Imunoterapie na základě výsledků klinických studií z posledního období prokázala, že má své místo v léčbě karcinomu prostaty. Biosyntéza extragonadálních androgenů může přispět k progresi kastračně-rezistentního karcinomu prostaty, proto se v jeho léčbě uplatní inhibitory androgen-regulačních enzymů., Based on the results of the TAX 327 study, docetaxel has become the standard therapy in the first-line treatment of hormone-refractory prostate cancer. Standardní léčba pokročilého maligního melanomu se v posledních letech zásadně změnila. Používaná chemoterapie a imunoterapie před rokem 2011 nebyla schopna významně prodloužit život našich pacientů. K pokroku dochází až díky cílené léčbě (BRAF a MEK inhibitory) a moderní imunoterapii (protilátky proti molekulám CTLA-4 a potenciálně PD-1/PD-L1). Kombinace cílených preparátů jsou intenzivně studovány, úkolem je oddálit nebo zabránit vývoji lékové rezistence a progrese onemocnění. Potřebujeme další molekulární cíle, zvláště u pacientů bez mutace BRAF. Nové znalosti týkající se optimálních lékových kombinací a sekvencí jsou důležité k překonání dnešních limitů léčby., The standard treatment of advanced malignant melanoma was radically changed. The available chemotherapy and immunotherapy have not been able to significantly prolong survival of our patients before 2011. The progress has been made with targeted therapy (BRAF and MEK inhibitors) and modern immunotherapy (antibody against CTLA-4 and potentially PD-1/PD-L1). Combinations of targeted drugs are being intensively studied to delay or prevent of resistance and tumor progression. New molecular targets are needed, particularly for patients without BRAF mutations. Maligní melanom je nejagresivnější kožní nádor. Incidence tohoto onemocnění roste rychleji než u jiných malignit a stále více postihuje mladé lidi. Standardní léčba založená na chemoterapii nebo biochemoterapii nedokázala prodloužit přežití. Pokroky v oblasti imunologie a molekulární biologie u melanomu umožnily objevení nových potenciálních cílů a pomohly tak k vývoji nových léků. Ipilimumab (anti-CTLA-4 protilátka) a vemurafenib (selektivní BRAF inhibitor) dokázaly v randomizovaných studiích ve srovnání s chemoterapií prodloužit celkové přežití. Tyto léky se brzy stanou novým standardem v léčbě pacientů s pokročilým melanomem. K dalším velmi nadějným preparátům z oblasti cílené léčby patří dabrafenib (selektivní BRAF inhibitor) a trametinib (MEK inhibitor)., Malignant melanoma is the most aggressive type of skin cancer. The recent advances in immunology and molecular biology of melanoma have discovered several potencial therapeutic targets and helped to develop new promising drugs. Ipilimumab (anti-CTLA-4 antibody) and vemurafenib (the selective BRAF inhibitor) have demonstrated improved overall survival compared with chemotherapy in randomized trials.
