Laryngeal electromyography is used in the evaluation of vocal cord paralysis to confirm the diagnosis, to guide the diagnostic work-up for etiology, to provide prognostic information and to help choose the correct treatment for the patient. Cardiovocal syndrome is characterised by vocal cord paralysis due to a cardiovascular disease. A wide spectrum of conditions can result in this syndrome. Here we present a case of cardiovocal syndrome in association with primary pulmonary hypertension. Capsular bag distension syndrome is a very rare complication of cataract surgery. Most cases occur after uneventful phacoemulsification with continuous curvilinear capsulorhexis and implantation of an intraocular lens in the capsular bag. The entity presents with reduction of visual acuity and myopic shift in the early-to-late postoperative period. Characteristic findings include the distension of the capsular bag due to the accumulation of milky material and the forward displacement of the intraocular lens. We present two cases with an unusually delayed presentation of 6 and 8 years respectively following phacoemulsification, and describe their successful management with Nd:YAG laser posterior capsulotomy. and P. Východiska: Americká společnost klinické onkologie (ASCO) vytvořila panel expertů a provedla systematický přehled dostupné literatury. Panel vytvořil konsenzuální doporučení expertů. Cílem naší práce je ve srovnání s tímto doporučením zhodnotit použitý léčebný postup. Klinická kazuistika: Uvádíme kazuistiku 32leté pacientky s karcinomem prsu, hormonálně dependentním, HER2 pozitivním, u které v průběhu adjuvantní léčby došlo ke generalizaci onemocnění do CNS (centrální nervový systém). Popsán průběh onemocnění, během kterého byly kombinovány různé modality lokální i systémové léčby, které vedly k přežití v délce 4 let od diseminace onemocnění. Léčebný postup byl ve shodě s ASCO doporučením, kombinoval při opakovaných recidivách uvedené možnosti lokální terapie, stereotaktickou radiochirurgii (SRS), ozáření celého krania (WBRT) a chirurgický výkon. U prezentované pacientky jsme použili systémovou léčbu, která byla v dané době dostupná, tedy trastuzumab, lapatinib, taxany, kapecitabin, vinorelbin a hormonální léčbu. V současnosti zavedení nových preparátů výrazně rozšířilo možnosti systémové léčby. Závěr: Provedený léčebný postup kombinace SRS a WBRT je ve shodě s doporučením pro danou velikost solitární metastázy do mozku. V oblasti systémové léčby se současné možnosti značně rozšířily díky zavedení nových preparátů HER2 terapie a provedená terapie odpovídá době její aplikace. Doporučení, která v roce 2014 byla prezentována v Journal of Clinical Oncology (JCO) považujeme za vysoce přínosná., Background: The American Society of Clinical Oncology (ASCO) has developed a panel of experts which conducted a systematic review of the available literature. Panel of experts developed consensus recommendations. The aim of our work is to compare guidelines and evaluate the used therapeutic procedure. Case: We present a case of 32 year old female with breast cancer, hormone dependent, HER2 positive. During adjuvant therapy occured metastatic lesion in the CNS (central nervous system). Course of the disease is presented, during which the various modalities were combined, local and systemic treatments were used, which led to the survival period of 4 years of disease from the time of dissemination. The treatment procedures were in accordance with ASCO recommendations, repeated relapses were treated with a variety of methods of local therapy (SRS WBRT and surgery). Currently the introduction of new drugs substantially expanded options for systemic therapy. Conclusions: Used treatment with a combination of SRS and WBRT is in line with the recommendation for a given size of solitary metastasis to the brain. In the area of systemic therapy the current options greatly expanded by the introduction of new drugs. Used antiHER2 therapy corresponds to the time of its application. Pudendální neuralgie je špatně snesitelný bolestivý stav způsobený postižením (většinou kompresí) pudendálního nervu. Klinicky je charakterizován progresivní bolestí v pánvi, která se horší sezením, bývá spojena se sexuální dysfunkcí, potížemi s močením a defekací. Ke stanovení jisté diagnózy jsou používána nanteská kritéria. V této práci prezentujeme soubor šesti pacientů, kteří