Trofoblastická nemoc představuje velmi heterogenní skupinu benigních i vysoce maligních stavů. Hlavní společnou charakteristikou je nízká incidence onemocnění v populaci, která může vést k diagnostickým rozpakům a závažným terapeutickým chybám. Současným cílem specializovaných center v Evropě i ve zbytku světa je stanovit jasná kritéria, která budou zárukou adekvátní léčby a porovnatelných výsledků. Článek přináší aktuální pohled na postavení chemoterapie a dalších léčebných modalit v léčbě trofoblastické nemoci., Trophoblastic disease is a very heterogeneous group of benign and also highly malignant entities. All forms of tro - phoblastic disease are characterized by rare incidence. This explains a potential risk of diagnostic and therapeutic errors which can lead to very serious consequences. The contemporary trend across different countries is to establish definite criteria for adequate management of patients with this specific disease which also allow the comparison of treatment results. Přehled problematiky: Magnézium je čtvrtým nejčastějším kationtem v lidském organizmu a po draslíku druhým kationtem v intracelulárním prostoru. Účastní se jako kofaktor enzymů metabolických reakcí, tvorby a utilizace energie a je nezbytný k udržení elektrolytové rovnováhy. Tělo dospělého člověka obsahuje asi 0,043 % (535-730 mmol) magnézia, což je v přepočtu asi 22-30 g (60 % v kostěném skeletu, 40 % intracelulárně ve svalovině a měkkých tkáních, 1 % (7,5-10 mmol) je obsaženo v extracelulární tekutině). Denní potřeba magnézia se odhaduje na 10-20 mmol, tj. 200-400 mg. Ve vztahu k podané dávce pak platí, že čím vyšší podaná dávka, tím nižší procento rezorbce ze střeva. Z doporučené denní dávky 15 mmol (365 mg) se rezorbuje pouze okolo 1/3. Význam magnézia v těhotenství: Důsledkem manifestního nedostatku magnézia může být: 1. zvýšené riziko potratu - předčasný porod (inkompetence hrdla děložního, předčasný odtok vody plodové), 2. placentární insuficience a hypotrofizace plodu, 3. rozvoj gestózy u matky, 4. zvýšené riziko astmatu a ekzému u dětí. Magnézium sulfát, podávaný těhotným ohroženým předčasným porodem (23+0 -30+0), snižuje riziko dětské mozkové obrny u přežívajících novorozenců. Souhrn: Nedoporučuje se používat intravenózní magnézium sulfát v indikaci léčby hrozícího předčasného porodu déle jak 5-7 dní. Tato omezení se nijak nevztahují na perorální užívání magnézia., Objectives: Magnesium is the fourth most common cation in the human body, and the second cation to potassium in the intracellular space. Ultrazvukové vyšetrenie maternice po pôrode zahŕňa niektoré špecifiká, ktoré vyplývajú z rýchlo sa meniacich rozmerov maternice, ale aj compliance pacientky vo včasnom šestonedelí. Pri riešení popôrodných komplikácií je potrebné poznať normálny ultrazvukový nález, dynamické zmeny maternice v priebehu šestonedelia, aby sme vedeli lepšie rozpoznať patologický priebeh od normálneho priebehu šestonedelia. Klinický význam ultrazvukového posúdenia veľkosti maternice ako aj oblasti jazvy po cisárskom reze v priebehu šestonedelia doteraz nie je dostatočne popísaný. Prehľadový článok rozoberá špecifiká ultrazvukovej diagnostiky po vaginálnom pôrode a cisárskom reze. Určení prahu viability je výchozím bodem pro zvažování léčebného postupu. Kromě stupně zralosti plodu a jeho zdravotního stavu závisí viabilita na vývoji medicínské technologie. Je podmíněna také sociálně-ekonomickými a kulturními faktory. Hranice viability z hlediska možnosti narodit se živě a přežít leží mezi 22.-24. týdnem. Dohoda odborné komunity o poskytování aktivní péče vychází z pravděpodobnosti přežití a přežití bez závažného postižení. Ve vyspělých zemích je resuscitace a intenzivní péče od 26. gestačního týdne většinou povinná, protože děti narozené v tomto týdnu mají vysoké šance na přežití bez závažného postižení a dobrou kvalitu života.
