Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. Cíl: Cílem práce bylo shromáždit dostupná klinická, radiologická a patologická data o kalcifikujícím pseudotumoru nervové osy (CAPNON). Soubor: Soubor je tvořen kazuistikou a současně přehledem všech pacientů publikovaných v anglické literatuře, vyhledaných v databázi PubMed pomocí klíčových slov: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi a fibro‑osseous lesion. Zahrnuty jsou taktéž všechny citace případů v nalezených publikacích. Výsledky: Včetně naší pacientky jsme v literatuře nalezli celkem 72 pacientů. Intrakraniálně bylo lokalizováno 62 % těchto lézí, v páteřním kanále 31 % a v kranio‑cervikálním přechodu zbylých 7 %. Průměrný věk výskytu je 45,6 (rozmezí 2–83) let. U intrakraniální lokalizace byla tato léze nalezena supratentoriálně v 74 % a infratentoriálně v 26 %. Z nejčastějším projevů intrakraniálních CAPNON jsme nalezli epilepsii v 33,3 %, bolesti hlavy v 24 %, nebo některý z příznaků zadní jámy v 16,7 %. U pacientů s epilepsii byl CAPNON nalezen v temporálním laloku v 50 %. U CAPNON nalezených v páteřním kanále byla dominantním projevem bolest v 82 %, následovaná poruchou chůze v 27 %. V kranio‑cervikálním přechodu byla dominantním příznakem bolest v 80 %. Závěr: CAPNON je velmi vzácnou, benigní, pomalu rostoucí lézí centrální nervové soustavy. Ve většině případů ji nalezneme intrakraniálně s lehkou převahou u mužského pohlaví. Původ této léze není zcela jasný. Diagnóza CAPNON by měla být zvažována setkáme‑li se s nálezem solidně kalcifikované léze při vyšetření CT mozku a hypodenzní léze v T1 a T2 obraze při vyšetření MR. Léčebnou modalitou je radikální odstranění této léze. Incidentální léze musí být monitorovány, jelikož mohou růst a stát se symptomatickými. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Aim: The aim of this study was to summarize clinical, radiological and pathological data on calcifying pseudoneoplasm of the neural axis (CAPNON). Methods: A case report of a female patient with CAPNON is presented. In addition, all CAPNON patient case histories published in English language are included. Articles were identified via PubMed searches using the following key words: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi and fibro‑osseous lesion. All cases referenced within the identified publications are also included in this review. Results: Seventy two cases were identified, including our case report. Of these, 62% were intracranial, 31% spinal and 7% were located in the cranio‑cervical junction (CCJ). The mean age of the CAPNON patients was 45.6 (range 2–83) years. 74% of intracranial CAPNON were supratentorial and 26% were infratentorial. Symptoms of intracranial CAPNON included epilepsy in 33.3%, headaches in 24% and posterior fossa symptoms in 16.7%. The majority of patients with epileptic seizures had lesions in the temporal lobe (50%). Pain was the dominant symptom in 82% and gait disorder in 27% of spinal CAPNON cases. Pain was the dominant symptom (80%) in CCJ CAPNON. Conclusion: CAPNON is a rare, benign, slowly‑growing lesion of the central nervous system. In the majority of cases, location is intracranial and there is a slightly higher prevalence in men. The origin of CAPNON remains unclear. CAPNON should be considered whenever a CT scan reveals a calcified lesion combined with hypointensity on T1 and T2-weighted MRI. Radical removal is the treatment of choice. Úvod: V roce 2016 to bude 60 let od doby, kdy Nissen poprvé publikoval svoji operační metodu 360° fundoplikace kolem distálního jícnu a její výsledný efekt. V současnosti je Nissenova fundoplikace pravděpodobně nejčastěji používanou metodou k chirurgické léčbě gastroezofageálního refluxu (GER) a hiátové hernie (HH). Incidence HH je v západní populaci mezi 25–50 %. Současná prevalence GER je v Evropě kolem 4–10 %, v Severní Americe 5–7 %, ale v některých studiích je tato prevalence udávána dokonce kolem 10–20 %. Metody: Na Chirurgické klinice 2. LF UK a ÚVN bylo v období 2010 až 2015 provedeno 157 laparoskopických operací pro hiátovou hernii nebo refluxní nemoc jícnu (GERD). Většina nemocných byla indikována pro symptomatickou HH s/bez GERD, jen minimum nemocných bylo indikováno pro GERD bez HH. V souboru bylo 73 mužů a 84 žen