Úvod: Zhoršení výživy u seniorů, bývá popisováno ve spojitosti s institucionální péčí. Jako vhodný nástroj pro posouzení stavu výživy se jeví Škála pro hodnocení stavu výživy – MNA. Cíl výzkumu: Zjistit stav výživy u seniorů ve vybraných institucích prostřednictvím Škály pro hodnocení stavu výživy – MNA a následně analyzovat hodnoty Body mass index (BMI) u sledované populace. Metodika: Výzkumný soubor tvořilo 200 osob ve věku nad 60 let v institucionální péči. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z 200 seniorů v institucionální péči dle Škály pro hodnocení stavu výživy (MNA) bylo ve stavu dobré výživy 32 %, rizikem podvýživy bylo ohroženo 38 % seniorů a podvýživou trpělo 30 %. Byl zjištěn statisticky významný rozdíl ve stavu výživy v závislosti na druhu instituce (p=0,001). Byl zjištěn statisticky významný rozdíl mezi sledovanými institucemi v hodnotách BMI (p=0,0084). Hodnoty BMI vyšší než 23 byly u 65 % seniorů, hodnoty BMI nižší než 19 byly u 16 % seniorů. Závěr: Z výsledku studie vyplývá, že pouze jedna třetina seniorů byla ve stavu dobré výživy, což vede k nutnosti včasné detekce stavu výživy prostřednictvím MNA., Introduction: Nutritional disorders in the elderly are described in relation to institutional care. Mini Nutritional Assessment (MNA) was used for assessing nutritional status. Objective: To assess the nutritional status of the elderly in selected institutions through Mini Nutritional Assessment (MNA) and to analyze the Body Mass Index (BMI) of the studied population. Methods: The research sample consisted of 200 people aged over 60 years in institutional care. Statistical tests were evaluated at a significance level of 5%. Results: Of the 200 seniors in institutional care according to MNA, 32% were in good nutrition condition, 38% of respondents were at risk of malnutrition and 30% of seniors suffered from malnutrition. A statistically significant difference in nutritional status was revealed depending on the type of institution (p=0.001) and a statistically significant difference between the observed values of institutions in terms of BMI (p=0.0084). Values of BMI in excess of 23 were identified in 65% of the elderly, values of BMI below 19 were found in 16% of elderly. Conclusion: The results show that only one third of the elderly were in good nutritional status. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. Cíl: Cílem výzkumného šetření bylo získat informace o protektivních a rizikových faktorech stravování a stravovacích návyků ve vztahu k prevenci kolorektálního karcinomu, a to u záměrně vybrané skupiny adolescentů a dospělých. Výsledky byly porovnávány mezi oběma skupinami a s údaji publikovanými v odborných časopisech. Metodika: Pro výzkum bylo zvoleno kvantitativní dotazníkové šetření za pouţití nestandardizovaného dotazníku vlastní tvorby hodnotícího způsob stravování u obou skupin respondentů. Šetření probíhalo ve dvou záměrně vybraných středních školách a ve čtyřech podnicích v Hradci Králové v období října aţ prosince 2012. Výsledky: Celkem bylo do výzkumného šetření zařazeno 343 respondentů, z toho 123 adolescentů (36 %) a 220 dospělých (64 %). Statistické testování prokázalo, ţe se dospělí stravují kvalitněji neţ adolescenti, přesto však obě skupiny konzumují ovoce a zeleninu v nedostatečném mnoţství a naopak často konzumují červené maso, masné výrobky a potraviny upravené smaţením. Špatné stravovací návyky dosvědčuje i prevalence nadváhy a obezity; 20,4 % dotázaných adolescentů a 63,6 % dospělých má vyšší neţ normální váhu. Třetina adolescentů se navíc stravuje v řetězcích s rychlým občerstvením. Závěr: Zdravá výţiva a vhodné stravovací návyky hrají klíčovou roli v prevenci mnoha onemocnění. Spolu s adekvátní pohybovou aktivitou a nekouřením zásadně sniţují riziko vzniku nejen kolorektálního karcinomu, ale i dalších nádorových i nenádorových onemocnění. Přesto však způsob stravování není ze zdravotního hlediska příznivý a přibývá osob