Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Women are significantly less affected than men, with the ratio of women to men being 4:1 in the Czech Republic. In terms of risk factors, smoking and alcohol consumption play a crucial role. The mean age of patients ranges from 50 to 60 years of age. The case reports present a patient with oropharyngeal cancer and another patient with cancer of the soft and hard palate and cancer of the palatine tonsil. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed. Centrum pro tyreoidální onkologii a endokrinní orbitopatie Nádory štítné žlázy vycházejí z folikulárního epitelu štítné žlázy, jsou hormonálně dependentní, vyskytují se 5x častěji u žen než u mužů ve všech věkových skupinách, obvykle dobře vychytávají jod. Klinicky se může projevovat jako lokálně rostoucí uzel ve štítné žláze, nádorová infiltrace spádových krčních lymfatických uzlin, vzdálené plicní a kostní metastázy. Diagnostika se opírá o sonografické vyšetření, tenkojehlou aspirační biopisii, hladinu Tg. Léčba je interdisciplinární. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T.
