Rilmenidin je antihypertenzivum ze skupiny agonistů imidazolinových receptorů. Podstatou jeho mechanizmu účinku je výrazné snížení kardiální a renální sympatické baroreflexní odpovědi na úrovni I1-imidazolinových receptorů s následným snížením aktivity sympatiku. Účinnost a snášenlivost rilmenidinu byly prokázány v randomizovaných srovnávacích studiích u starších pacientů, hypertoniků s diabetem a metabolickým syndromem i u pacientů s renálním selháním. Indikací pro podávání rilmenidinu je léčba esenciální hypertenze u dospělých pacientů všech věkových kategorií. The basis of its mechanism of action is significant reduction of cardiac and renal sympathetic baroreflex response at the level of I1-imidazoline receptors with subsequent reduction of the sympathetic activity. The effectiveness and tolerability of rilmenidine were proved in randomized comparative studies in elderly patients, patients suffering from hypertension together with diabetes and metabolic syndrome, and in patients with renal failure. A new myxozoan, Ceratomyxa bohari sp. n., infecting the gall bladder of two-spot red snapper, Lutjanus bohar Forsskål, in the Red Sea off Saudi Arabia, is described using light microscopy and characterised genetically. The infection was recorded as mature spores floating free in the bile. The overall prevalence of infection of the type host was 19% (67 fish infected of 360 examined), with the highest prevalence in autumn (31%; 28/90) and the lowest in winter at 12% (11/90). Mature spores are slender and slightly crescent-shaped in the frontal view, with anterior and posterior margins tapered gradually to rounded valvular tips. Spore valves are unequal with a prominent sutural line. The spore dimensions are 3-4 μm (mean 3.5 μm) in length and 16-19 μm (mean 17 μm) in thickness. Two polar capsules are spherical, equal in size, 1.5 μm in diameter. Coils of the polar filament are indiscernible. The sporoplasm is binucleated and fills nearly one third of the extracapsular space restricted to the area below the capsules. The molecular analysis based on the small subunit rDNA (SSU rDNA) sequence revealed a close relationship with majority of species of Ceratomyxa Thélohan, 1892 and phylogenetic clustering with species from different geographical location. Thus, the shorter spore of the present Ceratomyxa species and the divergence of the SSU rDNA sequences were the distinctive features that separate it from all previously described species and identified this parasite as a new species of Ceratomyxa., Lamjed Mansour, Abdel-Azeem S. Trophozoites of Ceratomyxa drepanopsettae Averintsev, 1907 (Myxosporea: Ceratomyxidae) containing prominent refractile granules were found in the gallbladders of all but one of eight halibut, the exception being a single juvenile. They ranged in shape and size from globular forms 5-10 pm in diameter, to rounded structures with pseudopodia and one or more processes that were up to 500 pm in length and packed with refractile granules. Some trophozoites were free in the bile, while others were attached to the epithelium of the gallbladder wall by pseudopodia which extended between the microvilli. Many free trophozoites were attached to each other by septate junctions between their pseudopodia. There were small cylindrical papillae on the surface of the trophozoites, and the rounded portions contained two vegetative nuclei, generative cells (some attached by junctions) and, in many cases, feeding vacuoles. During sporogony, a binucleate sporoplasmic cell and the capsulogenic cells of some sporoblasts were engulfed by valvogenic cells before they began to differentiate; whereas other sporoblasts consisted of six cells attached to each other, two being capsulogenic cells containing external tubes, two sporoplasmic cells and two valvogenic cells. There was a septate junction around the opening of the rounded polar capsule of the spore, between the capsulogenic and valvogenic cell. Sporoplasmosomes appeared to form in smooth membraned vesicles, possibly part of the Golgi apparatus. Spores had a thin, delicate membrane, and elongate shell-valves, most of which were asymmetric, and bent or folded. A sporo-plasm extended on either side of the distinct, straight suture line, but did not penetrate into the valves.
