Klasifikácia SZO (2008) je jediná celosvetovo akceptovaná a v praxi používaná klasifikácia nádorových ochorení kostnej drene (KD), ktorá definuje aj postavenie a význam biopsie KD v diagnostickom algoritme. Myeloproliferatívne neoplázie (MPN) tvoria pestrú skupinu ochorení s variabilne dynamickým rozvojom z počiatočných štádií, cez štádiá s plno rozvinutou klinicko-laboratórnou manifestáciou až do pokročilých štádií – fibrotickej, resp. blastovej transformácie, sprevádzaných zmenami morfológie a laboratórneho obrazu. V počiatočných štádiách je určenie typu MPN len na základe klinicko-laboratórnych údajov pre podobnosť ich klinickej manifestácie nespoľahlivé. Dôkaz pre MPN typických genetických mutácií potvrdzuje klonalitu ochorenia, ale jeho typizačný význam je limitovaný. Naopak, bioptické vyšetrenie KD v kontexte dostatočných klinických informácií, výsledkov laboratórnych vyšetrení a s podporou genetických vyšetrení umožňuje okrem potvrdenia MPN aj bližšiu typizáciu, ktorá je prognosticky relevantná a ovplyvňuje aj voľbu terapie. Zároveň umožňuje odlíšenie reaktívnych zmien hemopoézy, zhodnotenie prognosticky významných morfologických znakov (stupeň myelofibrózy, množstvo blastov) a sledovanie progresie ochorenia do fibrotickej, resp. blastovej transformácie. V týchto pokročilých štádiách a po terapii nie je však typizácia MPN len na základe morfológie KD možná. Výpovedná hodnota biopsie závisí od kvality a kvantity vzorky drene, hodnotenie morfológie KD závisí aj od vedomostí a skúseností vyšetrujúceho patológa., WHO classification (2008) is the only world-wide accepted and in routine praxis used classification of bone marrow (BM) neoplasms defining also the importance of BM biopsy within the diagnostic algorithm. Myeloproliferative neoplasias (MPN) represent a heterogenous group of disorders with variable dynamic transformational potential from initial into advanced stages such as blast crisis or myelofibrotic transformation. Because of similar manifestations, the typing of MPN in their initial stages based on clinical and laboratory parameters is not optimal. The finding of MPN-typical genetic mutations proves the disease clonality, but not a diagnosis of individual MPN types. In contrast, the BM biopsy verifies both the diagnosis and MPN type. It helps also to discriminate MPN from reactive hematopoietic changes, to evaluate prognostically relevant BM features (e.g. grade of myelofibrosis, blasts number) and the follow-up to terminal stages. However, the MPN typing in terminal and posttherapeutical stages based on morphology alone is not sufficient. The diagnostic value of BM biopsy depends on specimen´s quality, the objectivity of BM morphology evaluation depends on pathologist´s training and experiences. Nádory skupiny Ewingova sarkomu/primitivního neuroektodermálního tumoru (ES/PNET) jsou typické pro věkovou skupinu dětí a adolescentů. Devadesát procent pacientů je mladších 20 let. Prognóza je nepříznivá, s 5letým přežitím mezi 55–70 % v případě lokalizovaného, méně než 20 % u metastatického onemocnění. S narůstajícím věkem jsou výsledky léčby ještě méně příznivé. Léčba by měla být vedena na základě rozhodnutí a spolupráce v rámci multioborového týmu. Uvádíme naše zkušenosti s léčbou ES/PNET dospělých pacientů léčených dle protokolů EURO E.W.I.N.G. 99/EWING 2008., Ewing´s Family of Tumours is most frequently observed in children and adolescent age. 90 % of patients are younger than 20 years. Prognosis is unfavourable, with 5-year overall survival from 55 to 70 % in localized, not exceeding 20 % in primarily metastatic disease. The results of the therapy are getting worse by increasing age. Multidisciplinary treatment planning is mandatory in all cases of this tumor. Cíl: Posoudit možnosti diagnostiky cho-lesteatomu středouší pomocí magnetické rezonance (MR) za použití non-echo-planární difúzně-vážené (non-EPI DWI) sekvence. Metodika: Prospektivní studie zahrnovala všechny pacienty, kteří byli indikování k primární nebo revizní operaci cholesteatomu středouší v období mezi 10/2012 až 1/2015. Všichni tito pacienti před operací podstoupili vyšetření MR za použití non-EPI DWI techniky. Šlo o 25 pacientů, u dvou s oboustranným postižením. Deset bylo před primární operací, 17 před revizním výkonem. MR vyšetření bylo zhotoveno v rozmezí 0-106 dnů (medián 2 dny) před operací. Výsledky MR vyšetření byly srovnány s operačním a definitivním histologickým nálezem. Výsledky: MR vyšetření s non-EPI DWI sekvencí mělo pro diagnostiku cholesteatomu senzitivitu 93,75 %, specificitu 93,75 %, pozitivní prediktivní hodnotu 93,75 % a negativní prediktivní hodnotu 93,75 %. Nejmenší správně diagnostikovaný cholesteatom měl velikost 4 mm, největší 20 mm, průměr 11 mm. Závěr: Vyšetření MR s použitím non-EPI DWI sekvence je spolehlivou metodou k zobrazení cholesteatomu středouší. Lze ji využít jak u pacientů před primárním operačním výkonem, tak u pacientů před second-look či revizní operací., Aim: To evaluate the possibility of diagnosis of middle ear cholesteatoma on MRI using non-echo-planar diffusion-weighted imaging (non-EPI DWI). Method: The prospective study included all patients indicated to primary or second-look surgery of middle ear cholesteatoma between 10/2012 and 1/2015. All these patients underwent MRI using non-EPI DWI technique before the surgery. There were 25 patients, two patients had bilateral disease. 10 patients were before primary surgery, 15 patients before second-look surgery. The MRI was performed from 0 to 106 days (median 2 days) before surgery. The results of the MR investigation were compared with those obtained by surgery and confirmed by definitive histopatological evaluations. Results: The sensitivity of 93,75%, specificity of 93,75 %, positive predictive value 93,75 %, negative predictive value 93,75% was reached in detection of the middle ear cholesteatoma using MR with non-EPI DWI. The smallest correctly diagnosed cholesteatoma had the diameter of 4mm, the largest 20 mm, mean 11 mm. Conclusion: MR investigation using non-EPI DWI sequence is reliable method for the middle ear cholesteatoma diagnosis.
