Dehydroepiandrosterone (DHEA) and its sulphate-bound form (DHEAS) are important steroids mainly of adrenal origin. Their physiological and pathophysiological functions are not yet fully identified, although a number of various possible features have been hypothesized. Most popular is the description of the “hormone of youth” as the long-term dynamics of DHEA levels are characterized by a sharp age-related decline in the late adulthood and later. Low levels of DHEA are, however, associated not only with the ageing process but also with diabetes mellitus, cardiovascular diseases and some neurological or immunological entities. In the past decade, a number of brief studies have concentrated on these relationships and also on the role of exogenous DHEA in health, disease and human well-being. Osteoporóza je progredující systémové onemocnění skeletu charakterizované úbytkem kostní hmoty a poruchami mikroarchitektury kostní tkáně a v důsledku toho zvýšenou náchylností ke zlomeninám. Během života utrpí jedna ze tří žen a jeden z pěti mužů osteoporotickou zlomeninu. Spolu s nárůstem osteoporotických zlomenin se onemocnění stává významným zdravotním, sociálním a ekonomickým problémem. S prodlužující se délkou života narůstají zlomeniny nejen obratlových těl a proximálního femuru, ale i další nevertebrální zlomeniny. Osteoporotické zlomeniny snižují kvalitu života, zvyšují morbiditu i mortalitu. Cílem léčby je snížit riziko první zlomeniny v rámci primární prevence a zajistit léčbu nemocným s prodělanou nízkotraumatickou zlomeninou v rámci sekundární prevence zlomenin. Nejrozšířenější paletu antiresorpčních léků tvoří bisfosfonáty, které se deponují v kosti a snižují riziko zlomenin o 40–60 %. Dalším lékem s odlišným mechanizmem účinku a silným antiresorpčním potenciálem je denosumab. Velkým přínosem pro nemocné s pokročilou manifestní osteoporózou je léčba osteoanabolická. Článek pojednává o přístupech k diagnostice osteoporózy, posouzení rizika zlomenin a současných možnostech léčby osteoporózy. Consequently, the bone is prone to fractures. About 30% of women and 20% of men will suffer from an osteoporotic fracture in their lifespan. Over the last few decades, a rise in the osteoporotic complications has been observed. The disease has become an important medical, social and economic burden. With increasing life expectancy, the number of vertebral body fractures and also proximal femoral bone and other non‑vertebral fractures has been growing. Osteoporotic fractures worsen the quality of life, increase the need for hospitalisation and even mortality. The first treatment goal is to decrease the risk of first fracture as part of primary prevention and provide treatment to patients who have already suffered their first low trauma fracture, using secondary prevention methods to avoid repeated fractures. The most useful antiresorptive drugs are bisphosphonates. They are stored in bones and reduce the risk of fracture by 40 to 60 percent. Another very potent antiresorptive drug with a different mode of action is denosumab. Another therapeutic option is the osteoanabolic treatment, with a significant benefit for patients with serious forms of the disease. Gestageny mají různé biologické vlastnosti, na které bereme ohled při jejich volbě. V hormonální substituční terapii volíme progestin dle jeho metabolické neutrálnosti a bezpečnosti vzhledem k možnému riziku onemocnění karcinomem prsu. Mikronizovaný progesteron a dydrogesteron mají pravděpodobně lepší bezpečnostní profil než syntetické progestiny. Také v suplementaci gestagenů v těhotenství dáváme přednost přirozeným gestagenům před syntetickými progestiny. The best progestogen for the hormone replacement therapy could be metabolic neutral and safe for breast tissue. Micronised progesterone and dydrogesterone have better safety profile than synthetic progestagens. Also during pregnancy we prefer natural gestagens to synthetic ones. b1_The aim of our study was to compare the responses of heart rate variability (HRV) with two di fferent types of hormonal substitution therapy (HT) in post-menopausal women (cross-sectional study) and to reveal an effect of HT shortly after beginning of its administration (f ollow-up study). To