s průměrným věkem 53,9 roku v rozmezí 20–88 let. Z tohoto počtu mělo 11 nemocných syndrom hrudního žaludku. U 109 nemocných (69,4 %) byla provedena 360° fundoplikace dle Nissena (NFP) a u 48 nemocných (30,6 %) byla provedena 270° fundoplikace dle Toupeta (TFP). Po operaci byli nemocní ambulantně kontrolováni v intervalu 3 měsíce, 6 měsíců a 12 měsíců a dále po roce. Prospektivně byly sledovány příznaky pyrózy, dysfagie, možnosti či nemožnosti odříhnutí, meteorismu nebo výskyt recidivy, získaná data byla zkoumána retrospektivně. Pro vyhodnocení získaných dat byla použita metoda deskriptivní statistiky a explorační jednosměrné analýzy. Výsledky: V období 2010 až 2015 bylo provedeno 157 laparoskopických operací pro hiátovou hernii nebo GERD. V souboru se nevyskytlo úmrtí ve vztahu k provedené operaci. Tři měsíce po operaci došlo jak u pacientů po NFP, tak u pacientů po TFP k výraznému poklesu až vymizení symptomů refluxu, příznaků pyrózy a epigastrické bolesti. Pooperační dysfagické obtíže vymizely ve skupině po NFP do 4–7 týdnů, u nemocných po TFP do 3–5 týdnů. V intervalu 2–5 let po operaci došlo celkem u 36 (22,9 %) nemocných k recidivě obtíží. U 22 nemocných (14 po NFP, 8 po TFP), u kterých byla prokázána endoskopicky nebo RTG vyšetřením recidiva refluxu bez recidivy kýly, byla zahájena léčba inhibitory protonové pumpy s velmi dobrým efektem. U 14 nemocných s anatomickou recidivou hiátové hernie s/bez refluxu (8 po NFP, 6 po TFP) byla provedena laparoskopicky rehiátoplastika a refundoplikace dle Nissena. Závěr: Laparoskopická chirurgická antirefluxní léčba GERD a HH je bezpečná metoda s velmi nízkou perioperační morbiditou a s minimální mortalitou. NFP nebo TFP ve stejné míře dlouhodobě obnovují mechanickou bariéru proti regurgitaci žaludečního obsahu do jícnu se signifikantním efektem na vymizení příznaků GERD a HH a s výrazným zlepšením kvality života nemocných. U obézních nemocných je před operací nutná redukce hmotnosti. U morbidně obézních nemocných s HH indikovaných k bariatricko-metabolické operaci je doporučováno provést hiátoplastiku současně s bariatrickým výkonem., Introduction: In 2016 60 years will have elapsed from the first publication by Nissen about his surgical method of 360° fundoplication around distal esophagus and its final effect. Nissen’s fundoplication is currently the most common surgical method for the gastroesophageal reflux (GER) and hiatal hernia (HH) probably. Incidence of HH ranges between 25–50% in the western population. Current prevalence of GER ranges between 4–10% in Europe and 5–7% in North America, but some studies report even 10–20%. Methods: From 2010 to 2015, 157 laparoscopic procedures for HH or gastroesophageal reflux disease (GERD) were done at the Department of Surgery, the Second Faculty of Medicine, Charles University and Central Military Hospital in Prague. Most of the patients were operated because of symptomatic HH with/without GERD. Only very few patients were operated only for GERD without HH. The group of the patients included 73 men and 84 women with the mean age 53.9 years in the range 20–88 years. 11 patients of the group were operated because of the upside-down stomach syndrome. 109 (69.4%) patients underwent Nissen’s 360° fundoplication (NFP) and 48 (30.6%) patients underwent Toupet’s 270° fundoplication (TFP). The patients were followed after 3 month, 6 month, 12 month and then once a year after the surgery. Symptoms such as heartburn, dysphagia, belching, bloating or recurrence of HH or GERD were collected prospectively and analyzed retrospectively. Methods of descriptive statistics and explorative unidirectional analysis were used. Results: From 2010 to 2015, 157 laparoscopic procedures were done for HH or GERD. No death occurred in the group of the patients in connection with the surgery. Similar rates of considerable improvement or resolution of the symptoms such as the reflux, heartburn, and epigastric pain were observed after three months from the surgery among patients after NFP or TFP. Postoperative dysphagia resolved in 4–7 weeks in the group after NFP, and in 3–5 weeks in the group after TFP. Disease recurrence was observed in 36 (22.9%) patients in 2 to 5 years after the surgery. Recurrence of only the gastroesophageal reflux