byli od roku 2009 léčeni pro toto onemocnění ve FN Brno, a popisujeme současné terapeutické možnosti a hodnotíme jejich efekt., Pudendal neuralgia is a very painful condition caused by compression or entrapment of the pudendal nerve. Clinical characteristics include progressive pelvic pain, worsened by sitting, sexual dysfunction and difficulty with urination and defecation. Nantes criteria are used to confirm pudendal neuralgia. We present a group of six patients treated for pudendal neuralgie at the University Hospital in Brno since 2009 and discuss the options for the treatment of this syndrome. Je prezentován případ 29leté pacientky, u které bylo zjištěno vzácné nádorové onemocnění – primární leiomyosarkom ledviny. Diagnóza byla stanovena u pacientky ve 32. týdnu gravidity pomocí ultrazvuku, nádor se projevoval výraznou bolestivostí levého boku, kvůli které byla pacientka odeslána na naši ambulanci. Při vyšetření byl zjištěn objemný tumor levé ledviny, který byl po porodu, indukovaném v dřívějším termínu, potvrzen i při vyšetření počítačovou tomografií (CT). Nádor byl úspěšně odoperován, histologicky byl verifikován nález leiomyosarkomu. Nyní je pacientka 19 měsíců od operačního výkonu bez nálezu progrese onemocnění či metastáz., We present a case of 29-year-old patient with diagnosis of a rare cancer: primary leiomyosarcoma of the kidney. The diagnosis was determined in the patient in 32th week of pregnancy using ultrasound. Tumour manifested with severe pain on left side, for which the patient was referred to our clinic. During examination an extensive tumour of the left kidney was detected, which was confirmed by CT scan after an early delivery. The tumour was successfully surgically removed. Histologic examination proved diagnosis of leiomyosarcoma. Nineteen months after the surgery the patient has no evidence of disease progression or metastasis., and Wasserbauer R., Ivičič J.
V článku popisujeme kazuistiky dvou pacientů s maligním a benigním primárním tumorem retroperitonea, které byly diagnostikovány na základě symptomatologie z progredujícího narůstaní objemu tumoru. U jednoho pacienta byla provedena punkční biopsie se suspekcí na low grade liposarkom. Pacient byl indikován k operační revizi. S odstupem sedmi měsíců od operace došlo k recidivě onemocnění a byla provedena exstirpace recidivy tumoru s adjuvantní chemoterapií. Po ukončení systémové léčby byl pacient pro druhou recidivu opět operován s neradikálním odstraněním tumoru a byl indikován k paliativní systémové léčbě. V druhém případě byla na základě zobrazovacích metod indikovaná adrenalektomie pro suspektní tumor nadledviny. Definitivní histologie vyloučila afekci nadledviny a popsala ganglioneurom retroperitonea., We decribe a case reports of two patients with benign and malignant primary retroperitoneal tumors that presented with symptoms due to of the progressive tumor growth. One patient un150 Ces Urol 2015; 19(2): 149–156 Kazuistika derwent percutaneous biopsy with a suspicion of low grade liposarcoma. The patient was designated for surgical intervention. The recurrence of the tumor occurs in seventh months post-surgery, at which point the tumor was excised and followed by postoperative chemotherapy. Upon completion the patient underwent an additional surgery for a second recurrence. Incomplete removal of the tumor was performed, and palliative systemic therapy followed. In the second case, a patient was indicated to adrenalectomy for suspicion of adrenal tumor based on finding from imaging methods. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Congenital malformations such as lumbosacral transitional vertebrae and spina bifida occulta constitute unrare anomalies and could affect the symptomatology of low back pain. A transitional vertebra is characterized by elongation of one or both transverse processes, leading to the appearance ofa sacralized fifth lumbar vertebra or a lumbarized first sacral vertebra. Furthermore, sacral spina bifida occulta is a developmental anomaly that corresponds to the incomplete closure of the vertebral column. In the present case report, we describe a case of a dried sacrum presenting a partially sacralized fifth lumbar vertebra and total spina bifida, extended from first to fifth sacral vertebra. A pseudoarthrosis is formed on the left side and the specimen could be incorporated in Castellvi's type IIa.