elucidate the influence of menopause and effects of different protocols of a HT on autonomic control of heart rate, we evaluated the heart rate variability (HRV) in 5 groups: premenopausal women (n=140), postmenopausal women without HT (n=360), women on HT with conjugated estrogen only (n =168), women on continuous combined estrogen-progesterone HT (n=117), and men (n=140). Frequency-domain of short-term stationary R-R intervals was performed to evaluate the total variance, low frequency power (LF; 0.04-0.15 Hz), high freque ncy power (HF; 0.15-0.40 Hz), portion of low frequency power (LF%) and ratio of LF to HF (LF/HF). Significantly lower portion of the LF was found in premenopausal women [46.9 (±2.7) nu] when compared to untreated postmenopausal wome n [54.3 (±2.9) nu] and men [55.2 (±3.0) nu]. Treatment by estrogen only was proved to decrease the LF% [40.1 (±2.1) nu] while no effect on HRV was observed in women treated with combination of estrogen and progesterone [57.2 (±3.1) nu]. Also the HF was lower in postmenopausal wome n [4.16 (±0.16) ms 2 ] than in premenopausal women [4.79 (±0.22) ms 2 ] and women treated with estrogen only [4.98 (±0.25) ms 2 ] while in women treated with combined hormonal therapy the average value [3.99 (±0.21) ms 2 ] did not significantly differ from that of untreated postmenopausal women. The follow-up study also proved increase of high frequency power already after two months of estrogen substitution therapy [4.86 (±0.14) ms 2 vs. 4.19 (±0.15) ms 2 ]., b2_These results suggest that hi gher vagal modulation of heart rate that seems typical for younger women becomes after menopause similar to that of men. We also proved a positive shift of HRV parameters toward more beneficial values as for a cardiovascular risk in postmenopausal women treated with estrogens but not in those treated by combined estrogen - progesterone substitution therapy., S.-G. Všechna existující doporučení uvádějí hormonální substituční terapii (HRT) jako lék volby pro akutní klimakterický syndrom a vaginální nízkodávkovanou estrogenní terapii jako lék volby pro léčbu a prevenci urogenitální atrofie s jejími důsledky. Potvrzuje se, že časný start HRT je spojen s neutrálním až ochranným vlivem na ischemickou chorobu srdeční. HRT je jednoznačně účinnou prevencí i léčbou postmenopauzální osteoporózy, nicméně dlouhodobé podávání vyžaduje individuální hodnocení poměru přínos/riziko s rozdíly v závislosti na složení HRT., All existing recommendations sign hormone replacement therapy (HRT) as first therapy for acute climacteric syndrome. The low dose vaginal therapy is the first choice for prevention and therapy of urogenital atrophy and its complications. Early start of HRT has neutral or slightly beneficial influence on ischaemic heart disease. It is sure that HRT is effective in prevention and therapy of postmenopausal osteoporosis. Selektivní modulátory estrogenních receptorů, jakým je např. klomifen citrát, se užívají pro off-label léčbu (tj. mimo schválenou indikaci) hypoandrogenizmu a idiopatické mužské infertility po více než 30 let. V rámci systematické analýzy hodnotíme otázky týkající se dávkování, hormonální a symptomatické účinnosti podávání selektivních modulátorů estrogenních receptorů při léčbě mužů trpících hypoandrogenizmem a nežádoucí účinky spojené s touto léčbou. Výsledky naší analýzy ukazují, že léčba je pacienty dobře snášena a ve srovnání s tradiční exogenní substitucí testosteronu je rovněž účinná. Optimální dávkování klomifen citrátu je však třeba teprve stanovit. Dále uvádíme přehled našich postupů pro užívání klomifen citrátu a monitorování jeho bezpečnosti., Selective estrogen receptor modulators, such as clomiphene citrate, have been used as an off-label treatment of hypoandrogenism and idiopathic male infertility for over 30 years. We performed a systematic analysis of selective estrogen receptor modulators use in men with hypoandrogenism to examine issues related to dosage, hormonal and symptomatic efficacy, and adverse events. Our analysis indicates that treatment is well-tolerated and efficacious when compared to traditional exogenous testosterone supplementation. However the ideal dosage of clomiphene citrate has yet to be elucidated.