without HH was observed in 22 patients (14 after NFP; 8 after TFP), shown by endoscopy or x-ray assessment, with a satisfactory response to PPI medication. Laparoscopic rehiatoplasty and refundoplication according to Nissen were performed in 14 patients with HH recurrence with/without the reflux (8 after NFP; 6 after TFP). Conclusion: Laparoscopic antireflux surgery of GERD and HH is safe and is associated with very low perioperative morbidity and minimal morbidity. Both NFP and TFP have a similar effect on restoration of the mechanical gastroesophageal barrier against gastric reflux, with significant effect on resolution of GERD and HH symptoms and with a marked improvement of the patient’s quality of life. As regards morbid obese patients, weight reduction is needed before the surgery. Úvod: Perkutánní punkční cholecystostomie je považována za výkon v nouzi při selhávání konzervativní léčby těžké akutní cholecystitidy u starých a kriticky nemocných pacientů. Otázkou je, nakolik je v současnosti toto řešení aktuální či obsoletní. Metody: Retrospektivně byla analyzována data pacientů, kteří podstoupili v období 1/2010−1/2015 punkční cholecystostomii pod kontrolou počítačové tomografie (CT). Sledována byla data pacientů, úspěšnost a komplikace výkonu, krátkodobé a dlouhodobé výsledky. Výsledky: Punkční cholecystostomie pod CT navigací byla na Chirurgické klinice 2. LF UK a FNM za sledované období provedena u 30 pacientů. V souboru bylo 21 žen (70 %), 9 mužů (30 %), průměrný věk 78 let (SD?12,3), medián 82 let (34–93 let). Indikací výkonu byla těžká akutní cholecystitida, respektive empyém žlučníku nereagující na konzervativní léčbu u rizikových pacientů aktuálně neúnosných pro operační výkon. Primárně byla punkce úspěšná u 23 pacientů (77 %), selhala u 7 pacientů (23 %) s nutností opakované punkce u jedné pacientky a urgentní cholecystektomie u 6 nemocných. Průměrná doba hospitalizace byla 16,5 dne (SD?8,2), medián 15 dnů (7–49 dnů). Celková 30 denní mortalita byla 17 %, mortalita v souvislosti s primární indikací k výkonu byla 10 %. Recidivu onemocnění jsme zachytili u 3 pacientů (10 %). U jednoho byla řešena opakovanou drenáží, u druhého konzervativní terapií a u třetího akutní cholecystektomií. U jednoho pacienta (3 %) byla provedena odložená laparoskopická cholecystektomie bez komplikací. Závěr: Perkutánní cholecystostomie pod CT kontrolou u pacientů neúnosných k operaci představuje efektivní a bezpečnou terapeutickou možnost., Introduction: Percutaneous cholecystostomy is considered to be an emergency treatment option when conservative treatment of acute cholecystitis fails in elderly and critically ill patients. The question is: to what extent is this technique still up-to-date or obsolete. Methods: We retrospectively reviewed data of patients who underwent a computer tomography (CT) guided percutaneous cholecystostomy between 1/2010−1/2015. We analyzed the patient data, the success rate, complications of the procedure, short- and long-term outcomes. Results: 30 patients undergoing CT-guided percutaneous cholecystostomy at the Department of Surgery, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital during the study period were enrolled. The study group included 21 females (70%) and 9 males (30%) with mean age of 78 years (SD?12.3), median 82 years (range 34−93 years). Percutaneous cholecystostomy was indicated for patients with severe cholecystitis/empyema of the gallbladder not responding to conservative therapy who were poor candidates for operative cholecystectomy. Of these, 23 patients (77%) were successfully treated with initial percutaneous cholecystostomy whereas 7 patients (23%) experienced treatment failure – one was subsequently successfully treated with repeated percutaneous cholecystostomy and six underwent emergency cholecystectomy. The mean length of stay was 16.5 days (SD ?8.2), median 15 days (7–49 days). The total 30-day mortality was 17%, and indication-related mortality was 10%. Three patients (10%) had a recurrence. One patient required repeated percutaneous drainage, the second recovered on conservative treatment and the third patient underwent acute cholecystectomy. Only one patient (3%) underwent delayed laparoscopic cholecystectomy without complications. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Historical treatment options have been limited (e.g. dacarbazine, temozolomide, high dose interleukin-2) and associated with poor outcomes. Vemurafenib is a first-in-class, small molecule BRAFV600 inhibitor. Vemurafenib is approved in the EU as monotherapy in adult patients with BRAFV600 mutation-positive unresectable or metastatic melanoma. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Fyloidní tumor je velmi zřídka se vyskytující nádor prsu. Vyskytuje se pouze v 1 % případů všech nádorů prsu. Diagnostika fyloidního tumoru může být obtížná vzhledem k malému počtu případů. Léčba fyloidního tumoru je vždy chirurgická. Metody: V letech 2004−2013 jsme na Chirurgické klinice FN Plzeň operovali 12 pacientek s fyloidním nádorem prsu z celkového počtu 1564 operací pro nádory prsu (0,8 %). Hodnotili jsme věk, biologické chování nádoru v závislosti na velikosti nádoru a délce trvání, vzdálené metastázy, terapii a přežití. Výsledky: Průměrný věk v době operace byl 50 let (26−84), délka trvání nemoci do operačního řešení byla 1 měsíc až 10 let. Velikost tumoru se pohybovala v rozmezí 2−29 cm, tumory menší než 5 cm byly vždy benigní. 7x pro benigní fyloidní nádor byla provedena excize tumoru. 5x byl diagnostikován maligní fyloidní tumor a byla vždy provedena mastektomie, ve 3 případech s exenterací axily, kdy uzliny byly vždy benigní. V jednom případě po mastektomii následovala radioterapie pro dosahování nádoru do okraje resekátu, ostatní pacientky byly pouze dispenzarizovány. 2 pacientky měly generalizaci do plic diagnostikovanou 5 a 10 měsíců po operaci prsu. 1 pacientka s generalizací zemřela, ostatní žijí bez lokální recidivy onemocnění. Medián přežití je 52 měsíců, interval bez nemoci je 50 měsíců. Závěr: Z našeho souboru vyplývá, že pokud je fyloidní nádor diagnostikován včas, jedná se téměř výhradně o benigní variantu, při déle trvající anamnéze a s rostoucí velikostí tumoru nádory malignizují. Chirurgická léčba je dostačující i v případě maligních forem. Výkon na prsu není třeba doplňovat o exenteraci axily., Introduction: Phyllodes tumour is a breast tumour occurring very rarely. It accounts for only in 1% of all cases of breast tumour. The diagnosis of phyllodes tumours can be difficult in consideration of the small number of cases. Treatment of phyllodes tumours is always surgical. Methods: In 2004–2013, we operated on twelve female patients with phyllodes tumours out of the total number of 1564 surgeries for breast tumours (0.8%) at the Department of Surgery at Teaching Hospital in Pilsen. We evaluated the age, the biological behaviour of the tumour depending on the tumour size and duration, the distant metastases, therapy and survival. Results: The average age at the time of surgery was fifty years (26−84), the duration of disease to the surgical solution ranged from one month to ten years. Tumour size was in the range of two to twenty-nine centimetres, tumours measuring less than five centimetres were always benign. Tumour excision for benign phyllodes tumour was performed seven times. Malignant phyllodes tumour was diagnosed five times with mastectomy performed in each case, and the axilla was exenterated in three cases where nodes were benign in each of them. In one case, mastectomy was followed by radiotherapy because the tumour reached the edge of the resected part; the other patients were only monitored. In two patients, tumour spreading into the lungs was diagnosed at five to ten months after breast surgery. One patient with generalized disease died, the other ones live with no local recurrence of this disease. Median survival is fifty-two months; the disease-free interval is fifty months. Conclusion: The results show that if phyllodes tumour is diagnosed in time, it is almost exclusively benign. If the case history is longer and the tumour is growing, the likelihood of malignancy increases. Surgical treatment is also sufficient in the case of malignant forms. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Sleep disturbances are one of the most common non-motor symptoms in patient with Parkinson’s disease (PD) with community‑based studies reporting prevalence data of 60%. Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05.
