Cíl studie: zhodnocení efektivity cerkláže dle McDonalda ve Fakultní nemocnici Ostrava v období 1/2007 – 3/2014. Typ studie: retrospektivní studie Název a sídlo pracoviště: Porodnicko–gynekologická klinika, Fakultní nemocnice Ostrava Metodika: retrospektivní analýza provedených cerkláží ve Fakultní nemocnici Ostrava a rozbor výsledků takto ošetřených gravidit. Kritériem úspěšnosti byl stanoven porod po ukončeném 34. týdnu gravidity. Závěr: ve sledovaném období bylo indikováno devět cerkláží – 7 terapeutických a 2 záchranné. Výkony byly provedeny mezi 18. a 25. týdnem gravidity. Tři pacientky již měly v minulosti cerkláž provedenu. 57 % takto ošetřených těhotných porodilo po ukončeném 34. týdnu gravidity. Pouze v jednom případě (11,1 %) byla cerkláž neúspěšná a těhotenství skončilo potratem. Průměrná doba mezi provedením cerkláže a porodem činila 69 dnů., Objective: To evaluate the efficacy of cerclage according to McDonald at the University Hospital Ostrava in the period 1/2007 - 3/2014. Design: A retrospective study Obstetrics and Gynecology: Obstetrics - Gynecology, University Hospital Ostrava Methods: A retrospective analysis of cerclage performed at the University Hospital Ostrava and analysis of the results thus treated pregnancies. The criterion for success was determined after childbirth completed 34 weeks of preg - nancy. Conclusion: In the reporting period, nine indicated cerclage - 7 of therapeutic and 2 emergency. The procedures were performed between the 18th and 25th week of pregnancy. Three patients have had in the past cerclage performed. 57% of the treated pregnant women gave birth after 34 completed weeks of gestation. Only in one case (11.1 %) cerclage was unsuccessful and the pregnancy ended in a miscarriage. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Úkolem diagnostické terminologie v jakékoli morfologické metodě je poskytnutí jednoznačně interpretovatelné informace klinickému specialistovi tak, aby mohl zvolit adekvátní terapeutický postup. Cytologická klasifikace dysplastických změn na děložním hrdle se od původní Papanicolauovy pětistupňové postupně zjednodušila na dvoustupňovou. V histopatologické rovině hodnocení se rychle prosadila fúze kategorií CIN III a CIS, další vývoj je však v porovnání s cytopatologickým hodnocením pomalejší. Navrhovaná dvoustupňová klasifikace histopatologických lézí děložního hrdla nese v sobě potenciál zpřesněné komunikace klinika a patologa ve vyhodnocování cervikálních lézí a jejich následném ošetřování. V porovnání k předchozím je plně převoditelná. Zachovává si ovšem i předchozí omezení ve vyhodnocení základního histopatologického nálezu, zejména závislost na primárně reprezentativním vzorku. Hlubší porozumění procesům karcinogeneze děložního hrdla však rozšiřuje spektrum zpřesňujících prognostických markerů a možnosti individualizovaných léčebných postupů., The function of diagnostic terminology in any morphology method is to provide information to the clinical specialist that is interpreted unequivocally and which enables him to choose the appropriate treatment. The former Papanicolaou five-tiered cytological classification of dysplastic changes on the uterine cervix has been simplified to a two-tiered approach. In the histopathological evaluation, fusion of the categories CIN III and CIS was rather smooth. Further progress toward simplification of the histopathology terminology is proceeding more slowly. The proposed two-tiered classification of the histopathological lesions of the cervix has the potential to create a more precise communication between the clinician and the pathologist in the evaluation and subsequently in the treatment of the cervical lesions. In comparison to the previous classifications, it is fully convertible. It preserves the previous limits in the evaluation of the histopathological finding, mainly the dependence on the primary representativeness of the specimen. Deeper understanding of the cervical cancerogenesis will be achieved with the increasing spectrum of prognostication markers. Příspěvek seznamuje čtenáře s hlavními závěry výzkumného projektu, který byl realizován v rámci diplomové práce. Autorka čtenářům představuje základní teoretické modely citové vazby v romantickém vztahu. Kvantitativní výzkum byl realizován na vzorku 258 respondentů z České republiky. Výsledky potvrzují předchozí zahraniční studie, podle kterých jsou jedinci s jistou citovou vazbou spokojenější než jedinci s nejistou citovou vazbou. Faktory citové vazby se ukázaly jako lepší prediktory spokojenosti ve vztahu než věk, pohlaví nebo druh vztahu respondenta (partnerský vztah / manželství)., The article informs the readers about main conclusions of the research, what was realized in pursuance of the thesis. The author acquaints the readers with the main theoretical models of attachment in romantic relationship. Quantitative research was conducted on a sample of 258 respondents from Czech Republic. The results confirm the former foreign researches of attachment, according to which more securely attached individuals are more satisfied in a relationship than those less securely attached. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions.