s nadměrnou hmotností., Aim: The aim of the research was to get information about protective and dangerous factors of nutrition and eating habits in relation to the prevention of colorectal cancer. It focuses on intentionally chosen groups of adolescents and adults. The results were compared between both groups of respondents and with information published in professional periodicals. Methods: Quantitative research based on non-standardized questionnaires of own formulation evaluating a way of alimentation of both groups of respondents was chosen. The research took place in two chosen secondary schools and in four companies in Hradec Králové in a period from October to December 2012. Results: The total amount of 343 respondents participated in the research, namely 123 adolescents (36 %) and 220 adults (64 %). Statistical testing has shown that the eating habits of adults are of a higher quality than the eating habits of adolescents. Nevertheless, both groups consume insufficient amount of fruit and vegetables. On the other hand, they frequently consume red meat, meat products and fried food. Bad eating habits are proven even by the quantity of overweight and obesity occurrence; 20,4 % of interviewed adolescents and even 63,6 % of interviewed adults are of a higher than normal weight. Moreover, one third of adolescents eat in fast-foods. Conclusion: Healthy nutrition and correct eating habits play a key role in the prevention of many diseases. Together with adequate physical activity and no smoking they reduce essentially the risk of occurrence not only of colorectal cancer but also of other cancerous and noncancerous diseases. Článek sleduje problematiku stydlivosti jako emocion álního osobnostního rysu, pro který je typický strach z negativního hodnocení doprováze ný emoční nepohodou. Nejvýznamnější emocí uplatňující se při zaměření pozo rnosti na vlastní selhání a je stud. Stydlivost, která se stane negativní součá stí sebepojetí, působí negativně na životní spokojenost člověka a má rovněž úzký vztah k psychickým poruchám. K posouzení míry stydlivosti lze využít čes ký překlad „Dotazníku stydlivosti“ (2002). Dále sledujeme vztah stydlivosti k rysům pětifaktorového modelu. Výzkumný soubor tvořilo 51 osob se smíšenou úzkostně depresivní poruchou a 50 osob se z ávislostí na alkoholu., Our paper analyses the issues of shyness as emotiona l personality trait for which the fear of negative judgement is typical, accompanied by emo tional discomfort. The most significant emotion when focusing on our own failure s and functioning is shame. The shyness which becomes part of negative self-image has adverse impact on individual’s life satisfaction and also has close relationship t o mental disorders. To assess the degree of shyness the Czech translation of "Questionnaire of shyness" by Lynne Henderson and Philip Zimbardo (2002) may be used. Cíl: Cílem našeho příspěvku bylo zjistit vztah mezi konstrukty kvality života související se zdravím Health Related Quality of Life, používaný ve standardní zkratce HRQoL a subjektivní pohody Subjective well-being, používaný ve standardně zavedené zkratce SWB, u pacientů s chronickým onemocněním, konkrétně bronchiálním astmatem. Zaměřujeme se na diskriminační validitu obou konstruktů. Metodika: Výzkumný soubor tvořilo 316 pacientů, kteří byli vyšetřeni na plicní ambulanci ve Fakultní nemocnici Ostrava. HRQoL byla hodnocena prostřednictvím zkrácené verze Dotazníku kvality života astmatiků (Mini Asthma Quality of Life Questionnaire, Mini AQLQ), SWB prostřednictvím Indexu osobní pohody (Personal Wellbeing Index, PWI). Ke zjištění závislosti mezi vybranými proměnnými byl použit Pearsonův korelační koeficient a lineární regresní analýza. Diskriminační validitu jsme zkoumali prostřednictvím zkoumání rozdílů v HRQoL a SWB z hlediska vybraných klinických proměnných. Rozdíly v HRQoL a SWB byly zjišťovány vícefaktorovou analýzou rozptylu (ANOVA). Výsledky: Korelace mezi