As a consequence of enhanced production of oxygen free radicals, lipid peroxidation leads to the degradation of membrane lipids and disturbances of membrane permeability. Lipid peroxidation increases under stress conditions such as hypoxia, ischemia or acidosis as well as in metabolic diseases, e.g. diabetes mellitus. We have shown that subcomatous doses of insulin (6.0 IU/kg) significantly increase thiobarbituric acid reactive substances (TBARs), especially malondialdehyde (MDA) - the endproduct of lipid peroxidation, in the brain and heart of mice. In our model of insulin-induced hypoglycemia, mice were treated with the neuroprotective, peptide-containing drug Cerebrolysin (100 mg/kg b.w.). Animals were sacrificed by decapitation two or three hours after the injection of tested substance and samples were taken to determine several serum parameters (glucose, total protein, triglycerides and lactic acid) and TBARs in the brain and heart. Although Cerebrolysin was not able to affect serum parameters after subcomatous insulin injection, the drug significantly influenced lipid peroxidation. A single injection of Cerebrolysin already decreased TBARs levels in the brain and heart tissue. This study introduces an emblematic scheme within the stucco decoration of the Palace Chapel in Červené Poříčí. The given emblematic sheme glorifies the Most Sacred Heart of Jesus. It was inspired by the emblem book of the Bavarian theologian Anton Ginther, which was published in 1706. The article tries to place the programme of the decoration into the context of Middle European evolution of the devotion to the Most Sacred Heart of Jesus - a cult that was very popular at the time. Cíl: Naším záměrem bylo zjistit diagnostické charakteristiky Montrealského kognitivního testu (MoCA) v tréninkové podobě pro lepší administraci u pacientů s Alzheimerovou nemocí (AN) poprvé v českém písemnictví. <p align="">Soubor a metodika: Záznamový formulář jsme obohatili o instrukce pro vyšetřovaného, instrukce k administraci a návod ke skórování. Obsah a skórování zůstaly zachovány a odpovídají předchozí české verzi. Upravili jsme návod k administraci. Zpětný překlad záznamového archu a návodu do angličtiny byl zkontrolován, schválen a nazván kanadským tvůrcem jako tzv. tréninková verze MoCA‑CZ1. Tímto testem jsme vyšetřili 30 pacientů s již diagnostikovanou mírnou demencí způsobenou Alzheimerovou nemocí podle nových doporučení NIA‑AA (78 ? 6 let, 13 ? 3 let vzdělání, 56 % žen) a 30 kognitivně normálních seniorů (NOS) podobného věku, vzdělání a pohlaví. <p align="">Výsledky: Oproti standardní verzi obsahují záznamové archy tréninkové české podoby MoCA‑CZ1 navíc další informace, s nimiž předpokládáme snadnější a spolehlivější provádění a vyhodnocování testu. Celkové skóry MoCA‑CZ1 byly významně nižší u pacientů s mírnou demencí způsobenou AN (18 ? 5 bodů) než u NOS (26 ? 3 body) (p < 0,00001). Celkové skóry NOS nezávisely na věku, pohlaví a vzdělání. Při porovnávání obou skupin podle analýzy receiver operating curve byl zjištěn optimální hraniční skór ≤ 23 bodů se senzitivitou 87 % a specificitou 84 %. Plocha pod křivkou byla 0,92. <p align="">Závěry: Tréninková MoCA‑CZ1 je volně dostupná na http://www.pcp.lf3.cuni.cz/adcentrum/testy.html. Výsledek testu může přispět k dřívějšímu záchytu mírné demence způsobené AN, a to časnou detekcí kognitivního deficitu. Při používání tréninkové verze MoCA‑CZ1 doporučujeme používat hraniční skór ≤ 23 bodů., Aim: Our aim was to determine diagnostic characteristics of the Montreal cognitive assessment (MoCA) using its training version for better administration in patients with Alzheimer’s disease (AD) for the first time in Czech literature. <p align="">Patients and methods: We complemented the test form with instructions for the patient, and with administration and scoring instructions. The content and the scoring were preserved from the previous Czech version. A separate Czech administration instruction manual was refined. English back translation of the data collection form and the manual was checked and approved by the Canadian author who also named it the training version. We examined 30 patients previously diagnosed with mild dementia due to Alzheimer’s disease according to the new NIA‑AA recommendations (78 ? 6 years, 13 ? 3 years of education, 56% women) and 30 cognitively normal seniors (NOS) of similar age, education and gender by MoCA‑CZ1. <p align="">Results: The training version of MoCA‑CZ1 contains additional information. We assume that this ensures reliability, and facilitates administration and scoring of the test. Total MoCA‑CZ1 scores were significantly lower in patients with AD (18 ? 5 points) than in NOS (26 ? 3 points) (p < 0.00001). Total scores of NOS did not correlate with age, gender or education. Analysis of receiver operating curve comparing the two groups found optimal cut off score of ≤ 23 points with sensitivity of 87% and specificity of 84%.
