Cíl: Porovnání analýzy T1 mapování myokardu u osob s progresivní systémovou sklerózou (SSc) a kontrolním souborem zdravých dobrovolníků v rámci předběžných výsledků prospektivní studie zaměřené na detekci difuzní myokardiální fibrózy. Metodika: Třicet tři osob sledovaných pro SSc a 20 zdravých dobrovolníků bylo zařazeno do prospektivní studie a podstoupily MR vyšetření, echokardiografické vyšetření a stanovení laboratorních parametrů. Studie byla schválenou lokální etickou komisí a účastníci podepsali informovaný souhlas. V rámci komplexního MR vyšetření byly provedeny sekvence pro analýzu T1 relaxačního času (T1 mapování) před a po intravenózním podání Gd kontrastní látky s následnou kalkulací extracelulárního objemu (ECV). V první fázi bylo provedeno porovnání výsledků mezi SSc a kontrolním souborem. Výsledky: Při hodnocení nativních T1 map byla prokázána statisticky významně (p < 0,0001) vyšší hodnota u SSc souboru (průměr 1259,3 ? 51,2) v porovnání s kontrolním souborem (1203,6 ? 29,9). Obdobně statisticky významný (p < 0,0001) rozdíl byl prokázán při porovnání vypočtených hodnot ECV, kdy u souboru SSc byla prokázána vyšší průměrná hodnota (28,6 ? 4,9) než u kontrolního souboru (23,3 ? 2,1). V případě T1 hodnot po intravenózním podání Gd kontrastní látky byl také prokázán rozdíl průměrných hodnot, nicméně s pouze malou statistickou významností (p = 0,026), kdy průměrná T1 hodnota u SSc souboru klesla na 586,6 ? 64 a u kontrolního souboru na 639,4 ? 38. Závěr: Předběžné výsledky prospektivní studie prokázaly statisticky významně vyšší hodnoty T1 mapování a ECV u osob s diagnostikovanou systémovou sklerózou v porovnání s kontrolním souborem zdravých dobrovolníků. Tyto výsledky potvrzují přínos použité metodiky v posouzení difuzní fibrózy myokardu, což bude hlavní oblastí zájmu v další fázi studie. Methods: 33 SSc patients and 20 healthy volunteers were enrolled in a prospective study and underwent MR examination, echocardiographic examination and determination of laboratory parameters. The study was approved by the local ethics committee and the participants signed an informed consent. Within the complex MR examinations were performed sequences for analysis of T1 relaxation time (T1 mapping) in myocardium prior and after intravenous administration of Gd contrast agents with subsequent calculation of extracellular volume (ECV). Comparison of results between SSc and control group was performed in first phase of assessment. Results: Statistically significant (p < 0.0001) higher native T1 values was proved in SSc group (mean 1259.3 ? 51.2) compared with the control group (1203.6 ? 29.9). Similarly, statistically significant (p < 0.0001) difference was detected in calculated ECV (%) when in a group of SSc was demonstrated higher mean value (28.6 ? 4.9) than in the control group (23.3 ? 2.1) . In the case of T1 values after intravenous administration of Gd contrast agents has been also demonstrated a difference of average values, however, with only a small statistical significance (p = 0.026), when the average value of T1 in SSc group decreased to 586.6 ? 64 and control file to 639.4 ? 38th. Conclusions: Preliminary results of a prospective study showed significantly higher native T1 values and ECV in patients with systemic sclerosis compared with a control group. These results confirm the contribution of the methodology used in the assessment of diffuse fibrosis of the myocardium, which will is the main focus of the study. Cíl. Shrnout dostupné informace a analyzovat zobrazené nálezy na námi zachycených běžných i méně běžných případech tarzální koalice. Budeme se zabývat primárními i sekundárními nálezy při vlastním zobrazení i korelací s literaturou, udávanými histologickými nálezy, zobrazovacími metodami a protokoly, diferenciální diagnózou a krátkou informací o klinickém nálezu a léčbě. Metody. Vyšetřili jsme osm pacientů s různými typy tarzální koalice - s koalicí talokalkaneální mediální i dorzální, s koalicí kalkaneonavikulární, s oboustrannou talokalkaneální koalicí i s koalicí