HRQoL (operacionalizované prostřednictvím Mini AQLQ) a SWB (operacionalizované prostřednictvím PWI) jsou pozitivní a středně silné. Percepce kontroly astmatu byla identifikovaná jako signifikantní prediktor obou konstruktů, avšak vytvářela vyšší míru variability v rámci HRQoL. Závěr: SWB a HRQoL představují související, avšak odlišné konstrukty. Výsledky podporují chápání HRQoL jako konstruktu založeném na větší míře klinických proměnných než je tomu u SWB., Aim: The goal of the study was to determine the relationship between the health-related quality of life (usually abbreviated as HRQoL) and subjective well-being (usually abbreviated as SWB) of patients with chronic disease, namely bronchial asthma. The focus was on discriminative validity of both constructs. Methods: The research sample consisted of 316 patients examined at the Pulmonary Clinic of the University Hospital Ostrava. HRQoL was assessed by a short version of the Asthma Quality of Life Questionnaire (Mini AQLQ) and SWB was assessed by the Personal Wellbeing Index (PWI). Pearson’s correlation coefficient and linear regression analysis were used for evaluation of dependencies between selected variables. Discriminative validity was explored by examination of differences between HRQoL and SWB relevant to selected clinical variables. Differences in HRQoL and SWB were detected by multi-factor analysis of variance (ANOVA). Results: The correlations between HRQoL (assessed by the Mini AQLQ) and SWB (assessed by the PWI) are positive and moderate. Perception of asthma control was identified as a significant predictor of both constructs. However, it produced greater variance in the context of HRQoL. Conclusion: SWB and HRQoL are related but distinct constructs. Cíl studie: Vyhodnotit přínos interleukinu-6 u těžké akutní pankreatitidy pro odlišení sterilní nekrózy (SN) a nekrózy infikované (IN). Typ studie: Retrospektivní deskriptivní observační studie série případů. Metody: Ve vlastní práci bylo zahrnuto 59 pacientů hospitalizovaných s těžkou akutní pankreatitidou (Atlantská klasifikace 1992) na chirurgickém oddělení Nemocnice Písek v průběhu sedmi let. Pravidelně byly vyšetřovány C-reaktivní protein (CRP) a interleukin-6 (IL-6), přičemž dostatečná data byla u 42 pacientů: 14 s IN (průkaz mikrobiologicky či dle CT), 28 se SN. Statistické vyhodnocení pomocí Fisherova testu a chí-kvadrát testu. Výsledky: Byla nalezena statisticky významná diskriminační schopnost denních průměrů IL-6 5. a 6., resp. 7. a 8. den hospitalizace mezi IN a SN (p=0,0014 pro den 5 a 6, resp. p=0,0009 pro den 7 a 8). Rozvoj IN reálně nastával nejčastěji mezi 5. a 9. dnem (mikrobiologický průkaz agens či nepochybné CT známky infekce). CRP tuto schopnost diskriminace neprokázalo, jeho hladiny byly od 2. do 8. dne setrvale výrazně zvýšeny. Charakteristický byl průběh koncentrací IL-6, kdy vstupně byla přítomna výrazná elevace nezávislá na pozdějším rozvoji IN a již od druhého dne hospitalizace došlo k výraznému poklesu. K dalšímu vzestupu IL-6 došlo pouze u pacientů s rozvojem IN. Pro cut-off (diskriminace IN a SN) IL-6 (platný od 5. dne hospitalizace) 100 ng/l bylo p<0,005, pro 150 ng/l p<0,01. Závěr: Interleukin-6 v naší retrospektivní studii prokázal statisticky významnou schopnost diskriminovat infikovanou a sterilní nekrózu u těžké akutní pankreatitidy. Překročení cut-off IL-6 od 5. dne hospitalizace svědčilo pro přítomnost infikované nekrózy. Cut-off 100 ng/l diskriminoval obdobně (p<0,005, senzitivita 100 %, specificita 92 %) jako cut-off 150 ng/l (p<0,01, senzitivita 87 %, specificita 100 %)., Objective: Our retrospective study aimed to evaluate the contribution of interleukin-6 in severe acute pancreatitis. Mortality of severe acute pancreatitis ranges from 10-20% (for sterile necrosis, SN) to 20-85% (infected necrosis, IN). Resolution of SN and IN is clinically very important, but often difficult and therefore they are still searching for new laboratory