokalizováno 62 % těchto lézí, v páteřním kanále 31 % a v kranio‑cervikálním přechodu zbylých 7 %. Průměrný věk výskytu je 45,6 (rozmezí 2–83) let. U intrakraniální lokalizace byla tato léze nalezena supratentoriálně v 74 % a infratentoriálně v 26 %. Z nejčastějším projevů intrakraniálních CAPNON jsme nalezli epilepsii v 33,3 %, bolesti hlavy v 24 %, nebo některý z příznaků zadní jámy v 16,7 %. U pacientů s epilepsii byl CAPNON nalezen v temporálním laloku v 50 %. U CAPNON nalezených v páteřním kanále byla dominantním projevem bolest v 82 %, následovaná poruchou chůze v 27 %. V kranio‑cervikálním přechodu byla dominantním příznakem bolest v 80 %. Závěr: CAPNON je velmi vzácnou, benigní, pomalu rostoucí lézí centrální nervové soustavy. Ve většině případů ji nalezneme intrakraniálně s lehkou převahou u mužského pohlaví. Původ této léze není zcela jasný. Diagnóza CAPNON by měla být zvažována setkáme‑li se s nálezem solidně kalcifikované léze při vyšetření CT mozku a hypodenzní léze v T1 a T2 obraze při vyšetření MR. Léčebnou modalitou je radikální odstranění této léze. Incidentální léze musí být monitorovány, jelikož mohou růst a stát se symptomatickými. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Aim: The aim of this study was to summarize clinical, radiological and pathological data on calcifying pseudoneoplasm of the neural axis (CAPNON). Methods: A case report of a female patient with CAPNON is presented. In addition, all CAPNON patient case histories published in English language are included. Articles were identified via PubMed searches using the following key words: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi and fibro‑osseous lesion. All cases referenced within the identified publications are also included in this review. Results: Seventy two cases were identified, including our case report. Of these, 62% were intracranial, 31% spinal and 7% were located in the cranio‑cervical junction (CCJ). The mean age of the CAPNON patients was 45.6 (range 2–83) years. 74% of intracranial CAPNON were supratentorial and 26% were infratentorial. Symptoms of intracranial CAPNON included epilepsy in 33.3%, headaches in 24% and posterior fossa symptoms in 16.7%. The majority of patients with epileptic seizures had lesions in the temporal lobe (50%). Pain was the dominant symptom in 82% and gait disorder in 27% of spinal CAPNON cases. Pain was the dominant symptom (80%) in CCJ CAPNON. Conclusion: CAPNON is a rare, benign, slowly‑growing lesion of the central nervous system. In the majority of cases, location is intracranial and there is a slightly higher prevalence in men. The origin of CAPNON remains unclear. CAPNON should be considered whenever a CT scan reveals a calcified lesion combined with hypointensity on T1 and T2-weighted MRI. Radical removal is the treatment of choice. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus.
