Cieľ: Cieľom štúdie bolo preskúmať, ako sestry vnímajú svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb (DSS) a identifikovať, či existujú významné vzťahy medzi úrovňou dôstojného prístupu k pacientom v DSS a demografickými znakmi (vek sestier, dĺžka praxe, vzdelanie a pod.). Metodika: Výskumný súbor tvorilo 106 sestier pracujúcich v domovoch sociálnych služieb v zriaďovateľskej pôsobnosti Trnavského samosprávneho kraja. Výskum prebiehal v období december 2011 až február 2012. Na zistenie úrovne dôstojného prístupu sme využili autorizovaný dotazník: Posudzovanie etických problémov – dôstojný prístup k pacientovi. Tvorí ho 5 subškál (informovanosť pacientov; autonómia; dodržiavanie dôvernosti a ochrany informácií; princíp spravodlivosti; intimita). Štatistická analýza bola realizovaná softvérom SPSS, verzia 15.0. Výsledky: Svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb hodnotili sestry ako primeranú. Medzi vnímanou úrovňou dôstojného prístupu sestier a demografickými údajmi boli nízke korelácie. Signifikantne vyššiu úroveň dôstojného prístupu sme zistili u sestier s vyšším vzdelaním (Bc.). Najnižšiu úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb sme zaznamenali v subškále zameranej na autonómiu pacienta. Záver: Na základe poznania týchto výsledkov môžeme podporovať zmeny, ktoré budú prispievať k zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti v domovoch sociálnych služieb., Aim: The aim of the study was to examine nurses’ perception of their own level of dignified approach to patients in social care homes and to identify potential statistically significant relationships between the level of dignified approach to patients in social care homes and demographic data (nurses’ age, duration of clinical experience, education etc.). Methods: The sample consisted of 106 nurses working in social care homes run by the Trnava Region. An authorized questionnaire called Assessment of Ethical Issues – Dignified Approach to a Patient was used to determine the level of dignified approach. The questionnaire consisted of five subscales (patient awareness; autonomy; respecting confidentiality; principle of justice; intimacy). Statistical analysis was carried out using the SPSS version 15.0 software. Results: The level of dignified approach to patients in social care homes was self-assessed by nurses as adequate. There was a low correlation between the perceived level of dignified approach to patients and demographic data. Nurses with higher education (bachelor’s degree) demonstrated significantly higher levels of dignified approach. The lowest level of dignified approach was found in the subscale focused on patient autonomy. Conclusion: Based on the knowledge of these results, changes may be promoted that will contribute to increased quality of nursing care provided in social care homes., Silvia Lelkešová, Ingrid Juhásová, and Literatura 19
Cíl: Cílem studie bylo zjistit spokojenost pacientů s poskytovanou ošetřovatelskou péčí na vybraných odděleních dvou nemocnic Moravskoslezského kraje. Porovnat spokojenost pacientů podle zdravotnického zařízení a oddělení, kde byli hospitalizováni. Zjistit, zda existuje souvislost mezi demografi ckými a klinickými charakteristikami pacientů a jejich spokojeností s ošetřovatelskou péčí. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time. Cíl: Zjistit vyuţívání ošetřovatelských intervencí na chirurgických pracovištích neintenzivní péče a chirurgických jednotkách intenzivní péče. Porovnat uţití intervencí mezi uvedenými typy pracovišť a porovnat získané výsledky se závěry výzkumného týmu NIC. Metodika: Informace byly získány kvantitativní výzkumnou metodou za vyuţití dotazníku vlastní konstrukce. Dotazník byl sestaven s vyuţitím Nursing Intervention Classification (NIC) a obsahoval názvy i definice 101 vybraných intervencí klasifikace NIC. Sestry se prostřednictvím dotazníku u intervencí vyjadřovaly k četnosti uţití v chirurgické klinické ošetřovatelské praxi na jejich pracovišti. Pro induktivní analýzu byl pouţit Pearsonův chí–kvadrát test na hladině významnosti α = 0,05. Výsledky: Šetření se zúčastnilo 11 zdravotnických zařízení, a bylo vyhodnoceno 255 dotazníků vyplněných sestrami, které pracovaly na chirurgických pracovištích neintenzivní péče, a 45 dotazníků vyplněných sestrami, které pracovaly na chirurgických ošetřovatelských jednotkách poskytujících intenzivní péči. Padesát intervencí bylo vyhodnoceno jako stejně často vyuţívané na pracovištích neintenzivní péče i jednotkách intenzivní péče, 49 intervencí jako častěji vyuţívaných na chirurgických jednotkách intenzivní péče a 2 intervence byly vyhodnoceny jako častěji uţívané na chirurgických pracovištích neintenzivní péče. Závěr: V šetření jsme zjistili rozdílné uváděné četnosti uţití intervencí na chirurgických odděleních neintenzivní péče a pracovištích intenzivní péče. Z výsledků je patrné, ţe u nejvyššího počtu testovaných intervencí jsme neshledali rozdíl v četnosti uţití mezi pracovišti intenzivní a neintenzivní péče. V porovnání s výsledky šetření, které uskutečnil výzkumný tým NIC, jsme však zjistili, ţe sestry často vykonávají na chirurgických odděleních neintenzivní péče intervence, které byly výzkumným týmem NIC vyhodnoceny jako více uţívané na jednotkách intenzivní péče., Aim: Determine the frequency use of nursing interventions to non-intensive surgical departments and surgical intensive care unit (ICU). Compare the frequency of use of interventions between these departments and compare the results with the findings of the research team NIC. Methods: Informations obtained quantitative research method using a questionnaire. The questionnaire contained 101 names and definitions of selected nursing interventions classification (NIC). Nurses in these interventions reflect the frequency of their use in surgical clinical nursing practice. For inductive analysis was used Pearson's chi-square test at a significance level of α = 0.05. Results: The survey was attended by 11 hospitalis and were evaluated used 255 questionnaires completed by general nurses who worked at non-intensive surgical departments and 45 questionnaires completed general nurses who worked in intensive surgical departments. It was found 50 interventions that as often used on non-intensive department and intensive department, 49 interventions that are more ofen used in surgical intensive care and 2 interventions that are more ofen used in non-intensive surgical departments. Conclusion: In the survey, we have found different declarations frequency use of interventions between non-intensive surgical department and intensive care units. The results show that the highest number of tested interventions were no differences in frequency of use between the intensive and non-intensive care. Cíl: Cílem studie bylo vyhledat poznatky týkající se NANDA International ošetřovatelské diagnózy Porucha polykání (00103) se zaměřením na výskyt této ošetřovatelské diagnózy u různých skupin pacientů a její stanovení prostřednictvím znaků uvedených v klasifikačním systému dle NANDA International. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system.