markers for their differentiation. Design: Retrospective descriptive observational study of cases series. Methods: 59 patients hospitalized with severe acute pancreatitis (Atlanta classification 1992) in the surgery department of Hospital Písek in the 2000-2006 were included. Regularly they were investigated by C-reactive protein (CRP) and interleukin- 6 (IL-6), sufficient data was in 42 patients: 14 with IN (identified microbiologically or according to computed tomography, CT), 28 with SN. Statistical analysis was performed using Fisher‘s test and chi-square test. Results: There was found statistically significant discriminatory ability of daily averages of IL-6 5th and 6th + 7th and 8th day of hospitalization between the IN and SN (p = 0.0014 for day 5 and 6, respectively. p = 0.0009 for day 7 and 8). Developing IN realistically set in the most between 5th and 9th day (needle biopsy with identification of microbiological agents or CT signs of infection). The ability of CRP to discriminate SN and IN was not found, the levels were between 2nd to 8th days increased steadily. The run of the concentrations of IL-6 was characteristic, where at the admission there was present significant elevation irrespective of subsequent development of IN and from the second day of hospitalization it decreased significantly. Further increase in IL-6 was present only in patients with the development of IN. For the cut-off (discrimination of SN and IN) for IL-6 (valid from 5th of hospitalization) 100 ng/L was P<0.005, for 150 ng/L P<0.01. Conclusion: Interleukin-6 in our retrospective study showed a statistically significant ability to discriminate between infected and sterile necrosis in severe acute pancreatitis. The Purpose of the study: define the indications and contraindications to, combined operation in locally invasive tumor of the rectum. The Material and methods of the study: we have analysed results combined operation in cancer of the rectum with germination in genital organs, performed in department of coloproctology in National Oncological Scientific centre during 20052009 years. Under observation were 118 women at age from 21 to 68 years. This before 45 years 26 patients, from 46 to 59 years 54 patients, 60 and above years. The Results and their discussion: Postoperative complications suppurativeinflammatory character appeared beside 36 patients (30,5%), most of all after abdomeno perineal extirpation of rectum (35,5%) and abdominoanal resection of the rectum (33,3%). In lesser extend after front resection of the rectum and after operation Hartman (28,5% and 22,7%). The General lethality has formed 3,4%, have died after combined operation 4 patients from 118 operated patients. The Conclusion: thereby, brought data evident that such important factors, as frequency of the origin relapse (28%), 5year probability of survival (37,1%), under combined interference and operation of the standard volume in the cancer of the rectum practically the same., H. The Material of the study has formed 78 sick, found on stationary treatment in Republican Scientific Centre Coloproktologii since 1992 on 2010. As it is seen, from table, from 78 sick mans was 58(74,3%), womans 20(25,7%). 19(24,3), sick were at age from 15 before 20 years, 49(60,2%) at age from 21 before 40 years and 13 (16,6%) sick from 41 before 60 years. The Main complaint sick at arrival were a stubborn constipations, which noted beside 70 (89,7%) sick, including absence of the independent chair existed beside 55(70,5%), but beside 54(69,2%) sick were noted periodic stomachache, growing on measure of the absence of the chair. The Ballooned belly existed beside all 78 (100%) sick moreover beside 20(25,6%) of them flatulence was constant. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation. However, the other factors seem to play a role, since the relationship is not straightforward within entire FR range., and Vecka M., Tvrzická E., Votruba M.
