Osteochondritis dissecans(OCD) of the knee is identified with increasing frequency in the adolescent patient. Left untreated, OCD can cause significant impairment and restriction in physical activity and development of osteoarthritis at an early age. The diagnosis of lesions of OCD can be confirmed on plain radiographs. MRI has emerged as the gold standard to evaluate the stability of the lesion and the integrity of the overlying articular cartilage. Treatment of OCD lesions depend on the stability of the lesion. Stable lesions can be treated conservatively by physical activity modification and immobilization. Unstable lesions and stable lesions not responding to conservative measures should be treated surgically. Surgical options range from arthroscopic drilling, either transarticular or extra-articular drilling for stable lesions or salvage procedures such as autologous chondrocyte transplantation (ACT), mosaicplasty to restore joint and cartilage congruency. and H. Ozon (03) je složen ze tří atomů kyslíku a je silným oxidantem v důsledku značného dipólového momentu, který je velmi nestabilní. Ozon může být potencionálně nebezpečný, přesto mnohé studie dokazují taktéž jeho terapeutické účinky. Možné terapeutické využití ozonu jsou studovány již více než jedno století. Při správném dávkování nebyly zjištěny závažné nežádoucí účinky této léčby a v případě minimálních nežádoucích účinků se jedná o účinky preventabilní. Ozon se používá jako prostředek k dezinfekci a mnoha dalším léčebným účelům. Do skupiny onemocnění čelistního kloubu se řadí poměrně často se vyskytující stavy zejména u dospělých, které jsou charakterizovány především bolestmi a zvukovými fenomény provázejícími pohyb čelisti a změnou hybnosti čelisti. Zeny jsou postiženy častěji než muži, etiologie tohoto onemocnění zatím není plně objasněna. V současné době se uvažuje především o multifaktoriální teorii. V této kazuistice jsou popsány první zkušenosti s intraartikulární aplikací ozonu při léčbě onemocnění temporomandibulárního kloubu., Ozone (03) gas is a molecule consisting of three atoms of oxygen and powerful oxidant with dynamically unstable structure due to the presence of mesomeric states. Although 03 has potential dangerous effects, yet many studies confirm that it has also many therapeutic effects. Ozone therapy has been utilized and heavily studied for more than a century. Its effects are proven, consistent, safe and with minimal and preventable side effects. Temporomandibular joint disorders are common in adults; temporomandibular joint (TMJ) disorder refers to a cluster of conditions characterized by pain in the TMJ or its surrounding tissues, functional limitations of the mandible, or clicking in the TMJ during motion. The etiology of TMJ disorders remains unclear, but it is likely multifactorial. Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. U dvou pacientů byl ejakulát úspěšně použit k fertilizaci., Objectives: Following spinal cord injury (SCI), ejaculatory dysfunctions are frequent. In men who wish to father children, assisted sperm retrieval is often necessary. Penile vibratory stimulation (PVS) is a non-invasive method for sperm retrieval in patients with a lesion above T10. The aim of our research was to evaluate the effectiveness and safety of PVS, semen quality and further utilization of the ejaculate. Methods: Between 2010 and 2014, we performed PVS in 20 patients. An injury of the cervical (C) and thoracic (T) spinal cord was present in 13 and seven patients, respectively. The mean age of patients was 30.8 years. The mean time from the injury to PVS was 64 months. Seven patients had the first PVS more than 3.5 years after the injury and 13 patients less than 3.5 years. FERTI CARE Multicept was used for PVS. Results: Ejaculation was achieved in 11 patients (55%) – in nine with C and in two with T SCI. Success rate of PVS in patients less than 3.5 years after the injury was 77%, compared to 14% in patients more than 3.5 years after the injury, p = 0.027. Total sperm count (p = 0.03), sperm concentration (p = 0.025) and the number of sperm with progressive motility (p = 0.027) increased in the semen from the second PVS. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients. Perigraft serom je poměrně vzácná komplikace, která může vzniknout po implantaci cévní protézy z dakronu nebo expandovaného polytetrafluoretylenu (ePTFE). Předkládáme kazuistiku 54letého pacienta s perigraft seromem okolo axillofemorálního bypassu (ePTFE protézou). Konečným řešením byla explantace této extraanatomické rekonstrukce a implantace aortobiilického bypassu s použitím cévní protézy z jiného materiálu. Článek diskutuje terapeutické možnosti této komplikace na základě publikovaných prací. Žádná guidelines ani jiná doporučení neexistují. Závěrem, přístup k léčbě perigraft seromu zůstává striktně individuální. Náhrada cévní protézy s použitím implantátu z jiného materiálu se zdá být nejlepší možností řešení pro recidivující perigraft seromy tam, kde jiné méně invazivní postupy selhaly., Perigraft seroma is quite a rare complication that may occur after implantation of Dacron or expanded polytetrafluoroethylene (ePTFE) vascular grafts. We report a case of a 54-year-old patient with perigraft seroma around an axillofemoral bypass (ePTFE graft). Definitive treatment involved the explantation of this extraanatomic bypass with perigraft seroma and the implantation of an aortobiiliac bypass using vascular prosthesis made of a different material. Based on published studies, therapeutic options for this complication are discussed. No guidelines or recommendations are available. In conclusion, the approach to perigraft seroma treatment remains strictly individual. Časné pooperační komplikace hojení anastomózy představují pro pacienta významné riziko zvýšení morbidity, mortality i zhoršení dlouhodobých výsledků. Existuje řada opatření redukujících rizika vzniku těchto komplikací včetně peroperačního hodnocení kvality kolorektální anastomózy. Postupy hodnocení spolehlivosti anastomózy se vyvíjely od mechanického testování přes endoskopickou vizualizaci střevního lumen až po moderní techniky hodnotící mikroperfuzi perianastomotických tkání. Mechanické testování kolorektální anastomózy a částečně i endoskopické techniky prokazatelně do jisté míry redukují riziko pooperačních komplikací hojení kolorektální anastomózy. Moderní postupy hodnotící mikrocirkulaci, založené na fluorescenční angiografii pomocí indocyaninové zeleně, mají potenciál zásadně zlepšit posouzení bezpečnosti kolorektální anastomózy a významně přispět k rozhodnutí o dalším postupu včetně vynechání protektivní ileostomie., Early postoperative anastomotic complications have serious clinical implications for the patient’s morbidity, mortality as well as long-term results. A number of measures can be undertaken to reduce the risk of anastomotic complications, including intraoperative colorectal anastomotic integrity assessment. Methods used to assess anastomotic reliability have gradually developed from basic mechanical techniques, direct visual endoluminal inspection, to microperfusion assessment of perianastomotic tissue. Moderate benefit in terms of reduced postoperative anastomotic complications has been shown with mechanical patency testing and partly with intraoperative endoscopic visualization of colorectal anastomoses. Popisujeme případ multicentrické formy Castlemanovy choroby, s generalizovanou lymfadenopatií a splenomegalií, provázenou typickými B symptomy – úbytek hmotnosti 15 kg, horečky neinfekčního původu, noční pocení, symptomy anémie. Histologické vyšetření uzlin s nejvyšší akumulací fluorodeoxyglukózy, odebrané torakoskopicky z mediastina, prokázalo plazmocelulární typ Castlemanovy choroby. Vyšetření HIV a lidského herpesviru 8 (HHV-8) bylo negativní. Stanovení diagnózy nemoci předcházely 3 recidivy herpes zosteru, což svědčilo o alteraci imunity. Léčba byla zahájena kombinací talidomidu, dexametazonu a cyklofosfamidu. Po 2 měsících léčby nebyla zřetelná léčebná odpověď a pacient tuto léčbu špatně toleroval. Proto byl tento způsob léčby ukončen. Pro léčbu 2. linie byla zvolena léčba R-CHOP (Mabthera – rituximab, cyklofosfamid, adriamycin, vinkristin a prednison. V průběhu 2 cyklů této léčby se zmenšila lymfadenopatie a splenomegalie, ale léčbu provázely závažné infekční komplikace. Proto dále pokračovala pouze monoterapie přípravkem Mabthera v dávce 375 mg/m2 v 28denních intervalech. Tato léčba se ukázala jako účinná a dobře tolerovaná. Při kontrolním PET-CT vyšetření vymizela lymfadenopatie a splenomegalie a normalizovala se akumulace fluorodeoxyglukózy. Mabthera v monoterapii se v tomto případě ukázala jako účinný a velmi dobře tolerovaný lék. Pro multicentrickou formu Castlemanovy nemoci existuje v současnosti více účinných léčebných alternativ: léčba rituximabem v monoterapii či v kombinaci, léčba imunomodulačními léky (talidomid nebo lenalidomid), léčba protilátkami proti interleukinu 6 (siltuximab) nebo proti jeho receptoru (tocilizumab). V případně neúčinnosti jedné léčebné alternativy je nutné otestovat další alternativu. V tomto případně nebyla úspěšná léčba založená na talidomidu, zatímco léčba založená na přípravku Mabthera dosáhla vymizení projevů nemoci., We describe a case of multicentric Castleman disease with generalized lymphadenopathy