Stability conditions in a wider surrounding of the rock castle Drábské Světničky (Drábské Rooms) near the town of Mnichovo Hradiště were investigated. The area which has been intensively disturbed by large old as well as present slope movements is located in the north-western part of Příhrazy Platform. Solid, thick bedded sandstones, well resistant to weathering, are lying on claystones apt to plastic deformations. Marginal sandstone blocks separate, move down on the slope and sink into the plastic bedrock. As a result, block fields with many crevasses develop. In rock walls that separate individual blocks, rockfalls originate and central, as well as lower parts of the slopes develop large landslides. A zone comprising up to 400 m wide rim of the high and exposed platform has been subject to a process of loosening. A local group of tower-like sandstone blocks was used in the 15th century to build a small rock castle called Drábské Světničky. An extensive landslide that destroyed a substantial part of the village of Dneboh in June 1926, reached in its separating zone up to the toe of marginal rock towers belonging to the complex of Drábské Sv ě tni č ky with the result of local movement activation. Marginal zones of the flat land behind display fresh linear, as well as oval depressions and sinks. Fissure and pseudocarst caves develop. The author of the study analyses the influence of the Slovak element in the Czech literary development; it aims at the interpretation of various relations and functions which represented the gates through which the Slovak literature entered the common dialogue covering understanding and mutuality, but also non-understanding and rejection. The intertextual net of language, genre and motif-structural elements is manifested on the works of several both well-known and forgotten authors.
Slow breathing training reduces resting blood pressure, probably by modifying central autonomic control, but evidence for this is lacking. The pressor response to static handgr ip exercise is a measure of autonomic control and the aim of this study was to determine whether slow breathing training modulates the pressor responses to exercise of untrained muscles. Twenty hypertensive patients trained for 8 weeks, 10 with unloaded slow breathing (Unloaded) and 10 breathing against an inspiratory load of 20 cm H 2 O (Loaded). Ten subjects were untrained controls. Subjects performed a 2 min handgrip pressor test (30 % MVC) pre - and post- training, and blood pressure and heart rate (HR) were measured before the contraction, at the end and following 2 min recovery. Resting systolic (sBP) and HR were reduced as a result of tra ining, as reported previously. After training there was both a smaller pressor response to hand grip exercise and a more rapid recovery of sBP and HR compared to pre -training. There were no changes in the Controls and no differences between the Unloaded and Loaded groups. Combining the two training groups, the sBP response to handgrip exercise after training was reduced by 10 mm Hg (95 % CI: - 7, - 13) and HR by 5 bpm (95 % CI: - 4, - 6), all p<0.05. We have investigated slow inactivation in a rat axonal K+ channel, the I channel. Using voltage steps to potentials between -70 mV and +80 mV, from a holding potential of -100 mV, we observed a marked slowing of inactivation at positive potentials: the time constant was 4.5±0.4 s at -40 mV (mean ± S.E.M.), increasing to 14.7±2.0 s at +40 mV. Slowed inactivation at positive potentials is not consistent with published descriptions of C-type inactivation, but can be explained by models in which inactivation is preferentially from closed states (which have been developed for Kv2.1 and some Ca2+ channels). We tested two predictions of preferential closed-state models: inactivation should be more rapid during a train of brief pulses than during a long pulse to the same potential, and the cumulative inactivation measured with paired pulses should be greater than the inactivation at the same time during a continuous pulse. The I channel does not behave according to these predictions, indicating that preferential closed-state inactivation does not explain the slowing of inactivation we observe at positive potentials. We obtain a conditional, under the Generalized Riemann Hypothesis, lower bound on the number of distinct elliptic curves E over a prime finite field F_{p} of p elements, such that the discriminant D(E) of the quadratic number field containing the endomorphism ring of E over F_{p} is small. Gastrointestinal motility is an integrated process including myoelectrical and contractile activity, tone, compliance and transit. The techniques for the assessment of gastrointestinal motility are multiple and all have their advantages and disadvantages. In the case of suspected abnormal upper gut transit, gastric and small bowel