talokalkaneokuboidní, která byla součástí komplexnější dysplazie. Nálezy jsou analyzovány především na CT a MR, jsou doplněny RTG snímky. Výsledky. V závislosti na typu koalice a zvolené projekci, jsou známky na RTG snímku buď patognostické (u šikmé projekce a kalkaneonavikulární koalice), nebo diskrétní a nepřímé (u ostatních projekcí a ostatních typů koalice). CT a MR zobrazí tuto vadu při vhodně zvolených rovinách zobrazení a sekvencích nepochybně. Mezi nejvýznamnější nálezy patří zúžená nebo přemostěná kloubní štěrbina, nerovná kortikální kost, edém, osteoskleróza a změna tvaru kostí. Časté jsou sekundární degenerativní změny okolních kloubů. Závěr. Povědomí o známkách této vady umožňuje její rozpoznání i přes její málo frekventní výskyt, a to i v případech, kdy na ni klinický lékař nevysloví podezření. Na CT a MR je třeba při vyšetřování hlezna rutinně zachycovat i oblast subtalárních kloubů a tarzu., Aim. To summarize and display the imaging findings in patients with different types of both frequent and less frequent types of tarsal coalition, to discuss the imaging protocols, differential diagnoses and give brief information on clinical findings and treatment. Method. The authors display CT, MRI and X-ray findings in 8 patients with the main types of coalition (talocalcaneal, calcaneonavicular), with a bilateral talocalcaneal coalition and in a patient with a complex synostosis. Results. The encountered findings were: narrowed or absent joint space, thinned irregular cortical line, adjacent oedema and sclerosis and an unusual shape of the involved bones. Secondary degenerative changes in the adjacent joints were often encountered. Depending on the type of coalition and on the projection, this diagnosis presents with X-ray findings, which are either prominent (in calcaneonavicular coalition and an oblique X-ray) or subtle and indirect (all other cases). In MRI and CT the diagnosis is much more obvious. The pathognomonic signs as well as secondary degenerative signs will be pointed out together with the appropriate imaging protocols. A differential diagnosis will be discussed. The findings of the authors will be correlated with the literature. Conclusion. It is necessary to be aware of the findings in tarsal coalition, in order to establish the diagnosis even in cases unsuspected by the clinitian. Cíl práce: Zhodnotit bezpečnost a účinnost léčby bazálním inzulinem glargin v režimu bazál/bolus v podmínkách běžné klinické praxe v České republice. Soubor a metodika: Projekt LINDA byl neintervenční, multicentrický (n = 255), národní observační projekt, který probíhal v podmínkách běžné klinické praxe. Do sledování bylo zařazeno celkem 4 998 pacientů s diabetes mellitus 1. a 2. typu (DM1T, DM2T) s převážně inzulinovou terapií (99,7 %), po následném zahájení léčby inzulinem glargin v režimu bazál/bolus. Pacienti byli sledováni po dobu 6 měsíců od zahájení této terapie. Primárním cílem projektu bylo sledování četnosti výskytu závažných hypoglykemických příhod při léčbě bazálním inzulinovým analogem glargin (Lantus?) v podmínkách běžné klinické praxe. Sekundárními sledovanými parametry byla změna glykovaného hemoglobinu (HbA1c), glykemie nalačno (FPG), změna tělesné hmotnosti, změna dávek inzulinu, změna počtu hypoglykemických příhod ve srovnání s předchozí léčbou a sledování četnosti nežádoucích účinků. Výsledky: Závažné hypoglykemie byly pozorovány během léčby inzulinem glargin u 0,8 % pacientů. Při srovnání výskytu hypoglykemií s předchozí terapií bylo prokázáno klinicky a statisticky významné snížení jejich četnosti. Procento pacientů s hypoglykemickou příhodou (17,6 %), závažnou (0,8 %) a závažnou noční hypoglykemií (0,3 %) bylo za poslední měsíc léčby s inzulinem glargin v režimu bazál/bolus konzistentně menší ve srovnání s posledním měsícem léčby před zahájením této terapie (42,5 %; 17,6 %; 13,8 % pacientů). U pacientů s DM1T došlo k poklesu všech hypoglykemií z hodnoty 37,80 ? 15,95 příhod/pacient/rok na 8,76 ? 4,38 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 5,64 ? 