- Cíl: cílem našeho šetření bylo vyhodnotit moţnou přítomnost bolesti u klientů s demencí, kteří byli přeloţeni na gerontopsychiatrické lůţko psychiatrické léčebny v Kroměříţi z jiného zdravotnického zařízení. Metodika: studium překladové ošetřovatelské dokumentace z odesílajících zdravotnických zařízení, škála pro hodnocení bolesti u pacientů s demencí (PAINAD). Soubor: klienti přeloţení do psychiatrické léčebny v Kroměříţi, obojího pohlaví, starší 65 let, pro demenci jakéhokoliv typu. Tíţe demence byla středně těţká a těţká (bylo hodnoceno při přijetí na gerontopsychiatrické lůţko celkovým skóre dosaţeným v Mini Mental State Examination - MMSE - v rozmezí 0 - 17). Výsledky: pomocí škály PAINAD byla zachycena bolest u 26,7 % přeloţených klientů se středně těţkou nebo těţkou demencí. Bolest však byla řádně zadokumentována v překladových ošetřovatelských zprávách jen u 8,96 % klientů. Téměř třetina ošetřovatelské překladové dokumentace (29,9 %) měla zcela nevyplněný, nebo nedostatečně vyplněný formulář pro hodnocení bolesti. Téměř pětina (19,4 %) ošetřovatelských překladových zpráv fyzicky vůbec neobsahovala ţádný takový formulář. Závěr: přítomnost nepodchycené nebo nedostatečně vyhodnocené bolesti u klientů s kognitivními poruchami je závaţným problémem moderního ošetřovatelství. Nutná je potřeba neustálé edukace zdravotnického personálu, kvalitního vedení ošetřovatelské dokumentace, ale také potřeba důslednějšího systému kontrol z řad vedoucích pracovníků., Aim: to evaluate presence of pain in clients of both sexes, older than 65, who have been admitted to Mental hospital Kroměříţ due to dementia of any type. Methods: study of nursing documentation from dispatching health care facilities, PAINAD scale (Pain assessment in advanced dementia). Severity of dementia was moderate of severe (this was evaluated by Mini-Mental State Examination total score during admission process to our hospital, scores ranged from 0 to 17). Results: pain was detected in 26.7% of clients with moderate or severe dementia by means of PAINAD scale, compared to 8.96% of clients with pain which has been reported in nursing documentation. Almost in one third of nursing documentation (29.9%) no data or incomplete data about pain were found. Almost one fifth (19.4%) of nursing documentation have not contained any tool for pain screening. Concusion: undiagnosed pain in cognitively impaired clients is a severe problem of nursing care. - Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time. - Jaká je hodnota života, či poněkud specifičtěji, jaká je hodnota lidského života? Podle nauky o posvátnosti lidského života má každý lidský život svou vnitřní hodnotu, tato teorie však v současné bioetice rozhodně nepředstavuje ortodoxii. John Harris je jedním ze zastánců nové teorie hodnoty lidského života (především ve své publikaci The Value of Life), podle níž jsou vnitřně hodnotné pouze osoby. Osoby se definují jako bytosti - lidského či jiného druhu - schopné vyšších kognitivních funkcí, které díky tomu mohou hodnotit svůj vlastní život (vědomí sebe sama je nutnou podmínkou osoby) a mohou si přát dále žít. Hlavním cílem této kritické studie je zhodnocení Harrisových argumentů. V první části se odhaluje obecná logická struktura jeho argumentace a zdůrazňují se její důsledky pro některá témata současné bioetiky (interrupce, infanticida, eutanazie). V druhé části práce se odhalují nedostatky Harrisova myšlení. Ukazuje se, že jeho argumentace je kruhová, předpokládá teorii, kterou se snaží odvodit. Studie uzavírá, že Harrisova teorie hodnoty lidského života je nesprávná a nevhodná pro současnou bioetickou diskusi., What is the value of life? Or more specifically, what is the value of human life? According to the doctrine of the sanctity of human life, all human beings are intrinsically valuable, but this theory is far from being orthodoxy in current bioethics. John Harris is one of the advocates of the new theory of the value of human life (mainly in his book The Value of Life), according to which intrinsically valuable are only persons. Persons are beings -human or not--capable of higher mental functions that consequently can value their own life (self-consciousness is a necessary condition for personhood) and can have the desire to go on living.The overall aim of this critical study is to go into Harris's arguments in some depth. In the first part the general logical structure of Harris's argumentation is put forward and its consequences for current bioethical issues (abortion, infanticide, euthanasia) are pointed up. In the second part two fatal flaws of Harris's thoughts are exposed. It is showed that his argumentation is circular, it presupposes theory it aims to develop. The article concludes by stating that as fatally flawed, Harris's theory of the value of human life is unjustified and inadequate for dealing with current bioethical issues., David Černý, and Literatura

- Background: There are several environmental factors which influence the preschoolers psychomotor development and the most considerable ones are socioeconomic status and day cares learning&care environment. Aim of this study is to assess how the change of socioeconomic gradient and the quality of care environment influence the average and the differences in psychometric domains of ASQ3 in preschoolers. Method: We assessed the psychometric parameters of all preschoolers enrolled in some of day cares in Vlora city, Albania using the ASQ3. We also assessed the quality of care environment according to ECERSR and the socioeconomic status upon the fathers occupation. Results: In the end of this research we found that not only a considerable percentage of preschoolers included in this study have psychomotor delays, but also that these delays and the average of psychomotor development have a significant relationship with the environment factors, as socioeconomic status and care environment. Conclusions: Children development depends more on instructing capacities of kindergartens staff than mothers. - We have analyzed human resources in healthcare of Kazakhstan over 19852011, and determined general trends and regularities. By the level of provision with physicians, Kazakhstan ranks as one of the leaders in the world, at the same time there have been a deficit of physicians in the Republic over the last 20 years, particularly in rural areas. As per its regions, there is an irregularity in the physicians provision rate: the most part of specialists is concentrated in cities Almaty and Astana, there is a deficit of physicians in the Almaty, Коstanai, NorthKazakhstan and Jambyl Oblasts.
