V tomto sdělení popisujeme případ 71 leté pacientky se symptomatickou primární hyperparatyreózou s dominujícími neurokognitivními a psychiatrickými symptomy. Je demonstrován vývoj mentálního stavu po odstranění adenomu příštítného tělíska. V diskusi jsou zvažovány předpokládané patogenetické mechanizmy podílející se na částečně reverzibilitě dementního syndromu u této pacientky., In this article we describe a case of 71-years old female patient with primary hyperparathyroidism and dominant neurocognitive and psychiatric symptoms. We demonstrate the development of mental status after surgical removal of parathyroid gland adenoma. K tomu, aby každý mohl bez problémů vykonávat svoji práci, nebyl unavený, podráždený, vyčerpaný a byl fyzicky i psychicky v rovnováze, potřebuje zdravý spánek, který by měl trvat 6-9 hodin. Poruchy spánku, kterými trpí 35-50 % obyvatel v ČR, mohou být primární nebo sekundární, nicméně stanovit příčinu, potvrdit diagnózu a navrhnout správnou terapii je často velmi problematické. K objasnění příčiny poruchy spánku a bdění je nutné kompletní vyšetření neurologické, internistické, psychiatrické a biochemické se zaměřením na vyvolávající příčiny. Své místo má i tzv. „objektivizace poruchy", kdy základní informace o poruše spánku získáváme od druhé osoby, která je v kontaktu s postiženým. Získané informace a závěry jednotlivých vyšetření nám pomáhají adekvátně zahájit terapii nejen farmakologickou (hypnotika, antidepresiva, anxiolytika), ale i psychoterapii, která má význam hlavně u geriatrických pacientů., Everyone needs healthy sleeping of 6-9 hours a day enabling to perform work without problems, to avoid feeling of exhausting and irritation as well as to feel physically and psychically balanced. In Czech Repubhc 35-50 % of population suffer from sleeping defects which could be primary or secondary, anyhow identification of source of problems, confirmation of diagnosis and starting of correct therapy are often very complicated. Identification of causes requires complex neuralgic, internal and psychiatric examination as well as biochemical focused on the eventual initiating causes. It means objective identification of defect where basic information about sleeping defects is acquired from the person who is in close contact with the patient.
