Standardní terapií v I. linii léčby hormonálně refrakterního karcinomu prostaty se stal docetaxel, a to na základě výsledků studie TAX 327. Cytostatikum pro II. linii hormonálně refrakterního karcinomu prostaty přinesly výsledky studie fáze III TROPIC, kde účinnost a bezpečnost prokázal cabazitaxel v kombinaci s prednisonem. Imunoterapie na základě výsledků klinických studií z posledního období prokázala, že má své místo v léčbě karcinomu prostaty. Biosyntéza extragonadálních androgenů může přispět k progresi kastračně-rezistentního karcinomu prostaty, proto se v jeho léčbě uplatní inhibitory androgen-regulačních enzymů., Based on the results of the TAX 327 study, docetaxel has become the standard therapy in the first-line treatment of hormone-refractory prostate cancer. Karcinom prostaty je v úvodu hormonálně senzitivní onemocnění. Jednoznačné postavení má androgen deprivační terapie (ADT) v kombinaci s radioterapií u lokalizovaného onemocnění bez průkazu vzdálených metastáz u pacientů s vysokým rizikem. V I. linii léčby metastatického onemocnění se uplatňuje farmakologická nebo chirurgická kastrace v monoterapii. Hormonální léčba prokázala svůj vliv na přežití také u kastračně rezistentního onemocnění. Znalost hormonální závislosti karcinomu prostaty a možnosti jejího ovlivnění byly popsány již ve 40. letech minulého století. Kastraci chirurgickou u velké části nemocných nahrazuje kastrace medikamentózní pomocí LHRH agonistů. K omezení kolísání hladin testosteronu během podávání LHRH (miniflare up fenomen), a tím působení nekastračních hladin testosteronu v průběhu léčby na androgenní receptor, je výhodnější podávání LHRH antagonistů. Přes dobrou a často dlouhodobou odpověď vůči hormonální léčbě dochází k selhání léčebného účinku. Iniciálně podávanou monoterapii lze rozšířit na maximální androgenní blokádu, a tím k prolongování účinku hormonální léčby. Přesto zhoubný nádor nabývá odlišné růstové charakteristiky a mění zásadně prognózu onemocnění, stává se kastračně refrakterním (CRPC), tedy progredujícím při kastračních hladinách testosteronu. Začátkem tisíciletí byla standardizována I. linie léčby kastračně refrakterního karcinomu prostaty. Podání docetaxelu s prednisonem bylo první terapií, která vedla k prodloužení přežití u pacientů s CRPC. Nově registrované léčivé přípravky jsou v efektivitě a výskytu nežádoucích účinků srovnávány s tímto režimem. Postupně se objevují režimy s imunoterapií, nová cytotoxická látka kabazitaxel a především nové hormonální přípravky abirateron a enzalutamid. V klinickém zkoušení je však řada dalších přípravků aplikovaných samostatně nebo v kombinaci se standardní léčbou. U již schválených přípravků by nám měly probíhající studie odpovědět na otázky optimální doby použití a sekvence léčby. Despite a good and often long-term response to hormonal therapy, failure of the treatment modality occurs. The monotherapy administered initially can be extended to maximum androgen blockade and, thus, to prolongation of the effect of hormonal therapy. Nevertheless, a malignant tumour acquires different growth characteristics, fundamentally altering the disease prognosis and becoming castration-resistant (CRPC), i.e. progressive with castrate testosterone levels. In the early millennium, first-line therapy for castration-resistant prostate cancer was standardized. Administration of docetaxel with prednisone was the first treatment to have resulted in prolonged survival in patients with CRPC. Newly registered medicinal products are compared with this regimen in terms of efficacy and the rates of adverse effects. Regimens with immunotherapy, a novel cytotoxic drug cabazitaxel, and, in particular, the new hormonal products abiraterone and enzalutamide are gradually emerging. A large number of agents alone or in combination with standard therapy are being investigated in clinical trials. Kolorektální karcinom u dětí je velmi vzácné onemocnění. Léčebné výsledky jsou neuspokojivé a celkové přežití dětských pacientů s kolorektálním karcinomem je nízké. Pacienti mají často histologicky nepříznivý typ nádoru a často jsou diagnostikování až v pokročilém stadiu onemocnění. Úvod: Nedostatečný zbytkový objem jater po resekci (FLRV) je hlavní příčinou primární neresekability jaterních nádorů. Embolizace větve portální žíly (PVE) s aplikací hematopoietických kmenových buněk (HSC) může být způsob, jak významnou měrou zvýšit počet nemocných s dostatečným FLRV, a tím rozšířit resekabilitu primárně chirurgicky neřešitelných nádorů jater. Metodika: U dvanácti nemocných s FLRV < 30 % byla provedena PVE na straně nádoru s následnou aplikací HSC do druhostranné neembolizované větve vena portae. Výsledky: PVE s aplikací HSC proběhly bez komplikací. U všech nemocných došlo do 4 týdnů k dostatečnému nárůstu FLRV. Radikální resekce jater byla provedena u osmi nemocných s metastázami kolorektálního karcinomu. U dvou nemocných současně se zvýšením FLRV došlo i k významnému nárůstu objemu nádoru, resekce jater nebyla možná. U jednoho nemocného těžké nitrobřišní srůsty zabránily jaterní resekci. Jeden nemocný je čerstvě po aplikaci HSC. Celkové přežívání nemocných po dobu jednoho roku a tří let je 66,7 %, resp. 33,3 %. Medián přežití bez recidivy je 6 měsíců. Závěr: Kombinace PVE s aplikací HSC se jeví jako nadějná metoda pro zvýšení operability jaterních nádorů., Introduction: Insufficient future liver remnant volume (FLRV) is the main cause of liver tumors unresectability. Portal vein embolization (PVE) with application of hematopoietic stem cells (HSC) can be a matter how to increase resectability of liver tumors. Method: PVE with application of HSC were performed in 12 patients with primary unresectable liver tumors. Results: The procedures were without complications. After 4 weeks the sufficient FRLV grew. Radical liver resection was performed in 8 patients. Tumor progression was pattern in 2 patients and in one patients the sever intraabdominal adhesions were cause of inoperability. One patient is just after PVE and HSC application. Overall survival in one and three years is 66.7 %, resp. 33.3 %. Median of progression free survival is 6 months. Léčba karcinomu prostaty se v posledních letech stává stále složitější, převážně kvůli nástupu nových cytostatik, antiandrogenů a radiofarmak. Problém sekvenční léčby nebude pravděpodobně nikdy vyřešen, neboť se do léčebných schémat budou neustále zapojovat další preparáty. Vždy je nezbytné brát do úvahy preference pacienta, jeho výkonnostní stav a komorbidity a bohužel také i možnosti jednotlivých pracovišť. Skupiny pacientů vstupujících do klinických studií jsou pečlivě vybrané a neodpovídají realitě denní praxe. Multidisciplinární týmy za účasti urologa, klinického a radiačního onkologa a dalších odborností tak nabývají na jasném významu. V rámci konzilií je možné vybrat vhodnou léčebnou strategii individuálně pro každého pacienta, včetně možnosti účastnit se klinických zkoušek na pracovištích konzultantů. Abirateron a enzalutamid budou brzy schváleny i pro použití pro metastatický karcinom před aplikací chemoterapie s nespornou výhodou příznivého profilu nežádoucích účinků. Účinná cytotoxická léčba bude vždy k dispozici v případě selhání těchto antiandrogenů., Treatment of prostate cancer has become more complex in recent years, mainly due to the onset of new cytotoxic agents, antiandrogens and radiopharmaceuticals. The problem of sequential therapy will probably never be resolved, because there will always be new drugs involved in the therapeutic algorithms. It is always necessary to take into account the patient’s preferences, his performance status and comorbidities, and unfortunately also the possibilities of different departments. Groups of patients entering clinical trials are highly selected and do not reflect the reality of daily practice. Multidisciplinary teams with participation of urologist, clinical and radiation oncologists and other specialities are thus of great importance. The consultation can select an appropriate treatment strategy for each patient individually, including the possibility to participate in clinical trials at the respective departments. Abiraterone and enzalutamide will soon be approved for use in metastatic carcinoma before chemotherapy with indisputable advantage of a favourable side effects profile.
