Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. Současně je podána informace o budoucí možné právní úpravě, která rozšíří informační povinnost zdravotnických pracovníků a zařízení., The article deals with the legal aspects of informing oncological patients. Během loňského roku proběhl mezinárodní výzkum komunikace mezi urologem a jeho pacientem. Do tohoto projektu byla ve spolupráci s fi rmou IPSEN zařazena i Česká republika. Výsledky poukazují na skutečnost, že komunikaci mezi lékařem a pacientem se stále v České republice nevěnuje dostatek pozornosti, která ji náleží. Většina problémů pramení z odlišných očekávání pacienta a lékaře. Nenaplněná očekávání bývají zdrojem mnoha vzájemných komunikačních a postojových nedorozumění. Článek se zabývá strategií komunikace mezi urologem a jeho pacientem a defi nuje přímá doporučení komunikace s cílem zlepšit spolupráci a ovlivnit psychický stav pacienta včetně jeho spokojenosti s lékařskou konzultací., This research, focusing on communication between urologist and patient, has been conducted during last year. Czech Republic has been a part of this project in cooperation with IPSEN company. The results highlight, that in Czech Republic insuffi - cient attention is given to communication between doctor and patient. Most signifi cant problems stem from the discrepancy in expectations between two partners. Unfulfi lled expectations can be a source of mutual communication or attitudinal misunderstanding. Akutní iktus je častou příčinou mortality a invalidity. Skepse k léčeným možnostem přetrvává jak mezi lékaři, tak mezi laickou veřejností. Nicméně výsledky moderní reperfuzní léčby pacientů postižených akutním iktem se zlepšily – alespoň u těch, kteří se dostaví časně po projevení se symptomu. Tento článek představuje multidisciplinární přístup ke zvládání akutního iktu – spolupráci neurologů, kardiologů a radiologů. Pojednává se o obou strategiích reperfuze: katetrizační trombektomii i trombolýze., Acute ischemic stroke is a frequent cause of death and disability. Therepautic scepticism persists both among doctors and lay people. However, modern reperfusion therapy improved outcomes of acute stroke patients – at least of those presenting early after symptom onset. Dítě, které v neznámé situaci pociťuje úzkost a nějak ji projevuje, se chová normálně. Děti léčené na Klinice dětské hematologie a onkologie Fakultní nemocnice v Motole jsou vystavené mnoha zátěžovým faktorům, jako jsou protinádorová léčba, dlouhý čas strávený v nemocnici a neplánované hospitalizace při komplikacích, včetně nutnosti přijímání pro dítě nesrozumitelných zpráv. Součástí léčby bývá i řada nejrůznějších vyšetření včetně diagnostických zobrazení. Ve své práci se věnujeme především tomu, jak dětského pacienta připravit na náročná vyšetření a pojmenujeme vypozorované zásady, které se nám osvědčily v kontaktu s dětskými pacienty obecně. V textu přinášíme několik možných postupů, jak s dětmi komunikovat a snižovat jejich přirozeně úzkostnou reakci a strach., A child who feels anxiety in an unknown situation, and somehow it manifested, it behaves normally. The children treated in the Clinic of Paediatric Haematology and Oncology teaching hospital in Motol are exposed to many stress factors such as anticancer treatment, a long time spent in the hospital, and the unplanned hospitalisation for complications, including the need for commuinatipon with a child of incomprehensible messages. Part of the treatment is a series of tests including diagnostic display. In our work we show some posibilities, how to prepare child patient for intensive examination and we bring observed principles that are proven in contact with children's patients in general. Při snižování a řízení hluku v komunálním prostředí je třeba rozlišovat dvě kategorie účinků hluku na člověka. Jsou to jednak přímé zdravotní účinky a jednak obecné obtěžování hlukem spadající do oblasti kvality života. Hluk z hlediska obou kategorií musí být v životním prostředí regulován – otázkou je nejvhodnější regulátor a odpovídající nejvhodnější nástroje. Stávající hluková legislativa administrovaná Ministerstvem zdravotnictví se týká ochrany veřejného zdraví, a nikoliv snižování hluku v životním prostředí jako celku. Řešení obecného obtěžování je věcí veřejného pořádku v kompetenci obcí, Městské policie a Policie České republiky. Nepochopení rozdílů obou přístupů vede často ke zbytečným nedorozuměním ze strany laické veřejnosti i médií, ale v řadě případů i veřejnosti odborné., In environmental noise management and regulation two categories of noise effects on humans should be taken into account: first by there are direct health effect and second by there is general noise annoyance belonging to the area of the quality of life. Noise, from the point of view of in both categories, should be regulated in the environment – the question is, however, the best proper authority and proper legislative tools for this regulation. Existing noise legislation within the agenda of the Ministry of Health concerns public health safety only and not the general reduction of all noise in the whole living environment. Solving the problem of general annoyance by noise is the a matter for the local authorities and the police.