transit scintigraphy followed by small intestinal (antroduodenojejunalileal) manometry is recommended. Small bowel manometry can identify patterns suggestive of myopathy, neuropathy or obstruction. Information on procedures, indications, significance, pitfalls and guidelines for small bowel manometry is provided in this paper. Dosáhnout cílové hodnoty krevního tlaku je stále velkým problémem. Pouze 30 % léčených hypertoniků dosahuje hodnot krevního tlaku< 140/90 mm Hg. Pacienti s hypertenzí mají často další rizikové faktory – diabetes mellitus, dyslipoproteinemii, nadváhu a obezitu, které se společně skrývají v definici metabolického syndromu. Kardiovaskulární riziko spojené s jednotlivými rizikovými faktory se nesčítá, ale násobí. Bohužel, zatím léčíme jednotlivé rizikové faktory odděleně. Nicméně je snaha, aby léčba jednoho rizikového faktoru mohla pozitivně ovlivnit jiné rizikové faktory. Práce je zaměřena na léčbu diabetes mellitus 2. typu, dyslipidemii, nadváhu a obezitu k možnému ovlivnění hodnot krevního tlaku., Reaching the target value of blood pressure is still a big problem. A mere 30 % of treated hypertensives reach blood pressure values < 140/90 mm Hg. Hypertensive patients often exhibit further risk factors – diabetes mellitus, dyslipoproteinemia, excess weight and obesity, all of which are included in the definition of metabolic syndrome. The cardiovascular risks associated with the individual risk factors are not summed, they are multiplied. Unfortunately, the individual risk factors have been treated separately until now. However, there is an effort aimed at treating individual risk factors in the way that may positively affect others. Úvod: Kardiochirurgické výkony jsou zatíženy signifikantním rizikem vzniku ischemické cévní mozkové příhody (CMP), které se dle jednotlivých studií pohybuje mezi 1,3 a 3,6 %. Asymptomatické ischemické infarkty lze detekovat po kardiochirurgické operaci dokonce až u 32 % pacientů. Cílem studie je prokázat efekt sonolýzy (kontinuálního transkraniálního dopplerovského (TCD) monitoringu a. cerebri media (ACM)) u pacientů podstupujících kardiochirurgický výkon na snížení rizika vzniku nových ischemických lézí detekovaných pomocí magnetické rezonance (MR). Soubor a metodika: Do studie jsou zařazováni pacienti indikovaní k izolovanému aortokoronárnímu bypassu nebo k izolovanému výkonu na jedné srdeční chlopni. Pacienti jsou randomizováni do skupiny se sonolýzou (monitorování pomocí TCD) a kontrolní skupiny bez sonolýzy. U všech pacientů byla provedena MR mozku před výkonem a 24 hod po něm a byla hodnocena četnost výskytu, lokalizace a velikost nových ischemických změn po výkonu. Výsledky: Během 16 měsíců bylo zařazeno 78 pacientů (48 mužů, průměrný věk 63,7 ? 15,8 let), 35 pacientů bylo randomizováno do skupiny se sonolýzou, 43 pacientů do kontrolní skupiny. Ve skupině sonolýzy byly detekovány nové ischemické změny u osmi pacientů (23 %) – u pěti (14 %) v povodí pravé (stimulované) ACM, ale jen u dvou (6 %) větší než 0,5 cm3. V kontrolní skupině byly detekovány nové ischemické změny u 10 pacientů (23 %) – u osmi (19 %) v povodí pravé ACM, u pěti (12 %) větší než 0,5 cm3 (p > 0,05 ve všech případech). Závěr: Pilotní data studie prokázala trend k redukci počtu i objemu ischemických lézí mozku u pacientů se sonolýzou v průběhu kardiochirurgické operace., Background: Heart surgery is burdened with a significant risk of ischemic stroke. Asymptomatic cerebral infarctions can be detected in as many as 32% of patients after heart surgery. The aim of this study is to prove the efficacy of sonolysis (continual transcranial Doppler (TCD) monitoring) during open-heart surgery to decrease the risk of new brain infarctions detected by magnetic resonance imaging (MRI). Material and methods: Patients indicated to isolated coronary artery bypass or isolated one heart valve surgery were included in the study. Patients were randomized to a sonolysis group (TCD monitoring), and to a control group without sonolysis. All patients underwent brain MRI before and 24 hours after the surgery and the presence of new ischemic lesions was evaluated. Results: During 16 months, 78 patients (48 males, mean age 63.7 ? 15.8 years) were enrolled to the study. Thirty five patients were randomized to the sonolysis group and 43 to the control group. In the sonolysis group, new brain infarctions were found in eight (23%) patients – in the right (TCD-monitored) MCA territory in five patients (14%) but > 0.5 cm3 in two (6%) patients only. In the control group, new infarctions were found in 10 patients (23%) – in the right (TCD-monitored) MCA in eight (19%) patients and > 0.5 cm3 in five (12 %) (p > 0.05 in all cases). Conclusion: Pilot study results showed a trend towards reduction in a number and volume of new brain ischemic lesions in patients treated with sonolysis during heart surgery.
