Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). A total of 711 patients (17.0%) receiving LCZ696 and 835 patients (19.8%) receiving enalapril died (hazard ratio for death from any cause, 0.84; 95% CI, 0.76 to 0.93; P < 0.001); of these patients, 558 (13.3%) and 693 (16.5%), respectively, died from cardiovascular causes (hazard ratio, 0.80; 95% CI, 0.71 to 0.89; P < 0.001). As compared with enalapril, LCZ696 also reduced the risk of hospitalisation for heart failure by 21% (P < 0.001) and decreased the symptoms and physical limitations of heart failure (P = 0.001). The LCZ696 group showed higher proportions of patients with hypotension and non‑serious angioedema but lower proportions of patients with renal impairment, hyperkalemia, and cough when compared to the enalapril group. Conclusions: LCZ696 was more effective than enalapril in reducing the risk of death and of hospitalisation for heart failure. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Na nedávném kongresu Evropské urologické asociace (EAU) ve švédském Stockholmu byla prezentována data, která jednoznačně ukazují na to, že se lékaři při léčbě příznaků syndromu dolních cest močových u mužů až příliš soustřeďují na evakuační (obstrukční) symptomy a opomíjejí příznaky jímací (iritační), které zřejmě ještě negativněji ovlivňují kvalitu života jejich pacientů.
