Léčba metastazujícího kolorektálního karcinomu v posledních 10 letech významně pokročila od jednotně aplikované chirurgické a chemoterapeutické léčby přes zavedení biologické léčby, testování biomarkerů, a tím k individualizaci péče. Ten zahrnuje nejen onkology a chirurgy, ale také radiology, patology, gastroenterology či další obory podílející se na řešení průvodních chorob a vedlejších účinků léčby., The treatment of metastatic colorectal cancer has made significant progress in the last ten years, from a uniformly applied surgical and chemotherapeutic management to the introduction of biological therapy to biomarker testing and thus individualization of care. There have also been considerable advances in the resection techniques and a major shift towards modern methods of converting a re-resectable finding of liver metastases to a resectable one. The treatment of a patient with metastatic colorectal cancer (mCRC) is no longer only in the hands of an experienced oncologist, but rather is an individualized strategy decided upon and reappraised in the course of the treatment by an interdisciplinary team. It includes not only oncologists and surgeons, but also radiologists, pathologists, gastroenterologists or other specialists involved in the management of concomitant diseases and side effects of treatment., Zuzana Zafarová, and Literatura
Další zlepšení osudu těchto nemocných je podmíněno brzkým přechodem na individualizovaný přístup a personalizované pojetí léčby., Radical resection of metastases is currently the most effective treatment modality in patients with liver metastases of colorectal cancer. Conversion therapy is effective for the patients, whose metastases are not primarily resecable. With this therapy is achieved a downsizing in 25%, followed by performing liver resection. Large hepatic resection and biochemotherapy is suitable to perform at the specialized department on which is guaranteed a multidisciplinary approach to treatment. This approach not only guarantees low operating risk and use adequate (neo)adjuvant therapy, but also the complexity of reaching more than 50 % five year survival with good quality of life. Further improvements in the lives of patients is subject to an early transition to an individualized approach and personalized approach to treatment., Miroslav Ryska, and Literatura 21