3,27 příhod/pacient/rok na 0,0396 ? 0,012 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 3,84 ? 2,04 příhod/pacient/rok na 0,0096 ? 0,003 příhod/pacient/rok (p < 0,001). U pacientů s DM2T poklesla četnost všech hypoglykemií z hodnoty 12,48 ? 7,57 příhod/pacient/rok na 1,68 ? 0,78 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 2,04 ? 0,94 příhod/pacient/rok na 0,0132 ? 0,005 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 1,32 ? 0,77 příhod/pacient/rok na 0,0048 ? 0,0008 příhod/pacient/rok (p < 0,001). Bylo prokázáno statisticky významné zlepšení metabolické kompenzace při užití inzulinu glargin. U DM1T došlo ke snížení hladiny glykovaného hemoglobinu (HbA1c) z hodnoty 7,74 ? 1,71 % na 6,43 ? 1,39 % (∆ -1,31 ? 0,32 %; p < 0,001) a u DM2T z hodnoty 8,13 ? 1,56 % na 6,72 ? 1,40 % (∆ -1,41 ? 0,28 %; p < 0,001). Statisticky významně (p < 0,001) se zvýšil počet pacientů s hodnotami HbA1c < 5,4 %. Statisticky významný byl také pokles glykemie nalačno (FBG) a hodnot 6bodového glykemického profilu, a to u obou typů diabetu (p < 0,001). Změna terapie a následná léčba inzulinem glargin byla lékaři a pacienty vnímána pozitivně. Závěr: Za podmínek běžné klinické praxe zahájení léčby inzulinem glargin v režimu bazál/bolus u pacientů s předchozí inzulinovou terapií vedlo ke snížení četnosti výskytu hypoglykemických příhod včetně závažných a závažných nočních hypoglykemií a ke zlepšení metabolické kompenzace pacientů s diabetem (snížení glykovaného hemoglobinu, glykemie nalačno a hodnot 6bodového glykemického profilu). Byla zaznamenána větší spokojenost se stávající léčbou ze strany pacientů a lékařů. Klíčová slova: diabetes mellitus – inzulin glargin- observační projekt – režim bazál/bolus, Objective: To evaluate the safety and efficacy of basal insulin glargine using a basal-bolus regimen in a common clinical practice setting in the Czech Republic. Patients and methods: The LINDA project was a non-interventional, multicenter (n = 255), national, observational project. A total of 4,998 patients with Type 1 and 2 diabetes mellitus (T1DM, T2DM) with predominantly insulin therapy (99,7 %), after switch on insulin glargine at basal-bolus regimen, were enrolled in this project. The patients were followed up for 6 months after initiation of the therapy with insulin glargine. The primary objective of the project was to investigate the incidence of severe hypoglycemic episodes during the treatment with basal insulin analogue glargine (Lantus?) in a common clinical practice setting. The secondary endpoints were changes in glycosylated hemoglobin (HbA1c) levels, fasting plasma glucose (FPG), body weight, insulin dose, change of number of hypoglycemic episodes in comparison the previous therapy and the frequency of adverse effects. Results: Severe hypoglycaemia were observed during treatment with insulin glargine at 0.8 % patients. When comparing the incidence of hypoglycemia with the previous therapy, we demonstrated a clinically and statistically significant reduction in their frequencies. The percentage of patients with hypoglycemic episodes (17.6 %), severe hypoglycemia (0.8 %) and severe nocturnal hypoglycemia (0.3 %) over the last month of treatment with insulin glargine using the basal-bolus regimen was consistently lower compared to the last month of treatment before initiation of this therapy (42.5 %, 17.6 %, and 13.8 % of the patients, respectively). In patients with T1DM, the incidence of hypoglycemia decreased from 37.80 ? 15.95 episodes/patient/year to 8.76 ? 4.38 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 5.64 ? 3.27 episodes/patient/year to 0.0396 ? 0.012 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 3.84 ? 2.04 episodes/patient/year to 0.0096 ? 0.003 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. In patients with T2DM, the incidence of hypoglycemia decreased from 12.48 ? 7.57 episodes/patient/year to 1.68 ? 0.78 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 2.04 ? 0.94 episodes/patient/year to 0.0132 ? 