Psychosociální faktory významně participují na patogenezi komplexních chorob. Jejich působení na mnoha úrovních organismu koresponduje s bio-psycho-sociálním konceptem zdraví a nemoci. Mozek, chování a imunita jsou úzce propojeny a reagují na environmentální kontext. Účinky chronického psychosociálního stresu popisuje model alostatické zátěže, jenž ozřejmuje dynamičnost nastavení biologických procesů a význam mozku pro zpětnovazebnou regulaci v adaptaci na prostředí. Osobnost jedince, jeho kognitivní a emoční charakteristiky, kvalita sociálních vztahů a socioekonomické faktory modifikují stresovou zátěž a mají přímý i nepřímý vliv na zdraví. Řada účinků psychosociálních faktorů je zprostředkována epigeneticky; v období raného vývoje tento mechanismus formuje vulnerabilitu vůči rozvoji patologií souvisejících se stresem. Psychické, genetické, behaviorální a environmentální vlivy spolu průběžně interagují. Nepřímé účinky psychosociálních faktorů spočívají v modifikaci chování a životního stylu. Toto působení je obousměrné, neboť životní styl zpětně ovlivňuje psychické a fyzické zdraví. The effects of chronic psychosocial stress are described by the allostatic load model which highlights dynamic adjustment of biological processes and the importance of the brain for feedback regulation in adapting to the environment. The personality of an individual, his cognitive and emotional characteristics, quality of social relationships and socioeconomic factors modify stress load and have a direct and indirect impact on health. Many of the effects of psychosocial factors are mediated epigenetically. In the early development epigenetic mechanisms form the vulnerability to stress-related pathologies. Psychological, genetic, behavioural and environmental factors continuously interact. The indirect effects of psychosocial factors are contained in the modification of behaviour and lifestyle. These effects are bidirectional, because lifestyle in turn has an impact on mental and physical health. Cíl: Cílem výzkumu bylo zjistit vztah reminiscenčních aktivit (RA) a depresivity u seniorů žijících v institucionální péči. Sledována byla míra depresivity u seniorů starších 65 let. Byl sledován rozdíl mezi skupinou experimentální a kontrolní po 50 - ti seniorech. Metodika: Pro sběr dat a vymezení depresivity respondentů byl použit screeningový nástroj Škála deprese pro geriatrické pacienty (GDS). Studiem zdravotní dokumentace byla sbírána data, jako je věk, výsledky kognitivního testu MMSE a data z oblasti psychofarmakologické medikace, jelikož je to významný faktor ovlivňující míru depresivity. Hodnocení depresivity proběhlo ve dvou měřeních. U experimentální skupiny proběhla měření před a po absolvování strukturovaných RA. U kontrolní skupiny byla měření provedena ve stejném časovém období, jako ve skupině experimentální, ale již bez RA. Výsledky: Prevalence depresivity v celém souboru respondentů byla 43 %. Depresivita v experimentální skupině byla prokázána u 20 % respondentů. Ve stejném časovém období u skupiny kontrolní byla zjištěna depresivita u 23 % respondentů. Po absolvování RA klesla depresivita u experimentální skupiny o 4 %. Ve zkoumaném souboru bylo statisticky potvrzeno, že RA snižují míru depresivity u seniorů v institucionální péči. Přítomnost depresivní symptomatologie po absolvování RA u experimentální skupiny byla nižší než u skupiny kontrolní. Závěr: Deprese je vážným a život ohrožujícím onemocněním, které se podílí na snížení kvality života seniorů a jejich rodin. Správná diagnostika a léčba deprese vede ke zlepšení kvality života seniorů a jejich rodin. RA jsou významnou psychosociální nefarmakologickou intervencí, která může výrazně přispět ke zlepšení kvality života., Aim: The aim of this research was to determine the relationship between reminiscence activities (RAs) and depression in seniors living in institutional care. The rate of depression was studied in seniors over 65 years of age. Differences between experimental and control groups, each comprising 50 individuals, were assessed. Methods: The Geriatric Depression Scale was used for data collection and identification of depression in the participants. Medical records were used to ascertain their age, Mini Mental State Examination results and psychopharmacological drug use, a significant factor affecting the rate of depression. Depression was evaluated by two measurements. In the experimental group, the measurements were carried out before and after completion of structured RAs. In the control group, the measurements were carried out at the same times but no RAs were provided. Results: The overall prevalence of depression in all participants was 43%. In the experimental and control groups, the rates were 20% and 23%, respectively. After completion of RAs, the rate decreased by 4% in the experimental group. RAs were confirmed to lower the rates of depression in seniors in institutional care. Depressive symptoms were less frequent after RAs in the experimental group than in the controls. Conclusion: Depression is a serious and lifethreatening disease contributing to impaired quality of life of both seniors and their families. Correct diagnosis and treatment of depression lead to improved quality of life of seniors and their families.
