Také potenciální význam CA125 v imunoterapii karcinomu ovaria se nezdá být tak velký, jak se předpokládalo., The most fatal gynecologic malignancy, ovarian cancer, causes more than 50% deaths in this tumor group. Most of cases (>60%) are diagnosed in advanced stages with poor 5-year survival prognosis. Diagnostic tools for an effective early detection or screening have not been found yet. Treatment possibilities (surgery, chemotherapy) are insufficient while high tendency to recurrence and chemoresistance occurs. CA125/MUC16 has been one of the most extensively used markers for a detection of primary tumors, or recurrence for a long time since its discovery in 80´s. However, the structure and biological functions of CA125 have been discovered relatively recently. CA125 may play an important role in carcinogenesis and interactions with cells of immune system. We reviewed the known biological functions and particularly the current state of using this marker as the diagnostic tool in ovarian cancer. Recently, many new markers emerged ambitiously to replace CA125; moreover, the importance of monitoring CA125 for the recurrence detection has also been questioned in large clinical trials. These studies have not found an impact for overall survival/mortality of patients. Also a potential of using CA125 targeted antibodies has not brought previously expected results so far., Luděk Záveský, and Literatura 20

CD200 and its receptor were recognized as having the multiple immunoregulatory functions. Their immunoregulatory, suppressive, and tolerogenic potentials could be very effectively exploited in the treatment of many diseases, e.g. Alzheimer disease, rheumatoid arthritis, and allergy to name only some. Many research projects are aimed to develop clinically valuable methods being based on the structure and function of these paired molecules. In this review, we would like to introduce CD200/CD200R functions in a clinical context., Drahomíra Holmannová, Martina Koláčková, Kateřina Kondělková, Pavel Kuneš, Jan Krejsek, Ctirad Andrýs, and Literatura 51
Přestože chemodektom je všeobecně považován za radiosenzitivní, hlavní terapeutický postup je chirurgická exstirpace tumoru., Chemodectoma, paraganglioma of glomus caroticum or carotid glomus tumor occurs in the population with an incidence of 1:30 000. The tumor is macroscopically, microscopically and biologically very similar adrenal pheochromocytoma. An important feature of these tumors is their rich vascularization. Among the most blood supply tumors at all. Histologically, alveolar described arrangement of tumor cells, in contrast to pheochromocytoma, sharply demarcated from each other soft tissue with broad stripes capillaries and cells like Schwann cells. The cytoplasm of tumor cells is a major weakly eosinophilic and finely granular. Nuclei of cells may be pleomorphic and hyperchromic, but must be stated that nuclear atypia is not unequivocal sign of malignancy. Biologically they are mostly benign tumors. Clinically, it is a slow-growing, painless, firm and globular in shape for the angle of the mandible. Although chemodectomas were found is generally considered to be radiosensitive, the main therapeutic approach is surgical extirpation of the tumor., Alžběta Poprachová, Monika Váchalová, Radka Filipčíková, Martin Dobiáš, Zdeňka Blažková, Milada Dušková, Petr Utíkal, Marcela Bezdičková, and Literatura
Nová cílená léčiva nemají výraznější toxicitu než standardní cytostatika a jejich efektivita byla prokázána u řady jiných typů nádorů., This article is aimed to summarize new targeted therapy used for treatment of advanced cancer of stomach and gastroesophageal junction. Their efficacy is being tested in numerous clinical trials worldwide. Recent findings have suggested that this type of malignancy is considered as a group of heterogenous tumors that differ in the expression of cell-signalling molecules. Toxicities of new targeted therapy do not overlap those of cytotoxic agents and their effectivity was approved in other types of tumors., Andrea Jurečková, Michaela Všianská, and Literatura 18
Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Other therapeutic options, such as interferon alpha or anagrelide, may find place in selected patients including those who are resistant/intolerant to hydroxyurea., Tiziano Barbui, and Literatura 37
