3. Aged population disability processes analysis in the republic of Kazakhstan

Background: We have analyzed the aged population disability processes to establish specifics and regularities of the causal structure of disability among the working and nonworking aged population. Methods: In total, 1208 examination reports of the Medical & Social Expert Commission have been subjected to excerption in Almaty. Results: Persons having the second disability status prevail in the working aged contingent – 4,4%, which is much higher than the ratios for the first and second disability statuses (0,4% and 0,6%, respectively). Among the nonworking population, persons having the second disability status largely prevail too 8,1% (3,1% and 1,1%, respectively). The casual structure of disability among the nonworking disabled persons includes as follows: blood circulatory system diseases (40%), malignant neoplasms (27,2%), and diseases of the eye and its appendages (10,2%). They are followed by endocrine diseases, nutritional and metabolic disturbances (7,6%), bodily injuries (3%), and urogenital system diseases with musculoskeletal system ones 2,3% each. The data collection for the working aged population contingent has found out blood circulatory system diseases (47%) and malignant neoplasms (34,4%). Alongside with that, the distinctive feature among the said aged population cohort is a substantial predominance of bodily injuries (7,4%), endocrine diseases, nutritional and metabolic disturbances (2,3%), and only 1,4% is accounted for diseases of the eye and its appendages., Akmaral K. V roce 2013 vysoké české soudy již vydaly druhé zásadní rozhodnutí ve věci regulačních poplatků podle § 16a zákona č. 48/1997 Sb., o veřejném zdravotním pojištění a o změně a doplnění některých souvisejících zákonů, ve znění pozdějších předpisů. První rozhodnutí vydal jako své usnesení vedené pod sp. zn. Konf 38/2012 zvláštní senát zřízený podle zákona č. 131/2002 Sb., o rozhodování některých kompetenčních sporů. Účelem tohoto rozhodnutí pak bylo rozhodnout o právní povaze regulačních poplatků. Druhé rozhodnutí pak vydal Ústavní soud ČR., In the spring 2013 the special tribunal consisting of judges of the Supreme Court and the Supreme Administrative Court resolved a conflict of competencies between civil court and health insurance company. The dispute was who should decide on the obligation to pay regulatory fees under the Public Health Insurance Act. The special tribunal concluded that the competent authority is a civil court. The reason for this decision is that regulatory fees are subject to private law. However, arguments to support the decision are not too relevant. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The choice of researched sample was intentional. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. 7. Bolest u novorozenců a možnosti jejího nefarmakologického tlumení

Cíl: Cílem příspěvku je literární analýza dosavadních nejvíce využívaných nefarmakologických metod tlumících bolest u novorozencůpro nejlepší praxi založenou na důkazech. Metodika: Pro získání dat za období 2000–2012 byly použity elektronické licencované a volně přístupné databáze. Hodnocení nalezených důkazů bylo realizováno dle metodiky Fineout-Overholt, Johnstona (2005), kteří řadí studie dle specifi ckých kritérií do sedmi kategorií. Do výběru předkládaného příspěvku byly zařazeny důkazy úrovně I, II, III. Zvolená kritéria splňovalo 42 studií (4 meta-analýzy, 6 systematických přehledů, 1 multicentrická studie a 31 randomizovaných kontrolovaných studií). Výsledky: Nefarmakologické metody využívané k tlumení bolesti u novorozenců jsou mnohem více efektivní, pokud jsou použity v kombinaci s dalšími nefarmakologickými metodami, jako jsou muzikoterapie, zavinutí, usnadněné zasunutí, multisenzorická stimulace, klokánkování a non-nutritivní a nutritivní sání. Závěr: Nefarmakologické postupy jsou účinné a vedou k efektivnímu zmírnění bolesti především při drobných procedurálních výkonech jako je odběr krve z patičky pomocí lancety, venepunkce, apod. Léčba bolesti a řízení její prevence by měla být nezbytnou součástí standardní i intenzivní zdravotní péče o novorozence., Aim: The aim of study is a literary analysis of the current most used non-pharmacological methods of relieving pain in neonates for the best evidence-based practice. Methods: For obtain data for the period 2000–2012 were used e-licensed and freely accessible database. Evaluation of the evidence found was carried out according to the methodology Fineout-Overholt, Johnston (2005), who put the study according to specifi c criteria in seven categories. The selection of the present study were classifi ed evidence level I, II, III. Selected criteria met the 42 studies (4 meta-analyzes, 6 systematic reviews, 1 multicenter study and 31 randomized controlled study). Results: Non-pharmacological methods used to control pain in neonates are much more eff ective when used in combination with other non-pharmacological methods, such as swaddling, facilitated tucking, multisensory stimulation, kangaroo care and non-nutritive and nutritive sucking. Conclusion: Non-pharmacological methods are eff ective and lead to eff ective pain relief especially when minor procedural performance as the heel lanceting, venipuncture, etc. Treatment of pain and management prevention should be an essential part of the standard and intensive care of the newborn., Jana Chromá, Lucie Sikorová, and Literatura
