Cílem článku je prezentovat současná doporučení k léčbě karcinomu močového měchýře po iniciální transuretrální resekci tumoru. Adjuvantní intravezikální léčba chemoterapií a imunoterapií prokazatelně snižuje recidivy karcinomů a v případě imunoterapie při udržovací dlouhodobé léčbě dokonce i oddaluje progresi onemocnění. Proto má svoji nezastupitelnou roli v komplexní péči pacientů s karcinomy měchýře neinfiltrujícími svalovinu., The aim of this article is to present current recommendations for treatment of bladder cancer after initial transurethral resection of tumor. Adjuvant intravesical treatment with chemotherapy and immunotherapy unequivocally lowers cancer recurrence and in case of maintenance immunotherapy even more prolongs the time to bladder cancer progression. Léčba chronické lymfocytární leukemie (CLL) dosáhla v posledních letech mimořádných pokroků. Důležitým mezníkem v léčbě CLL bylo zavedení monoklonálních protilátek a kombinované imunochemoterapie, které vedlo k významnému zlepšení léčebných výsledků a poprvé i k prodloužení celkového přežití. Navzdory těmto léčebným úspěchům je CLL stále považována za nevyléčitelné onemocnění. Pouze alogenní transplantace má potenciálně kurativní potenciál. Ta je však proveditelná pouze u malé, selektované skupiny mladších nemocných. S rozšiřujícími se poznatky o CLL a novými léčebnými možnostmi se snažíme léčbu nemocných maximálně individualizovat s přihlédnutím k věku nemocných, biologickým faktorům, přidruženým onemocněním a našim terapeutickým cílům. Vývoj nových léků a léčebných kombinací u CLL postupuje rychle vpřed a poskytuje tak naději na další zlepšování prognózy našich pacientů., The treatment of chronic lymphocytic leukemia (CLL) has achieved extraordinary progress over the last years. An important milestone in the treatment of CLL was reached with the incorporation of monoclonal antibodies and combined chemoimmunotherapy which led to significant improvements in treatment outcomes and survival advantage for the first time. Despite these therapeutic successes, CLL is still considered to be an incurable disease. Only the allogenic transplantation is potentially curative but it is feasible only for selected group of younger patients. There is an effort to individualize the treatment of patients with regard to the age of patients, biological factors, comorbidities and our therapeutic goals.