0.005 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 1.32 ? 0.77 episodes/patient/year to 0.0048 ? 0.0008 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. A statistically significant improvement in the metabolic control was demonstrated when using insulin glargine. The glycated hemoglobin (HbA1c) decreased from 7.74 ? 1.71 % to 6.43 ? 1.39 % (∆ -1.31 ? 0.32 %, p < 0.001) in patients with T1DM, and from 8.13 ? 1.56 % to 6.72 ? 1.40 % (∆ -1.41 ? 0.28 %, p < 0.001) in patients with T2DM. A statistically significant (p < 0.001) increase in the number of patients with HbA1c < 5.4 % was further demonstrated. The decrease in fasting blood glucose (FBG) and 6-point blood sugar profile was also statistically significant in both types of diabetes (p < 0.001). Changes in therapy and subsequent treatment with insulin glargine were perceived positively by both physicians and patients. Conclusion: In the common clinical practice setting, the initiation of treatment with insulin glargine using the basal-bolus regime in patients with previous insulin therapy resulted in a reduction in the incidence of hypoglycemic events, including severe hypoglycemia and severe nocturnal hypoglycemia, and improved metabolic control in patients with diabetes (reduced glycated hemoglobin, fasting glucose values and 6-point blood glucose profile). Greater satisfaction with the current treatment was reported by both patients and physicians. Figurální (neboli neverbální) fluence je schopnost exekutivních funkcí, která nám poskytuje informace o divergentním myšlení, rozdělené pozornosti, plánování a mentální flexibilitě. Zhoršený výkon v testech zaměřených na figurální fluenci nacházíme u pacientů s neurologickým i psychiatrickým postižením. Pětitečkový test (Five Point Test, 5TT) je jeden z neuropsychologických testů, jenž slouží ke zhodnocení figurální fluence. Úkolem probanda je vytvořit co nejvíce obrazců v časovém limitu. Cílem této studie bylo vytvořit normy k 5TT pro českou dospělou populaci. Předkládáme normativní data pro dospělé ve věku od 20 do 85 let (n = 503). Hodnotili jsme počet správných odpovědí a počet perseverací. Počet správných odpovědí je ovlivněn věkem a vzděláním (r = –0,3; resp. 0,4; p < 0,0001), proto jsou normy rozděleny na pásma po 10 letech a dále podle ukončeného vzdělání. Počet perseverací s těmito proměnnými souvisí jen slabě (rs = 0,1; resp. –0,1; p < 0,05). Pohlaví nemá vliv na počet správných odpovědí ani perseverací (t = 0,09; p > 0,9 pro oba skóry)., Figural (or nonverbal) fluency is the ability of executive functions to provide information about divergent reasoning, divided attention, planning and mental flexibility. Impairments of figural fluency have been found in individuals with various neurological or psychiatric diseases. Five Point Test (5TT) is a neuropsychological test that assesses figural fluency. A participant is asked to generate as many unique designs as possible in a certain time limit. The aim of this study was to create Czech population norms for the Five Point Test. Normative data for adult population aged between 20 and 85 years (n = 503) are presented. We assessed the number of correct answers and the number of perseverations. The number of correct answers is influenced by age and education (r = –0.3 and 0.4, respectively, p < 0.0001); for this reason the norms are stratified into ten age ranges and also according to completed education. The number of perseverations correlates with these variables only weakly (rs = 0.1 and –0.1, respectively, p < 0.05). Gender has no impact neither on the number of correct answers nor on perseverative responses (t-test, p > 0.9 for both scores). Cíl studie: Vyhodnotit přínos interleukinu-6 u těžké akutní pankreatitidy pro odlišení sterilní nekrózy (SN) a nekrózy infikované (IN). Typ studie: Retrospektivní deskriptivní observační studie série případů. Metody: Ve vlastní práci bylo zahrnuto 59 pacientů hospitalizovaných s těžkou akutní pankreatitidou (Atlantská klasifikace 1992) na chirurgickém oddělení Nemocnice Písek v průběhu sedmi let. Pravidelně