Juxtapapilární divertikl duodena je literárně uváděn jako možná příčina řady pankreatobiliárních onemocnění. Etiologická souvislost však často není považována za jednoznačnou a v reálné klinické praxi bývá možná role divertiklu podceňována, až popírána. Tento článek na základě kazuistiky a přehledu literatury na tuto problematiku poukazuje. Za klinicky významnou známku možného podílu divertiklu na potížích lze považovat vyplnění lumen divertiklu bezoárem. Tento nález by, po vyloučení jiných příčin, měl vést ke zvažování etiologické souvislosti divertiklu s pankreatobiliárními potížemi. Řešení představují endoskopická papilosfinkterotomie jako metoda první volby a chirurgický výkon (divertikulektomie, biliodigestivní spojka či kombinace) v případě recidiv potíží a selhání méně invazivního postupu., Juxtapapillary duodenal diverticula are reported as a potential cause of many pancreatobiliary diseases. However, data concerning this association is inconsistent and the role of the diverticulum is often underestimated or even denied in clinical practice. This case report and literature review is aimed at pointing out this problem. Obstruction of the diverticulum with a food bezoar can be considered as an important clinical clue of the etiological role of the diverticulum in pancreatobiliary disease development. Úvod: Gastrointestinální stromální tumory (GIST) jsou specifické neepitelové mezenchymální nádory gastrointestinálního traktu. Většina GISTů (95 %) vzniká v důsledku mutace genu transmembránového tyrozinkinázového proteinu (KIT), což je receptor pro růstový faktor. Tento receptor na svém povrchu exprimují nádorové buňky GISTu. Nejčastěji postiženými orgány bývají žaludek a tenké střevo, ale můžeme ho nalézt kdekoli v průběhu trávicí trubice a orgánů dutiny břišní. Klinická manifestace bývá různá a je dána lokalizací nádoru, jeho velikostí či vztahem k okolním orgánům. U resekabilních nádorů je indikováno primární chirurgické řešení s cílem R0 resekce. Cílená léčba imatinibem (kompetitivní inhibitor KIT receptoru) je určena pro pacienty s metastatickým onemocněním a dále jako adjuvantní léčba po operaci nebo v indikovaných případech jako neoadjuvantní terapie. Práce se snaží o přehledné zpracování komplexní terapie GIST s důrazem na postavení chirurgické léčby a upozorňuje na některá kontroverzní témata, např. roli resekčního výkonu u pacientů s metastatickým onemocněním nebo neoadjuvantní podání cílené biologické léčby. Závěr: I přes nezpochybnitelný úspěch biologické léčby má chirurgická léčba nezastupitelné místo v terapii tohoto onemocnění. Diagnosticko-terapeutický přístup k pacientům s GISTem musí být komplexní. Preskripční omezení biologické léčby přímo nutí směrovat tyto pacienty do specializovaných center, kde by s výhodou měla být prováděna i léčba chirurgická., Introduction: Gastrointestinal stromal tumors (GIST) are specific mesenchymal tumors of the gastrointestinal tract. Most of GISTs (95%) result from activating mutations in one of the receptor tyrosine kinase proteins (KIT). Tumor cells express this protein. GIST is most common in the stomach and small intestine, but may occur anywhere in the gastrointestinal tract and intra-abdominal soft tissues. The variety of its clinical presentations is related to localization of the tumor, its size and relationship to surrounding organs. Surgery is the first choice of treatment for patients with localized or potentially resectable tumors with the intention of R0 resection. Targeted therapy with imatinib (a selective inhibitor of the KIT protein) is the primary option for patients with metastatic GIST, as adjuvant treatment after surgery or neo-adjuvant therapy prior to surgery in indicated cases. This paper describes comprehensive therapy of GIST with an emphasis laid on the status of surgical treatment, and it highlights some controversial topics, e.g. the role of surgery for metastatic disease or neo-adjuvant targeted therapy. Conclusion: GIST is a relatively rare tumor most commonly affecting the stomach and small intestine. Surgical treatment is not replaceable in the treatment strategy of this disease. The diagnostic-therapeutic approach to patients with GIST must be comprehensive. ONSTEP (Open New Simplified Totally Extraperitoneal) technika reparace tříselné/femorální kýly je inovativní metoda, jejímiž autory jsou portugalští chirurgové Dr. Augusto Lourenço a Dr. Rui Soares da Costa. Prezentace formou posteru proběhla na EHS/AHS kongresu v březnu 2012. Na území České republiky jsou prvními pracovišti provádějícími tento typ operace Chirurgické oddělení nemocnice Prostějov a Chirurgické oddělení Nemocnice ve Frýdlantě. ONSTEP technika představuje minimálně traumatizující přední přístup k operaci tříselné kýly s použitím samoexpandibilní síťky (oválná, anatomicky tvarovaná polypropylenová síťka s obvodovým paměťovým prstencem z polyetylen tereftalátu). Hlavní výhody této metody spočívají v krátké době operačního výkonu, možnosti provedení v lokální anestezii, krátké době rekonvalescence, nízkém výskytu pooperační chronické bolesti, nízkém počtu recidiv, krátké době hospitalizace a dobrém kosmetickému efektu. Je vhodná pro pracoviště zabývající se jednodenní chirurgií. Klíčová slova: tříselná kýla – operace – ONSTEP, The ONSTEP (Open New Simplified Totally Extraperitoneal) inguinal/femoral hernia repair technique is an innovative procedure presented by Portuguese surgeons Augusto Lourenço and Rui Soares da Costa within the EHS/AHS Congress in March 2012. The first institutions to perform this kind of hernia surgery in the Czech Republic are the Department of Surgery at Prostejov Hospital and the Department of Surgery at Frydlant Hospital. The ONSTEP technique represents a minimally traumatizing frontal approach to inguinal hernia repair using a special self-expanding mesh patch (an oval, anatomically-shaped moderate-pore polypropylene mesh with the circumferential memory-ring consisting of polyethylene terephthalate). The main advantages of this method are: short duration of surgery, the possibility of performing the procedure under local anaesthesia, short recovery time and rapid return to daily activities, low incidence of chronic post-operative groin pain, low recurrence rate, short hospital stay, good cosmetic effect and a low price.
