Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. Závěry: Výzkumné studie (n = 34) a jeden systematický přehled ukazují pozitivní vliv metody klokánkování na nedonošeného novorozence umístěného na Jednotce intenzivní péče v oblastech biopsychosociálních. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů s demencí pohledem rodinných příslušníků. Dále pak porovnat vnímání kvality života seniorů s demencí rodinnými příslušníky v závislosti na prostředí, ve kterém senior žije. Metodika: Byl proveden kvalitativní výzkum, metodou sběru dat byl nestandardizovaný rozhovor. Výzkumu se zúčastnilo 9 rodinných příslušníků pacientů s demencí. Na základě analýzy dat byly stanoveny 4 kategorie: 1. změny v psychické oblasti, 2. změny kognitivních funkcí, 3. změny v oblasti aktivit denního života a instrumentálních všedních činností a 4. celkové vnímání kvality života. Výsledky: Z pohledu rodinných příslušníků nejvíce zhoršuje kvalitu života seniorů s demencí ztráta paměti, která je vyřazuje z běžného života, dále ztráta schopnosti orientace v prostoru a čase a ztráta schopnosti vykonávat činnosti běžného denního života. Kvalitu života snižují také smyslové poruchy, které pacienty uvádějí do ještě větší sociální izolace a mohou v nich podporovat nárůst paranoidity. Rodinní příslušníci hodnotili kvalitu života svých blízkých v domácím prostředí výrazně lépe v porovnání s kvalitou života pacientů s demencí umístěných v domově pro seniory. V domácím prostředí měli senioři více podnětů, byli více aktivizováni a vtahováni do rodinného a společenského života. Po nástupu do institucionální péče došlo k zhoršení příznaků onemocnění a postupnému vyřazení nemocných z aktivizačních programů. Závěr: Důležitou součástí zvýšení kvality života pacientů s demencí je podpora rodinných pečovatelů, aby nemocní mohli zůstat co nejdéle ve svém přirozeném prostředí., Objective: The aim of the research was to determine the quality of life of seniors with dementia from the perspective of family members. Furthermore, to compare the perceptions of quality of life of seniors with dementia by family members depending on the environment in which the senior lives. Methodology: A qualitative research was performed, for the method of data collection was used non-standardized interview. Research was attended by nine family members of patients with dementia. Based on analysis of data there were determined four categories: 1. changes in the psychological field, 2. changes in cognitive function, 3. changes in the area of activities of daily living and instrumental activities of everyday life and 4. overall perception of quality of life. Results: From the perspective of family members most deteriorating quality of life of seniors with dementia is the memory loss, which excludes them from normal life, as well as loss of ability to orientation in space and time, and loss of ability to perform normal activities of daily life. The sensory disturbance also reduces the quality of life, which leads patients into a greater social isolation and this may promote the growth of the paranoia. Family members rated the quality of life of their loved ones at home as significantly better in comparison with the quality of life of patients with dementia placed in the home for the elderly. In a domestic environment have seniors more stimulations, are more activated, and are more drawn into the family and social life. After coming into institutional care occurs worsening of symptoms and the gradual removal of patients from activation programs. Cíl: Cílem práce bylo zjistit a následně porovnat kvalitu života žen s přirozeným otěhotněním s kvalitou života žen po umělém oplodnění. Kvalita života byla u těchto dvou skupin žen hodnocena během těhotenství dvakrát, a to ve 20. – 28. týdnu těhotenství a před termínem porodu. Metodika: Ke sběru empirických dat byl použit standardizovaný dotazník Světové zdravotnické organizace WHOQOL – BREF. Výzkumný vzorek tvořilo 160 těhotných žen, z nichž bylo 80 žen po spontánním otěhotnění a 80 žen po umělém oplodnění. Výzkum byl realizován ve Fakultní nemocnici v Ostravě, Gyncentru v Ostravě a Gyncentru v Opavě. Výsledky: Těhotné ženy po spontánním i po umělém oplodnění hodnotily kvalitu života v těhotenství jako dobrou při prvním (20. – 28. týden těhotenství) i při druhém hodnocení (před termínem porodu). Spokojenost se zdravím hodnotily oba soubory žen při prvním i druhém hodnocení v průměru jako dobrou. Největší rozdíl v hodnocení byl v první doméně posuzující fyzické zdraví. Ve 20. – 28. týdnu těhotenství hodnotily ženy po spontánním otěhotnění své fyzické zdraví lépe než před termínem porodu (p < 0,001). Naproti tomu u žen po umělém oplodnění nedošlo k tak významnému rozdílu mezi prvním a druhým hodnocením. Při hodnocení dalších tří domén těhotnými ženami ať už po spontánním nebo umělém oplodnění nedošlo k výrazným rozdílům v hodnocení. Závěr: Kvalita života přímo nezávisí na způsobu otěhotnění, ale je velmi individuální, stejně tak i výskyt nepříjemných pocitů provázejících těhotenství., Objective: The aim of the work was to find out and consequently compare the quality of life of women who got pregnant naturally with those who underwent artificial insemination. Quality of life was evaluated twice during the pregnancy, between the 20th and 28th week and just before actual childbirth. Methods: A standardized questionnaire WHOQOL – BREF was used for empirical data collection. The sample was made up of 160 pregnant women divided into two groups: 80 women who had got pregnant naturally and 80 women who underwent artificial insemination. The research was realized at the Faculty hospital Ostrava, Gyncentre Ostrava and Gyncentre Opava. Results: Both groups confirmed quality of life as good during the first and the second evaluation and satisfaction with one's health was on average good as well. The biggest difference was related to physical health. Between the 20th and 28th week of pregnancy women who got pregnant naturally determined their physical health as better than before actual childbirth (p < 0,001). However, not so for women who were artificially inseminated, where differences were not so significant. In the other areas of the study no significant differences were found between either groups.
