AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Cíl: Cílem je zhodnotit zkušenosti s léčbou aneurysmatu podkolenní tepny stentgraftem. Metodika: V období od června 2011 do října 2014 jsme na našem pracovišti léčili 18 aneurysmat podkolenní tepny u 14 pacientů pomocí stentgraftu. Jednalo se o 13 mužů a jednu ženu ve věku od 50 do 90 let. Věkový průměr byl 70 let, medián 69,5 let. Léčili jsme tři aneurysmata o průměru menším než 2 cm, 12 aneurysmat o průměru 2-5 cm a tři výdutě o průměru větším než 5 cm. Výsledky: Pacienti byli sledováni 2-35 měsíců, průměrná doba sledování byla 13, 4 měsíce, medián 10 měsíců. Technická úspěšnost byla 100 %. Primární průchodnost byla 78 %, sekundární průchodnost byla 94 %. Podle Kaplanovy-Maierovy analýzy byly pravděpodobnost primární průchodnosti za 6 a 12 měsíců po implantaci stentgraftu stejná, a to 75,8 % (95 % CI: 54,6-97,0 %), pravděpodobnost sekundární průchodnosti za 6 a 12 měsíců 94,4 % (95 % CI: 83, 8-100 %). Závěr: Endovaskulární léčba stentgraftem je možnou alternativou chirurgické léčby aneurysmatu podkolenní tepny. Endovaskulární léčba je jednoduchá, bezpečná a účinná., Aim: To evaluate the experience in the treatment of popliteal artery aneurysm with stentgraft. Method: From June 2011 to October 2014, we were treated 18 aneurysms of the popliteal artery in 14 patients using the stentgraft at our department. There were 13 men and one woman aged from 50 to 90 years. The mean age was 70 years. We treated three aneurysms with a diameter of less than 2 cm, 12 aneurysms 2-5 cm in diameter and 3 aneurysms with a diameter greater than 5 cm. Results: Patients were followed-up 2-35 months, mean follow-up was 13.4 months. The technical success rate was 100%. Primary patency was 78%, secondary patency was 94%. According to Kaplan-Maier analysis the probability of primary patency at 6 and 12 months after stentgraft implantation was 75.8 % (95% CI: 54.6 to 97.0%), the probability of secondary patency at 6 and 12 months was 94.4% (95% CI: 83,8-100%). Conclusion: Endovascular treatment with stentgraft is a possible alternative of surgical treatment of popliteal artery aneurysm. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Cíl: Autoři prezentují první zkušenosti s hemihypoglossofaciální anastomózou při řešení postoperačních plegií lícního nervu u neurochirurgických pacientů. Soubor a metodika: V průběhu let 2009–2014 byli na našem oddělení operováni tři pacienti, u kterých byla provedena hemihypoglossofaciální anastomóza dle Cushimana a Sekhara. Důvodem plegie nervus facialis byl u dvou pacientů objemný vestibulární schwannom (v jednom případě operován po ozáření gama nožem) a u jednoho pacienta objemný schwannom nervus facialis pod bazí lební. Výsledky: U dvou pacientů bylo dosaženo reinervace ve stupni III a u jednoho pacienta bylo dosaženo reinervace ve stupni IV dle House a Brackmanna. U všech pacientů se rozvinula pouze lehká hemilinguální atrofie, která u žádného z nich neměla žádné funkční následky ve smyslu poruchy žvýkání, polykání či fonace. Průměrný follow‑up pacientů byl 39,3 měsíce. Závěry: Naše první zkušenosti ukázaly u tří pacientů dobrou reinervaci nervus facialis po hemihypoglossofaciání anastomóze. Metodu považujeme spolu s dalšími variantami hemihypoglossofaciálních spojek (Darrouzet, LeClerc) za efektivní způsob neurotizace plegického lícního nervu při malém riziku vzniku nežádoucích příznaků., Aim: Authors present their early experience with a hemihypoglossofacial anastomosis in the treatment of neurosurgical patients with complete postoperative facial palsy after removal of tumors in the posterior fossa or skull base. Patients and method: In three patients undergoing an open surgery for complete facial palsy between 2009 and 2014, the hemihypoglossofacial anastomosis according to Cushimano and Sekhar was performed. Facial palsy was due to a large vestibular schwannoma in two patients and a facial nerve schwannoma in one patient. Results: Functional reinnervation of grade III according to the House and Brackmann scale was achieved in two patients and reinervation of grade IV in one patient. All patients had mild hemilingual atrophy only that did not lead to any functional disturbances in chewing, swallowing or speech. The mean follow-up period was 39, 3 months. Conclusions: Our early experience showed very good functional reinervation in patients after hemihypoglosofacial anastomosis. We consider this method together with other modifications (Darrouzet, LeClerc) to be very effective in the treatment of facial plegia with low risk of side effects. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %.