and splenomegaly, accompanied by typical B symptoms - loss of 15 kg, fever of non-infectious origin, night sweats, symptoms of anemia. Histological examination of the nodes with the highest accumulation of fluorodeoxyglucose, taken from mediastinum by thoracoscopy, revealed plasmocellular type of Castleman disease. Tests for HIV and human herpesvirus 8 (HHV-8) were negative. Three recurrences of herpes zoster indicating an alteration of immunity preceded the diagnosis of disease. Treatment was initiated with combination of thalidomide, dexamethasone, and cyclophosphamide. The response after 2 months therapy was not clear and patient doesn´t tolerated the therapy well. Therefore, this treatment was terminated and R-CHOP (Mabthera - rituximab, cyclophosphamide, adriamycin, vincristine, and prednisone) was selected as a second-line therapy. Lymphadenopathy and splenomegaly were reduced during the 2 cycles of treatment, however, serious infectious complications accompanied the therapy. Therefore, only use of Mabthera monotherapy 375 mg /m2 was administered in 28-day intervals. This treatment has shown efficacy and tolerability. PET-CT scan has demonstrated disappearance of lymphadenopathy and splenomegaly, in addition, normalized accumulation of fluorodeoxyglucose. Monotherapy with Mabthera has proved to be effective and well tolerated drug in this case. Currently, there are more effective therapeutic alternatives in multicentric Castleman disease: treatment with monotherapy of rituximab or in combination therapy with immunomodulatory drugs (thalidomide or lenalidomide, treatment with anti-IL-6 (siltuximab) or against its receptor (tocilizumab). In the case of ineffectiveness of one treatment option must be tested other alternative. AIM/BACKGROUND: Pistacia terebinthus is used as a coffee substitute in the East and Southern Anatolia regions of Turkey. It contains unsaturated fatty acids, tocopherols, polyphenols and carotenoids. P. terebinthus has anti-inflammatory and potential antioxidant activity. In this study we evaluated the protective effects of P. terebinthus coffee (PTC) on thioacetamide (TAA)-induced liver injury in rats. MATERIALS AND METHODS: Twenty-eight male Sprague-Dawley rats were equally randomized into four groups. Chronic liver injury was induced with TAA (100 mg/kg i.p. three times weekly). The first group of rats served as control and received only tap water (G1), and the remaining groups of rats received PTC, p.o (G2); TAA (G3); TAA plus PTC, p.o (G4), respectively. RESULTS: After 8 weeks, PTC intake significantly reduced fibrosis/inflammation scores (p PTC intake reduced transforming growth factor beta (TGF-β) concentrations in the liver (p PTC intake. DISCUSSION AND CONCLUSION: PTC intake provided beneficial effects against TAA-induced liver injury in rats. Endoskopická ventrikulocisternostomie III. komory (ETV) je v současnosti považována za metodu první volby v léčbě obstrukčního hydrocefalu. Je indikována v případě obstrukce v komorovém systému distálně od akveduktu, některými autory pak i u jiných typů hydrocefalu. Úspěšnost této metody se pohybuje v průměru kolem 2/3 operovaných pacientů a je závislá na celé řadě faktorů. V našem sdělení se snažíme shrnout současné poznatky zabývající se předpovědí úspěšnosti ETV. Popisujeme faktory před-, peri- a pooperační a jejich vliv na úspěšnost ETV., At present, endoscopic third ventriculostomy (ETV) is regarded as the method of the first choice in the treatment of obstructive hydrocephalus. ETV is indicated in patients with obstruction in the ventricular system distally from the aqueduct; some authors had also used the method in patients with other types of hydrocephalus. The method is successful in about 2/3 of patients and its success depends on numerous factors. In our report, we attempt to summarize current knowledge on predicting success of ETV. We describe pre-, peri- and postoperative factors and their effect on the success of ETV. Cíl: Stručný přehled analytických a klinických aspektů měření parathyroidního hormonu v séru s důrazem na posouzení srovnatelnosti současně používaných metod v klinických laboratořích. Metoda: Prostudování a vyhodnocení recentní odborné literatury z let 2009-2014, doplněné experimentem u souboru pacientů léčených na hemodialýze. V něm bylo sledováno 99 pacientů třemi metodami druhé generace (Roche, DiaSorin, Siemens DPC) a dvěma metodami 3. generace (DiaSorin, Roche). Výsledky a diskuse: Výsledky získané různými metodami jsou velmi odlišné. Hodnoty mediánů jsou až dvojnásobně odlišné a u zvýšených hodnot jsou rozdíly ještě vyšší. Systematické diference jsou významné nejen mezi metodami různých