Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. The aim of this study is to retrospectively evaluate the effectiveness of intraluminal high dose rate brachytherapy in the palliative treatment of tumorous esophageal stenoses. Patients and methods: Between February 1996 and June 2011 intraluminal brachytherapy was performed in 41 patients with esophageal carcinoma (squamous cell carcinoma in 19 cases and adenocarcinoma in 22 cases). All patients had dysphagia at presentation. Brachytherapy was performed using high dose rate afterloading system. Results: Dysphagia was improved in majority of patients. The median survival was 396 days (95 % CI: 270–492 days). No mechanical complication was observed during introduction of the applicator. Brachyterapie je metoda léčby zářením, při které se radioaktivní zdroje zavádějí přímo do oblasti nádoru. Umožňuje aplikaci vysoké dávky záření v krátkém čase a dosažení vysokého stupně lokální kontroly nádoru. Největší klinická zkušenost byla získána s brachyterapií s kontinuálním nízkým dávkovým příkonem (0,4–2,0 Gy/hod), kdy se zářiče zaváděly manuálně. V současně době byla tato metoda nahrazena na většině pracovišť automatickými afterloadingovými přístroji využívajícími vysoký dávkový příkon (> 12 Gy/hod) a frakcionaci. U časných ORL nádorů lze užít brachyterapii jako samostatnou léčebnou metodu. Její výhodou v porovnání s chirurgickou léčbou je většinou lepší kosmetický a funkční efekt, v porovnání se zevním ozařováním lepší kontrola nádoru a eliminace postradiační xerostomie. U rozsáhlejších nádorů je brachyterapie indikována k navýšení dávky po zevní radioterapii. Lze ji použít i pro léčbu recidiv v ozářeném terénu. Klasickými indikacemi jsou nádory dutiny ústní, orofaryngu a nasofaryngu. Přes současný technologický pokrok metod zevní radioterapie zůstává brachyterapie důležitou léčebnou modalitou u nádorů ORL., Brachytherapy is a method of radiation treatment, when the radioactive sources are directly applicated into the tumor region. Brachytherapy allows to deliver high doses to the tumor in a short time with excellent local tumor control. The largest experience was achieved with continual low dose rate brachytherapy (0.4–2Gy/hr) and manual insertion of sources. Recently this method was mostly replaced by automatic afterloading devices and high dose rate (> 12Gy/hr) fractionated treatment. Brachytherapy can be used in early stages of head and neck cancer as a sole treatment method. The advantage in comparison with surgery is a better functional and cosmetic outcome and in comparison with external beam radiotherapy a better tumor control and absence of xerostomia. In locally advanced tumors brachytherapy can be used for dose escalation of external beam radiotherapy. Brachytherapy is also useful for salvage treatment of recurrences after previous radiotherapy. Classical indications are oral cancer, oropharyngeal and nasopharyngeal tumors.
