Stadium III nemalobuněčného karcinomu plic (NSCLC) má nízké 5leté přežití (méně než 30 %). Abychom měli reálnou šanci zlepšit přežití nemocných, je nutné dodržet kvalitní staging a použít kvalitní léčbu. V rámci stagingu je důležité přesně určit postižení regionálních mízních uzlin a vyloučit generalizaci. Pro staging regionálních uzlin je doporučeno použití PET/CT trupu (pozitronové emisní tomografie/výpočetní tomografie). Pozitivní nálezy, měnící stadium, je doporučeno biopticky verifikovat. Indikace léčby je vždy přísně individuální, ideálně v rámci multidisciplinárních týmů; používáme léčbu multimodální, s využitím více léčebných metod (chemoterapie, radioterapie, operace). Nadějným příslibem do budoucna je pro nemocné se stadiem III imunoterapie., Stage III non-small-cell lung cancer (NSCLC) has a low 5-year survival rate (less than 30%). In order to have a real chance of improving patient survival, both staging and treatment must be of appropriate quality. During staging, it is important to precisely determine regional lymph node involvement and rule out generalization. PET/CT (positron emission tomography/computed tomography) of the torso is recommended to be used for regional lymph node staging. Positive findings that alter the stage are to be confirmed bioptically. Treatment is always indicated strictly individually, ideally within multidisciplinary teams; it is multimodal, using multiple therapeutic methods (chemotherapy, radiotherapy, surgery). For stage III patients, immunotherapy is a promising approach for the future., Gabriela Krákorová, and Literatura
Ionizing radiation and somatostatin analogues are used for acromegaly treatment to achieve normalization or reduction of growth hormone hypersecretion and tumor shrinkage. In this study, we investigated a combination of somatostatin (SS14) with ionizing radiation of 60Co and its effect on reparation of radiation-induced damage and cell death of somatomammotroph pituitary cells GH3. Doses of γ-radiation 20-50 Gy were shown to inhibit proliferation and induce apoptosis in GH3 cells regardless of somatostatin presence. It has been found that the D0 value for GH3 cells was 2.5 Gy. Somatostatin treatment increased radiosensitivity of GH3 cells, so that D0 value decreased to 2.2 Gy. We detected quick phosphorylation of histone H2A.X upon irradiation by the dose 20 Gy and its colocalization with phosphorylated protein Nbs-1 in the site of double strand break of DNA (DSB). Number of DSB decreased significantly 24 h after irradiation, however, clearly distinguished foci persisted, indicating non repaired DSB, after irradiation alone or after combined treatment by irradiation and SS14. We found that SS14 alone triggers phosphorylation of Nbs1 (p-Nbs1), which correlates with antiproliferative effect of SS14. Irradiation also increased the presence of p-Nbs1. Most intensive phosphorylation of Nbs1 was detected after combined treatment of irradiation and SS14. The decrease of the number of the DSB foci 24 h after treatment shows a significant capacity of repair systems of GH3 cells. In spite of this, large number of unrepaired DSB persists for 24 h after the treatment. We conclude that SS14 does not have a radioprotective effect on somatomammotroph GH3 cells., M. Řezáčová, J. Úvod: Stanovení strategie léčby karcinomu rekta vyžaduje komplexní a multidisciplinární přístup, na základě kterého je indikována racionální léčba. Byly stanoveny tři okruhy otázek: 1. charakteristika pracoviště a spolupráce s onkologií; 2. analýza léčby pacientů s karcinomem rekta; 3. detailní analýza skupiny pacientů s kompletní patologickou odpovědí. Klíčová slova: karcinom rekta – neoadjuvantní terapie – chemoradioterapie – kompletní patologická odpověď, Introduction: Deciding on the strategy in rectal cancer´s treatment requires a complex and multidisciplinary approach. The primary rectal resection is indicated in early stages, while locally advanced tumors should be pretreated by one of the modes of neoadjuvant (chemo) radiotherapy. The main goal of this study was to explore the therapeutic strategy in patients with rectal cancer in the Czech Republic. The second aim was to determine the incidence of the pathological complete response after neoadjuvant therapy. Methods: This is a retrospective multicenter clinical study, which includes data from all patients with rectal cancer who were treated at participating centers in the period from 01/01/2011 to 31/12/2012. The required data has been passed into the online registry PATOD C20.Three issues have been set up: 1. Characteristics of the center and cooperation with the oncological department; 2. Characteristics of the treatment of patients with rectal cancer; and 3. Detailed analysis of the group of patients with complete pathological response. The analysis was performed with regard to the nature of individual departments, i.e. departments of surgery in university hospitals with complex oncological centres, departments of surgery within complex oncologic centers, and departments of surgery outside complex oncologic centers Results: In total, 21 departments of surgery in the Czech Republic provided data about 1860 patients with rectal cancer for the study. The treatment strategy for rectal cancer was determined at multidisciplinary seminars at 19 centers (90.5%). Statistically significant differences between the centers were found in the indication for neoadjuvant treatment (p<0.001), rectal resection with anastomosis (p = 0.048), and resection without anastomosis (p = 0.022). Complete pathological response was found in 61 (8.7%) patients. Positivity of mesorectal lymph nodes (ypN+) was found in the case of ypT0 stage in 7 (9.7%) patients. Conclusion: PATOD study showed that therapy of rectal cancer is highly heterogeneous in the Czech Republic. Despite the best conditions provided, university hospitals and large departments within complex oncologic centers do not fully utilize this benefit.
