The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Cíl. Posoudit stupeň korelace výsledků chemické analýzy urolitiázy pomocí výpočetní tomografie s duální energií záření (DECT) s laboratorní chemickou analýzou konkrementů a zhodnotit přínos metody DECT pro klinickou praxi. Metoda. Celkem 67 pacientů bylo vyšetřeno pomocí DECT s požadavkem na chemickou analýzu již známé či suspektní urolitiázy. K analýze chemického složení konkrementů pomocí DECT jsme využili simultánní expozici 140 kV a 80 kV. Získaná data byla analyzována pomocí programu Syngo Dual Energy Siemens v modulu Kidney Stones. U 30 pacientů byl následně proveden urologický výkon s extrakcí litiázy a jejím chemickým rozborem. V tomto souboru jsme pak korelovali výslednou chemickou skladbu urolitiázy získanou oběma metodami. Výsledky. V souboru 30 pacientů se podařilo prokázat u konkrementů velikosti nad 3 mm 100% senzitivitu a 100% specificitu DECT analýzy. V případě velmi drobné urolitiázy (tj. pod 3 mm) je přesnost detekce 100%, ale schopnost diferenciace na jednotlivé typy urolitiázy snížená na 97 %. Závěr. Metoda DECT analýzy urolitiázy vykazuje u konkrementů nad velikost 3 mm 100% shodu výsledků v porovnání s chemickou laboratorní analýzou močových konkrementů a má tedy velký přínos pro klinickou praxi, zejména v možnosti včasného zahájení vhodného terapeutického postupu., Aim. To assess the degree of correlation between the results of chemical analysis of urolithiasis using dual energy computed tomography (DECT) with laboratory chemical analysis of stone, and to evaluate the contribution of this method to clinical practice. Method. Total 67 patients underwent DECT in order to performe the chemical analysis of already known or suspected urolithiasis. To analyze the chemical composition of the stone using DECT, we used the simultaneous exposure to 140 kV and 80 kV. The obtained data were analyzed using software Syngo Siemens Dual Energy in program "Kidney Stones". The method of the analysis was based on a material decomposition algorithm. In 30 patients, the extraction ofthe stone followed and the chemical and crystalograpic analysis was made. The results of virtual chemical analysis and laboratory findings were compared. Results. In the stones greater than 3 mm, sensitivity and specificity reached 100% using DECT analysis. In the case of very small stone disease (ie. less than 3 mm), the detection reached 100% accuracy, but the ability to differentiate the correct type of stone disease decreased to 97 %. Conclusion. V České republice je kolorektální karcinom jedním z nejčastějších zhoubných nádorů a až 40 % pacientů je diagnostikováno v pokročilém stadiu. Chirurgická léčba je základní léčebnou modalitou vedoucí k vyléčení, a to i v případě pokročilého onemocnění. Zavedení nových cytostatik do praxe výrazným způsobem rozšířilo léčebné možnosti i u pacientů, kteří nejsou primárně chirurgicky léčitelní. V kombinaci s biologickou léčbou pak lze dosáhnout operability i u donedávna inoperabilních stavů, a tím tak významně prodloužit život pacientů s metastatickým kolorektálním karcinomem. U pacientů s metastatickým postižením jater jak primárně operabilním, tak inoperabilním je ve většině případů indikována neoadjuvantní chemoterapie/chemobioterapie. I když dosud nebyl definován nejvhodnější režim, základním požadavkem na něj je jeho nízká toxicita a vysoká účinnost., Colorectal cancer is one of the most common malignant tumors in the Czech Republic, and up to 40 % of patients are diagnosed at an advanced stage. Surgical treatment is the primary modality of therapy leading to cure colorectal cancer even in advanced disease. The introduction of new chemotherapeutic agents in practice greatly increased treatment options in patients who are not surgically curable. In combination with biological therapy, it can be achieved operability even in recently unresectable stage and thereby significantly prolonged life of patients with metastatic colorectal carcinoma. In patients with metastatic liver disease, both resectable or unresectable, is usually used neoadjuvant chemotherapy/chemobiotherapy. Úvod: Incidence kolorektálního karcinomu narůstá. Nejlepší léčebné výsledky má multimodální léčba, jejíž základní součástí je snaha o R0 resekci všech nádorových ložisek. Materiál a metodika: Byla provedena literární rešerše v souvislosti z vlastními zkušenostmi. Výsledky: Spektrum chirurgických zákroků zahrnuje endoskopickou submukózní resekci, transanální endoskopickou mikrochirurgii, laparoskopické resekce, otevřené resekce a cytoredukční chirurgii s peritonektomií a hypertermickou intraperitoneální chemoterapií. Nově se kromě konceptu totální mezorektální excize začíná uplatňovat také koncept totální mezokolické excize, který vede k nižšímu počtu lokálních recidiv a zvýšenému přežívání. Závěr: Stejně jako protinádorová farmakoterapie se významně stále vyvíjí i chirurgická léčba. K určení správné pozice jednotlivých chirurgických postupů v multimodálním algoritmu léčby je nutné sledování pacientů ve vhodných databázích, případně je léčit v rámci studií., Background: The incidence of colorectal carcinoma has been increasing. Only a multimodal treatment provides the best results. Surgical R0 resection of all tumour sites is the goal of all treatment regimes. Material and methods: A survey on the literature and our experiences has been conducted. Results: Surgical treatment comprises endoscopic submucosal dissection, transanal endoscopic microsurgery, laparoscopy, open surgery and cytoreduction surgery with the hyperthermic intraperitoneal chemotherapy. The news is the total mesocolic excision that similarly to the total mesorectal excision provides fewer local relapses and higher long-term survival. Conclusion: Similarly to anti-tumoral medical therapy, the surgery has been developing as well.