byly vyšetřovány C-reaktivní protein (CRP) a interleukin-6 (IL-6), přičemž dostatečná data byla u 42 pacientů: 14 s IN (průkaz mikrobiologicky či dle CT), 28 se SN. Statistické vyhodnocení pomocí Fisherova testu a chí-kvadrát testu. Výsledky: Byla nalezena statisticky významná diskriminační schopnost denních průměrů IL-6 5. a 6., resp. 7. a 8. den hospitalizace mezi IN a SN (p=0,0014 pro den 5 a 6, resp. p=0,0009 pro den 7 a 8). Rozvoj IN reálně nastával nejčastěji mezi 5. a 9. dnem (mikrobiologický průkaz agens či nepochybné CT známky infekce). CRP tuto schopnost diskriminace neprokázalo, jeho hladiny byly od 2. do 8. dne setrvale výrazně zvýšeny. Charakteristický byl průběh koncentrací IL-6, kdy vstupně byla přítomna výrazná elevace nezávislá na pozdějším rozvoji IN a již od druhého dne hospitalizace došlo k výraznému poklesu. K dalšímu vzestupu IL-6 došlo pouze u pacientů s rozvojem IN. Pro cut-off (diskriminace IN a SN) IL-6 (platný od 5. dne hospitalizace) 100 ng/l bylo p<0,005, pro 150 ng/l p<0,01. Závěr: Interleukin-6 v naší retrospektivní studii prokázal statisticky významnou schopnost diskriminovat infikovanou a sterilní nekrózu u těžké akutní pankreatitidy. Překročení cut-off IL-6 od 5. dne hospitalizace svědčilo pro přítomnost infikované nekrózy. Cut-off 100 ng/l diskriminoval obdobně (p<0,005, senzitivita 100 %, specificita 92 %) jako cut-off 150 ng/l (p<0,01, senzitivita 87 %, specificita 100 %)., Objective: Our retrospective study aimed to evaluate the contribution of interleukin-6 in severe acute pancreatitis. Mortality of severe acute pancreatitis ranges from 10-20% (for sterile necrosis, SN) to 20-85% (infected necrosis, IN). Resolution of SN and IN is clinically very important, but often difficult and therefore they are still searching for new laboratory markers for their differentiation. Design: Retrospective descriptive observational study of cases series. Methods: 59 patients hospitalized with severe acute pancreatitis (Atlanta classification 1992) in the surgery department of Hospital Písek in the 2000-2006 were included. Regularly they were investigated by C-reactive protein (CRP) and interleukin- 6 (IL-6), sufficient data was in 42 patients: 14 with IN (identified microbiologically or according to computed tomography, CT), 28 with SN. Statistical analysis was performed using Fisher‘s test and chi-square test. Results: There was found statistically significant discriminatory ability of daily averages of IL-6 5th and 6th + 7th and 8th day of hospitalization between the IN and SN (p = 0.0014 for day 5 and 6, respectively. p = 0.0009 for day 7 and 8). Developing IN realistically set in the most between 5th and 9th day (needle biopsy with identification of microbiological agents or CT signs of infection). The ability of CRP to discriminate SN and IN was not found, the levels were between 2nd to 8th days increased steadily. The run of the concentrations of IL-6 was characteristic, where at the admission there was present significant elevation irrespective of subsequent development of IN and from the second day of hospitalization it decreased significantly. Further increase in IL-6 was present only in patients with the development of IN. For the cut-off (discrimination of SN and IN) for IL-6 (valid from 5th of hospitalization) 100 ng/L was P<0.005, for 150 ng/L P<0.01. Conclusion: Interleukin-6 in our retrospective study showed a statistically significant ability to discriminate between infected and sterile necrosis in severe acute pancreatitis. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Pavlova 6, 775 20 Olomouc, Czech Republic; Hospital Písek, Karla Čapka 589, 397 23 Písek, Czech Republic. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Conclusion: The role of ADP, A-FABP and FGF 21 has been limited in a prediction of the disease severity in day 4 after admission, while CRP and IL-6 might be useful to exclude a severe AP., Cíle studie: Studie byla zaměřena na vyhodnocení sérových hladin adiponektinu (ADP), adipocytárního proteinu vázajícího mastné kyseliny (A-FABP) a fibroblastového růstového faktoru 21 jako potenciálních prediktorů závažnosti akutní pankreatitidy (AP) 4. den po přijetí. Laboratorní markery byly měřeny v podskupinách podle klasifikace lehká/ těžká AP, a částečně také v podskupinách vytvořených na základě CTSI (computed-tomography severity index) skóre. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Pro oba parametry byly stanoveny cut-off hodnoty: 100 mg/l pro CRP a 37 ng/l pro IL-6, s negativními prediktivními hodnotami (NPV) 96 % a 92 %, a s pozitivními prediktivními hodnotami (PPV) 39 % a 29 %. Závěr: Ve studii jsme zjistili, že hladiny ADP, A-FABP a FGF 21 nejsou vhodnými parametry pro predikci tíže AP 4. den po přijetí. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. Cíl studie: Cílem studie bylo zjištění vztahů mezi manifestací tinnitu, nálezem na sluchových evokovaných potenciálech a genetickým pozadím u receptoru pro gamaaminomáselnou kyselinu typu A (GABA(A) receptor), podporující desinhibiční hypotézu vzniku tinnitu. Soubor a metodika: Bylo vyšetřeno 131 pacientů z hlediska sluchové ztráty, provedena kvantifikace tinnitu, sluchové evokované potenciály střední latence (MLR) a kmenové sluchové evokované potenciály (BAEP) a dále stanovení genotypu (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru. Následně byly hledány vztahy mezi jednotlivými výsledky a manifestací tinnitu. Výsledky: Byla nalezena korelace tinnitus skóre s amplitudovým poměrem vln V/III v BAEP (R = 0,22, p < 0,001) a s průměrným sluchovým prahem (R = 0,22, p = 0,17). Rovněž byla nalezena korelace tinnitus skóre s amplitudou vlny PA v MLR (R = 0,31–0,37; p < 0,001). Výsledky MLR neukázaly žádný vztah k průměrnému sluchovému prahu. U skupiny s kratší anamnézou tinnitu (méně než devět měsíců) byl prokázán rozdíl v manifestaci tinnitu na genotypu pro (CA)n repetitivní sekvenci genu pro beta‑3 podjednotku GABA(A) receptoru (p = 0,002). Tento výsledek byl rovněž konzistentní s rozložením amplitudy vlny PA v dané subpopulaci. Závěr: Tyto výsledky svědčí o existenci dvou hlavních regulačních mechanizmů vzniku tinnitu: první, který je závislý na velikosti sluchové ztráty, je na úrovni mozkového kmene, zatímco druhý je na úrovni korové s možnou souvislostí s genotypem (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru., Study aim:Study objective was to explore associations between manifestation of tinnitus, auditory evoked potentials and genetic background of gamma‑aminobutyric acid type A (GABA(A) receptors) to support the disinhibited feedback hypothesis of tinnitus generation. Materials and methods: A population of 131 patients was assessed for severity of hearing loss, quantification of tinnitus, mid‑latency responses (MLR) and brainstem auditory evoked potentials (BAEP), and (CA)n tandem repeat polymorphism in GABA(A) Beta‑3 subunit gene to establish any correlation with manifestation of tinnitus. Results: It was observed that tinnitus score correlates with V/III amplitude ratio in BAEP (R = 0.22, p < 0.001) and with mean pure tone audiometry (PTA) threshold (R = 0.22, p = 0.017). Analysis of the MLR results showed a significant correlation between the PA wave amplitude and the tinnitus score (R = 0.31–0.37; p < 0.001). MLR result analysis showed no statistically significant correlation between the wave amplitudes and the mean auditory threshold. An analysis of a subgroup with shorter clinical history (less than nine months) revealed a statistically significant difference in the tinnitus score in relation to the genotype of (CA)n tandem repeat of the GABRß3 receptor subunit gene (p = 0.002). This result was also consistent with the distribution of the PA wave amplitude in the given subpopulation. Conclusion: Our findings indicate existence of two main regulatory mechanisms of tinnitus generation: first, the brainstem mechanism is dependent on the severity of the hearing loss; second, the cortical mechanism is likely to be dependent on the genotype of (CA)n tandem repeat in GABA(A) beta‑3 subunit gene., and J.
