Backround: Worldwide, approximately 1 billion people, from which 200250 million are women, smoke. Based on the Estonian Medical Birth Register data, in 2010, 7,5% of pregnant women were smokers, and in 2012, 7% of pregnant women were smoking during their pregnancy. Thus, smoking rates among pregnant women fell only 0,5%. The objective of the research was to find out the views of pregnant women who were smoking during pregnancy of the effect of smoking on the health of them and their babies, and experiences in quitting smoking, changing indicators, e.g. smoking and socioeconomical status (initial exposure to smoking, employment, longterm partnerships etc.) in 20092013. Methods: This research is a phenomenological study, which was being carried out from October 2009 until January 2013. Data collection methods were semistructured interviews with 45 pregnant women smoking during their pregnancy and being registered for antenatal care in three health care institutions of Republic of Estonia. An inductive approach for qualitative analysis was used. Results: Most women smoking during pregnancy started smoking in their teens, their parents smoked, they were under 30 years old housewives or unemployed. On an average of 1120 cigarettes were consumed each day, the pregnancy of the research group did not influence frequency and tobacco intake. The participants in the research group reported that tobacco consumption helped them relax, gave them a chance to spend time in a good company, and it was not regarded as a bad habit but a social addiction, being caused by availability of tobacco products. Although most of them had an opinion about harmful effects of smoking, e.g. a cough, asthma and rapid fatigue, on the health of them and less on their expected babies, they continued smoking. Conclusions: The smoking and socioeconomical status of women smoking during pregnancy in 20092013 has not changed, health awareness of them is low and has not improved. Support groups with close relatives are needed to motivate cessation. In the first place, prevention needs a good example, purposeful cooperation, which starts in daily life context at homes, in educational institutions and in the community. Limiting the availability of tobacco products, postponing and preventing an initial exposure to smoking are thought to be of great importance. Současný stav: Výzkumné studie prokázaly, že nejenom rozdíly v pohlaví mají vliv na zdravotní stav, ale též etnická příslušnost a společenská třída. Imigranti jsou zranitelnou skupinou, která je ve vyšším riziku řady onemocnění ve srovnání s majoritou. Sociální determinanty zdraví jsou významné při hledání příčin sociálních nerovností ve zdraví (Bates et al., 2009; Lindström et al., 2001). Cíl: Hlavním záměrem výzkumu bylo zmapování využití zdravotní péče imigranty. Metodika: Kvantitativní metoda byla realizována pomocí dotazování. Výzkumné šetření probíhalo v období leden až červenec 2012. Bylo kontaktováno 236 imigrantů ve dvou regionech (Jihočeský kraj a hlavní město Praha). Byli osloveni legálně usazení imigranti, ve věkové kategorii 18-65 let, vietnamské, mongolské a ukrajinské národnosti. Výsledky: Čerpání zdravotní péče imigranty bylo ovlivněno typem zdravotního pojištění, délkou pobytu, pohlavím a znalostí českého jazyka. Imigranti, kteří mohli vstoupit do systému veřejného zdravotního pojištění, čerpali zdravotní péči častěji než imigranti s komerčním zdravotním pojištěním. Závěry: Mezi opatření, vedoucí k odstranění sociálních nerovností a zlepšení situace imigrantů v ČR patří zvyšování jazykových znalostí imigrantů a umožnění všem legálně usazeným imigrantům přístup k veřejnému zdravotnímu pojištění., Background: Research shows that health state is influenced not only by gender but also by class and ethnic membership. Immigrants form a vulnerable group in comparison to the majority, more susceptible to a wide range of diseases. Social determinants of health play an important role in search for reasons for social inequality in health (Bates et al., 2009; Limdström et al., 2001). Goal: The main aim of the research was to map use of healthcare by immigrants. Methodology: The quantitative method used were interviews. The survey was carried out in the period from January to July 2012. 246 immigrants were interviewed from two regions (South Bohemian Region and the capital Prague). They were legally settled immigrants in the age group 18-65, of Vietnamese, Mongolian and Ukrainian nationality. Results: The factors influencing use of healthcare by immigrants are the type of health insurance, length of residence, sex and knowledge of Czech language. Cíl: Cílem kvalitativního šetření bylo zjistit, zda sociální nerovnost mezi rodičkami ovlivňuje přístup zdravotnického personálu k rodící ţeně a ţeně po porodu hospitalizované na oddělení šestinedělí a porovnat názory na tento přístup mezi zdravotnickým personálem a těmito ţenami. Metodika: Výzkumný soubor tvořilo 11 porodních asistentek a 12 ţen po porodu, které byly v době provádění výzkumu hospitalizované na oddělení šestinedělí. Výběr respondentek byl záměrný. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden polostrukturovaným, individuálním rozhovorem a metodou obsahové analýzy. Výsledky: Z analýzy rozhovorů vyplynulo, ţe věk a výše vzdělání rodičky mají vliv na edukaci a spolupráci rodičky s porodní asistentkou, u některých porodních asistentek ovlivňují také samotný přístup porodní asistentky k rodičce. Co však také ovlivňuje přístup některých porodních asistentek, a to spíše v negativním slova smyslu, je příslušnost rodičky k minoritní skupině. Závěr: Sociální nerovnost mezi rodičkami má vliv nejen na přístup porodních asistentek k rodící ţeně a ţeně po porodu hospitalizované na oddělení šestinedělí, ale ovlivňuje také jiţ samotnou přípravu rodičky na porod, moţnost vyuţít přítomnost doprovodu u porodu či moţnost vyuţít zpoplatněných forem analgetizace v průběhu porodu., Aim: The aims of the qualitative research were to determine whether social inequality among women in labor affects the approach of health professionals to laboring women and those staying on a postnatal ward and to compare opinions on this approach between health professionals and these women. Methods: The group, selected by purposive sampling, comprised 11 midwives and 12 women staying on a postnatal ward while the research was conducted. Data for the qualitative research were collected using semi-structured individual interviews and content analysis. Results: The interview analysis showed that the age and level of education of women in labor have an impact on their education by and cooperation with midwives. And in some midwives, these also affect their attitude to laboring women. But what also influences the approach of some midwives, mostly in a negative sense, is laboring women’s belonging to a minority group. Conclusion: Social inequality among women in labor not only has an impact on midwives’ approach to laboring women and those staying on a postnatal ward but also affects childbirth preparation and possibility to benefit from the presence of birth support people or pain management options not covered by insurance., and Veronika Sopuchová, Radka Bužgová
