Biologické cirkadiánní hodiny mají všichni obratlovci, včetně člověka. Jejich receptory jsou na oční sítnici, informace o střídání dne a noci jsou vedeny do centra v hypothalamu a odtud do epifýzy. Ta produkuje hormon melatonin (N-acetyl-5-metoxytryptamin), téměř výhradně v období tmavé části dne. Hormon je rychle vyplavován do krve a mozkomíšního moku a je bezprostředním nosičem informace o fázi cirkadiánního cyklu buňkám celého těla, které podřizují fyziologické funkce orgánů a organismu střídání dne a noci. Podle dosavadních výzkumů se melatonin účastní řady významných fyziologických funkcí: regulace spánku a cirkadiánních rytmů, nálady, imunitní, onkostatické a antioxidační aktivity. Syntéza melatoninu klesá s věkem. Umělé osvětlení večer nebo brzy ráno zkracuje tmavou část dne, také jen krátkodobé rozsvícení v noci redukuje množství melatoninu produkovaného během 24 hodin. Syntézu melatoninu významně narušuje letecký přesun přes několik časových zón. Zdravotní následky narušení biologických rytmů zahrnují poruchy spánku, zvýšený výskyt některých zhoubných nádorů, obezity, metabolického syndromu, diabetu, zhoršení projevů některých neurodegenerativních onemocnění. V primární prevenci je třeba omezovat noční směny a edukovat veřejnost o významu nočního spánku., All vertebrates, including humans, have a biological circadian clock. Their receptors are located on the retina, and information regarding the alternation of day and night are transferred to a centre in the hypothalamus and from there into the pineal gland which produces melatonin (N-acetyl-5-metoxytryptamin), almost exclusively during the hours of darkness. The hormone is quickly discharged into the bloodstream and cerebrospinal fluid into all cells. Such immediate information about the circadian phase of day/night changes allows to organs and organism to regulate their physiological functions. Current research suggests that melatonin is involved in several important physiological functions: sleep regulation and circadian rhythms, mood, immunity, oncostatic and antioxidant activities. Synthesis of melatonin reduces with age. Artificial light in the evenings and early mornings reduces the dark part of the day and even brief use of artificial light at night reduces the levels of melatonin produced over a 24-hour period. Air transport over a number of time-zones likewise significantly disrupts melatonin production. The health consequences of the disruption of biologcal rhythms include sleep disorders, higher frequency of some types of cancer, obesity, metabolic syndrome, diabetes and aggravation of some neurodegenerative symptoms. Malé regulační molekuly, tzv. mikroRNA, se podílejí na regulaci všech základních životních procesů v buňkách. Jsou regulátory exprese genů na posttranskripční úrovni. Ovlivňují tuto expresi vazbou na cílové mRNA, která vede k blokaci translace a štěpení mRNA. Nejnovější výzkumy ukázaly, že jejich výskyt není omezen jen na buňky, ale mikroRNA se vyskytují také v tělních tekutinách. Vzhledem k tomu, že diferenciální exprese určitých mikroRNA je často asociovaná s mnoha patologickými procesy, detekce deregulovaných mikroRNA v diagnosticky relevantních vzorcích (např. krvi, plazmě/séru, moči) se může stát základem nových diagnostických postupů. U gynekologických nádorových onemocnění je nalezení nových diagnostických biomarkerů důležitým cílem současných výzkumů. V tomto článku naleznete přehled cirkulujících a extracelulárních mikroRNA, které byly nejnověji nalezeny deregulovány u karcinomu ovaria, endometria a cervixu, především ve vzorcích krve, séra/plazmy a moči., Small, non-coding molecules of RNA, microRNAs, have been shown to participate in regulation of key biological processes in the cells. They regulate the expression at post-transcriptional level by binding to target mRNA, resulting in mRNA degradation or translation inhibition. The most recent investigations revealed that microRNAs may occur not only in the cells, but also in many body fluids. Differential expression of particular microRNAs is often associated with many pathological states. Therefore, the detection of deregulated microRNAs in diagnostically relevant samples such as blood, plasma/serum, or urine may become the basis for the establishment of novel diagnostic procedures. Finding alternative diagnostic biomarkers for gynecological cancers is the goal of many recent investigations. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. Winter climate determines the success of the two main reproductive strategies employed by aphids. Permanent parthenogens survive as parthenogenetic females in mild winters, but are regularly eliminated by low temperatures; while cyclical parthenogens, which switch to sexual reproduction by the end of summer, produce every year fertilised diapausing eggs resistant to frost. We have studied the variation in sexual morph production of several clones of the cereal aphid Rhopalosiphum padi (L.) showing both strategies. Twenty clones of this species differing by their geographic origin and their mode of reproduction were placed in two laboratory environments mimicking the changes of photoperiod and thermoperiod occurring naturally from the end of summer and during the autumn in oceanic and continental conditions. The analysis of clonal responses in both climatic conditions showed (i) a wide variation in investment of clones in sexual reproduction with, in particular, evidence for a mixed strategy employed by clones producing both sexuals without ceasing parthenogenetic reproduction, (ii) no geographic adaptation among clones belonging to cyclical parthenogenetic populations, (iii) an earlier production of sexuals in continental conditions and a higher production of males in oceanic conditions. Furthermore, we have compared the dates of first appearance of sexuals in our experiments with those occurring in the field based on a suction trap database and found that sexuals were caught in nature at least four weeks earlier than in the lab. Riziko CNS progrese u vysoce agresivních lymfomů (lymfoblastový a Burkittův lymfom) dosahovalo v historických souborech až 30%, podařilo se jej však výrazně snížit pomocí intratekální profylaktické terapie a inkorporací systémových cytostatik prostupujících hematoencefalickou bariéru do léčebných protokolů. U ostatních agresivních lymfomů je riziko CNS progrese nižší (pod 10 % v recentních studiích), avšak u vybrané skupiny nemocných se může i zde pohybovat kolem 25–30 %. Bohužel, na rozdíl od lymfoblastového a Burkittova lymfomu se u jiných agresivních histologií nepodařilo profylaktické úspěchy zopakovat. Tento článek se zabývá incidencí, rizikovými faktory a možnostmi profylaxe CNS relapsu u imunokompetentních pacientů s difuzním velkobuněčným B-lymfomem, lymfomem z buněk pláště, periferními T-lymfomy a primárním mediastinálním B-lymfomem. Zmíněny jsou rovněž možnosti terapie CNS progrese, pokud nastane., The risk of CNS progression in highly aggressive lymphomas (lymphoblastic, Burkitt‘s) reached up to 30% in historical cohorts, but it was significantly diminished by intrathecal prophylactic treatment and by incorporation of systemic cytostatics penetrating blood-brain barrier into treatment protocols. In other aggressive lymphomas, the risk of CNS progression is lower (under 10% in recent studies), but in selected patients can even here reach 25–30 %. Unfortunately, unlike in lymphoblastic and Burkitt‘s lymphomas, prophylaxis does not seem to be particularly effective in other histologies. This article discusses incidence, risk factors and prophylactic strategies in immunocompetent patients with diffuse large B-cell lymphoma, mantle-cell lymphoma, peripheral T-cell lymphomas and primary mediastinal B lymphoma.