generací, ale (méně) i mezi metodami generací stejných. Četnost výsledků uvnitř rozhodovacího limitu KDIGO 2009 se v závislosti na použitém kitu lišila u pacientů na hemodialýze až 2,5 násobně. Prvním krokem k postupné harmonizaci měření PTH u renálně nemocných pacientů by mělo být použití 3. generace imunochemických metod. Principiálním řešením bude standardizace na bázi separačních metod LC-MS/MS., Method: Overview of recent literature on the PTH comparability from 2009-2014. Comparison of PTH results obtained from 99 patients on the hemodialysis during their regular controls. We used for this purpose three methods of 2nd generation (DiaSorin, Roche, Siemens DPC) and two methods of 3th generation (DiaSorin, Roche). Results and discussion: Results obtained by different methods are not comparable. Median values are two times or more (in higher concentrations) different between results obtained by 2nd or 3th generation of methods. Differences between methods of the same generation are also significant. Number of patients on the hemodialysis with values in required interval of decision limits according to KDIGO 2009 was strongly dependent on the used method and maximum of these differences was approximately 250%. In our opinion is introducing the 3th generation methods of measurement PTH first step to improving the use of PTH in chronic renal diseased patients. Definitive step to rational use of PTH measurement is standardization. Myelodysplastický syndrom (MDS) je jedno z nejčastějších hematologických onemocnění ve věku nad 60 let. Přes intenzivní výzkum je však terapie této heterogenní krevní choroby velmi komplikovaná. V posledních letech se objevily 2 nové terapeutické přístupy, a to imunomodulační léčba a demetylační terapie. Zvláště imunomodulační léčba, reprezentovaná preparátem lenalidomid, znamená velký pokrok v léčně anemických nemocných specificky s 5q- aberací. Odpověď, především dosažení transfuzní nezávislosti, se pohybuje v původních studiích kolem 60–80 %. U nás byla tato terapie zahájena v roce 2008. Prezentujeme analýzu 34 nemocných, 31 žen, 3 mužů s mediánem věku 69 (46–80 let), z toho 27 nemocných (79,4 %) mělo diagnózu 5q- syndromu, 5 nemocných diagnózu refrakterní anémie s multilineární dysplazií, 1 nemocný refrakterní anémii s excesem blastů (RAEB 1) a 1 pacient neklasifikovatelný myelodysplastický syndrom/myeloproliferativní syndrom. Data nemocných jsou ukládána do lenalidomidové lišty Českého MDS registru. Jde o nemocné chronicky transfundované s diagnózou MDS s nižším rizikem a 5q- aberací. V našem souboru jsme zaznamenali odpověď na terapii reprezentovanou dosažením kompletní transfuzní nezávislosti u 91 % nemocných. Kompletní cytogenetická odpověď byla dosažena u 15 % nemocných a parciální odpověď u 67 %. U 15 % nemocných byla zjištěna mutace tumor supresorického genu TP53 (3 z 18 vyšetřených nemocných, 2 z nich progredovali do MDS s vyšším rizikem a následně do AML). Lenalidomid byl u většiny nemocných velmi dobře tolerován. Na tomto, byť zatím malém souboru prezentujeme vysokou efektivitu tohoto preparátu a základní principy a problematiku terapie lenalidomidem., Myelodysplastic syndrome (MDS) is a common hematological disease in patients over sixty. Despite intensive research, the therapy of this heterogeneous blood disease is complicated. In recent years, two new therapeutic approaches have been proposed: immunomodulation and demethylation therapy. Immunomodulation therapy with lenalidomide represents a meaningful advance in the treatment of anemic patients, specifically those with 5q- aberrations. As much as 60–70 % of patients respond and achieve transfusion independence. We present the initial lenalidomide experience of the Czech MDS group. We analyze Czech MDS register data of 34 (31 female; 3 male; median age 69 years) chronically transfused low risk MDS patients with 5q- aberration treated by lenalidomide. Twenty-seven (79.4 %) patients were diagnosed with 5q- syndrome, 5 patients with refractory anemia with multilineage dysplasia, 1 patient with refractory anemia with excess of blasts 1, and 1 patient with myelodysplastic/myeloproliferative unclassified. Response, as represented by achieving complete transfusion independence, was achieved in 91 % of patients. A true 5q- syndrome diagnosis in most our patients may be responsible for such a high response rate. Complete cytogenetic response was reached in 15 % of patients and partial cytogenetic response in 67 %, within a median time of 12 months. TP53 mutation was detected in 15 % (3 from 18 tested) and 2 of these patients progressed to higher grade MDS. The majority of patients tolerated lenalidomide very well.
