Náplní sdělení je aktuální informace o přínosu nové, vysoce senzitivní metody analýzy imunoglobulinů séra (Hevylite™), umožňující separátní kvantitativní vyšetřování párů těžkých/lehkých řetězců imunoglobulinu (HLC), tj. monoklonálního (involved) a polyklonálního (noninvolved) izotypu včetně jejich poměru (HLC-r) u monoklonálních gamapatií. Hlubší pozornost je věnována východiskům a charakteristice této analytické metody, srovnání jejího klinického přínosu se standardními technikami používanými v diagnostice, sledování průběhu, časné detekce relapsu a progrese nemoci, stratifikace, hodnocení léčebné odezvy a prognózy monoklonální gamapatie nejistého významu, mnohočetného myelomu, Waldenströmovy makroglobulinemie, systémové AL-amyloidózy a některých nehodgkinských lymfomů. Z dosavadních výsledků vyplývá, že ve srovnání s dosavadními standardně používanými metodami obohacuje metoda Hevylite™ klinickou praxi o vyšetření hladiny noninvolved Ig s možností vyhodnocení hloubky imunoparézy, stanovení indexu HLC-r uplatňujícího se při stratifikaci MM do rizikových skupin a přispívajícího k odhadu prognózy a zlepšení hodnocení hloubky léčebné odezvy, zatímco v případě MGUS poskytuje informaci o riziku maligní evoluce. Ke zpřesnění indikací metody Hevylite™ u MG v běžné klinické praxi zřejmě přispěje zveřejnění výsledků aktuálně probíhajících validačních studií., The aim of the paper is to inform about the contribution of novel, highly sensitive analytic technique for the assessment of serum immunoglobulins (Hevylite™), enabling separate quantitative assessment of heavy/light chain pairs of immunoglobulin (HLC), i. e. the monoclonal („involved“) and polyclonal („noninvolved“) isotype including their ratio (HLC-r) in monoclonal gammopathies. We particularly target the characteristics of this technique, the comparison of its clinical contribution with standard methods used in the diagnostics, course and the detection of relapse and progression of the disease, as well as the stratification, assessment of therapeutic outcome and prognosis in monoclonal gammopathy of undetermined significance, multiple myeloma, Waldenström´s macroglobulinemia, systemic AL-amyloidosis and some non-Hodgkin lymphomas. Present results show that in comparison with existing routinely used techniques the Hevylite™ method enriches clinical practice with the assessment of serum levels of „uninvolved“ Ig. It enables the evaluation of the depth of „immunoparesis“, and the determination of HLC-r index that is needful for the stratification of MM into „risk cohorts“. It also contributes to prognostic assessment and improvement of the evaluation of the depth of therapeutic response. In MGUS individuals the HLC-r index provides information about the risk of malignant transformation. Současné poznatky moderně chápané nutriční imunologie mají zásadní význam z hlediska aktivit v oblasti ochrany a podpory zdraví. Existuje nespočet dokladů, že neadekvátní nebo snížený nutriční příjem se neodráží jen klasickými příznaky proteinové a energetické malnutrice, ale také nepříznivým ovlivněním až selháváním imunitních funkcí. Stále rostoucí počet in vitro a in vivo experimentálních a klinických studií dokládá, že produkty neenzymatických reakcí, které vznikají při technologickém a kulinárním zpracování potravin mohou hrát významnou roli při vzniku a globálním šíření chronických nesdělných onemocnění. Prokazuje se, že hlavní patogenetický význam mají tzv. konečné produkty pokročilé glykace (AGE, Advanced Glycation End products), lipoxidace (ALE, Advanced Lipoxidation End products), a oxidace proteinů (AOPP, Advanced Oxidation Protein Products), které představují nejdůležitější nutriční složky kumulující se ve vnitřním prostředí organismu v průběhu jeho celého života. V současnosti jsou považovány za nejčastější složky poškozující zdraví a zároveň představující jednu z významných příčinu růstu incidence chronických nesdělných nemocí ve všech zemích, které přijaly "západní" životní styl., The recent results of research in the field of nutritional immunology have a wide impact on health protection and promotion-related activities. There is a great deal of evidence showing that the inadequate or lowered nutritional intake is reflected in both protein-energetic malnutrition and immune system disorders. A growing number of in vitro/in vivo experimental and clinical studies documents that the products of non-enzymatic reactions formed during technological and culinary processing of food may play a major role in the emergence and global spread of chronic non-communicable diseases. Evidence mainly points to the pathogenic role of advanced glycation end products (AGE), advanced lipoxidation end products (ALE), and advanced oxidation protein products (AOPP), which represent the most important dietary compounds that accumulate within the internal milieu of an organism over a whole life span. Zlomeniny lopatky představují otevřenou kapitolu traumatologie pohybového aparátu. Jejich pochopení není možné bez základních anatomických a klinických znalostí. Značná část zlomenin lopatky je sdružena s dalšími závažnými poraněními, především hrudníku. Zásadní pro diagnostiku a léčebný postup je rtg vyšetření, především obě Neerovy projekce a 3D CT rekonstrukce. Dosud používané klasifikace je třeba revidovat, neboť nereflektují reálné typy zlomenin. U dislokovaných zlomenin je nutno zvážit operační léčbu. Ta není urgentní a lze ji provést až do tří týdnů po primárním úrazu. Vzhledem k tomu, že jde o závažný, ale řídký typ poranění, je lépe tyto zlomeniny koncentrovat na specializovaná pracoviště., Scapular fractures are still a challenge in traumatology of the musculoskeletal apparatus. Their proper understanding is impossible without fundamental anatomical and clinical knowledge. A considerable part of scapular fractures is associated with other severe injuries, particularly to the chest. Essential for diagnosis and treatment of these fractures is radiographic examination, primarily both Neer projections and 3D CT reconstructions. The classifications used so far should be revised as they do not reflect real types of these fractures. Operative treatment should be considered in displaced scapular fractures. Such treatment is not urgent as these fractures may be operated on within up to three weeks of the primary injury. Zásah porušené regulace metabolizmu glukózy do metabolizmu kostní tkáně je mnohostranný a velmi komplikovaný. Ve většině případů nacházíme u diabetiků zvýšené riziko zlomenin. Důvody, které k tomuto stavu vedou, jsou mnohočetné. U diabetiků nebývá hlavní příčinou pokles kostní minerální hustoty, spíše jde o snížení kvality kostní tkáně a změnu její struktury. Významnou roli hraje nepochybně inzulin a inzulinová rezistence, uplatňuje se ale i zvýšená glykace proteinů, včetně zvýšené glykace kolagenu v kostech, změna produkce a hladiny sklerostinu, zásah do diferenciace pluripotentních kmenových buněk a zvrat jejich diferenciace směrem k adipocytům a mnohé další faktory. Nemalý podíl na zvýšeném riziku fraktur mohou mít i některá antidiabetika, především perorální. Vzhledem ke stoupající incidenci obou chorob, diabetu i osteoporózy, bude nezbytné podrobněji zkoumat i jejich vzájemné vazby a ovlivnění. In most cases we observe an increased risk of fracture in people with diabetes. The reasons causing this condition are varied. Its main cause in diabetic patients is typically not the decrease in bone mineral density, it is rather deterioration of bone tissue and its structure. An important role of insulin and insulin resistance is beyond dispute, with numerous other factors at work, such as increased glycation of proteins, including increased glycation of collagen in bones, change in sclerostin production and levels, intervention in the pluripotent stem cells differentiation and reversal of their differentiation toward adipocytes and many more. Some antidiabetics, mainly oral, may also significantly contribute to the increased risk of fracture.
