Článek představuje projekt UNICEF Škola přátelská dětem; seznamuje ve stručnosti s manuálem projektu. Projekt byl motivován skutečností, že denně navštěvuje základní a střední školy více než dvě miliardy dětí a školní docházka není pro všechny pozitivní zkušeností (např. pitná voda, školní nábytek, trestání, ponižování dětí atd.). Cílem projektu je vytvořit přátelské prostředí pro všechny děti. Klíčové principy projektu jsou: inkluzivní vzdělávání, demokratická participace dětí, podporující prostředí s cílem rozvoje osobnosti dětí a zajištění kvalitního vzdělávání. Jádrem projektu jsou práva dětí. Model Škola přátelská dětem zahrnuje následující oblasti: 1. umístění, exteriér a dispozice interiéru, 2. škola a komunita, 3. škola jako ochraňující prostředí a 4. vyučování. Záměrem projektu je, aby všechny školy mohly být identifikované jako školy přátelské dětem. To předpokládá, že si každá škola vypracuje svůj vlastní projekt a jeho realizaci ověří metodou dotazníkového šetření. Škola spolupracuje při realizaci projektu s orgány veřejného zdraví., The article presents the UNICEF project Child Friendly Schools; it acquaints us with the manual of projects in brief. The project was motivated by the reality that basic and secondary schools are visited by more then two billions children daily, and schooling is not for every child a positive experience (for example: drinking water, school furniture, punishment and humiliation and others). The aim of this project is the creation of a friendly school environment for all children. The key principles of the project are: inclusive education, democratic participation of children, a supporting environment with the aim of developing the child's personality and ensuzing an excellent education. The core of the project are the rights of children. The model of Child Friendly Schools consists of four areas: 1) location, design and construction, 2) the school and the community, 3) the school as a protective environment, and 4) learners, teachers and school management. The purpose of the project is for all schools to be identified us child friendly schools. It anticipates that each school elaborates its own project and its realization is verified by a questionnaire survey. Karcinoid patří mezi neuroendokrinní tumory, které mohou produkovat endokrinně aktivní látky. Představuje 1–2 % plicních tumorů. Dle svých histopatologických charakteristik se dělí na typický a atypický karcinoid, od čehož odvisí i prognóza (13, 24). Typický karcinoid (TC) má příznivou prognózu, je často zjištěn náhodně, pětileté přežití je přes 90 %. Atypický karcinoid (AC) se vyznačuje agresivnějším chováním, časnějším metastazováním i nižším dlouhodobým pětiletým přežitím (40–60 %) (5, 13). Sledovali jsme soubor 52 pacientů s histologicky potvrzenou diagnózou karcinoidu, kteří se na naší klinice léčili v období 2002–2013. Typický karcinoid byl diagnostikován u 35 pacientů (67 %), u 17 pacientů (33 %) se jednalo o karcinoid atypický. Sledování potvrdilo v literatuře uváděné četnosti výskytu a prognózu pacientů, analyzuje též možnosti a efektivitu endobronchiální léčby a výsledky léčby obou typů nádoru. Zajímavým zjištěním je vysoká incidence duplicitních (i triplicitních) malignit u pacientů s atypickým karcinoidem (25 % nemocných)., Carcinoid is a neuroendocrine tumour that can produce endocrine active substances. It represents 1-2 % of pulmonary tumours. Based on histopathological characteristics, it is divided into typical and atypical carcinoid, which also affects the prognosis. {11,18} Typical carcinoid (TC) has a favourable prognosis, is often found incidentally, and the five-year survival rate is over 90 %. Atypical carcinoid (AC) exhibits a more aggressive behaviour, more frequent metastasizing, and a lower five-year survival rate (40-60 %). {5,11} We followed up a group of 52 patients with a histologically confirmed diagnosis of carcinoid who were treated at our clinic in the years 2002 to 2013. Typical carcinoid was diagnosed in 35 patients (67 %) and 17 patients (33 %) had atypical carcinoid. The follow-up confirmed the incidence rates and prognosis of patients reported in the literature, while analyzing the options and efficacy of endobronchial treatment and the results of treatments for both types of tumours. Jedná se o prospektivní pilotní studii zaměřenou na vyšetřování vývoje tvaru a statiky páteře u dětí pomocí neinvazivní somatografické metody systémem DTP při vyšetřování kvality posturální funkce. Sledovaný soubor tvořilo 47 dětí v mladším školním věku a byly vyšetřeny dvakrát (v 1. a v 5. třídě ZŠ). Během sledování došlo ke statisticky významnému prohloubení krční lordózy v celém souboru, tj. u dívek i chlapců, k prohloubení bederní lordózy došlo pouze u chlapců. Celkově ke zlepšení držení těla došlo u 15 %, k celkovému zhoršení držení těla u 53 % a beze změny postury zůstala 4 % dětí vyšetřovaného souboru. Z našich šetření vyplývá 1) nestabilita posturální funkce v tomto sledovaném období, která naznačuje možnost efektivity preventivních opatření v podobě optimální zátěže; 2) možnost upřesnění stávajícího somatoskopického vyšetřování použitím neinvazivního přístroje., The aim of the prospective pilot study was the investigation of the development of postural function (spinal shape and statical properties of the spine) in children by the non-invasive DTP system. The sample consisted of 47 children of primary school age who were examined twice – in the 1st and 5th grade of elementary school. Exacerbation of cervical lordosis was statistically documented during follow-ups in both genders, while worsening of lumbar lordosis was proven only in boys. Overall, posture was improved in 15%, deteriorated in 53% and remained unchanged in 4% of examined children. Our study reveals: i) postural instability in this reporting period, which suggests the possible effectiveness of preventive measures in the form of optimal load; ii) the possibility of clarifying somatoscopic investigation using non-invasive apparatus., Jana Langmajerová, Marta Bursová, Jana Dvořáková, Dana Müllerová, and Literatura 13
Organization and public financing of health care is task and competence of the member states. However, economic integration cannot spare it entirely. Financing of urgent treatment of tourists is based on coordination of reimbursement among competent institutions. The Court of Justice established right of patients for public reimbursement of treatment intentionally found in other member states with domestic rates with broad interpretation of basic economic freedoms, albeit measures for cost-containment justify restrictions. The Directive on the application of patients’ rights in cross-border healthcare brings clarification. The Czech Republic has implemented the directive benevolently within short delay. There is no fear of outflow of public money to other member states due to economic, social and linguistic reasons and for absent litigation of patients as clients of public health insurance funds. Opportunity to require prior authorization with reimbursement was thus not used. Increasing complexity of public financing of Czech healthcare is revealed. It is unclear whether rates of reimbursement are fair and regulated prices are feasible.
