- Článek popisuje terapeutické spektrum možností a jeho specifika u gastrointestinálního stromálního tumoru (GIST) se zaměřením na adjuvantní léčbu. V jeho úvodní části se věnuje obecným poznatkům o GIST – epidemiologii, histopatologii či symptomatologii. Vzhledem k faktu, že GIST patří mezi radiorezistentní a chemorezistentní tumory, účinnost imatinib mesylátu (Glivec®), selektivního inhibitoru tyrozinkinázových domén receptorů c-kit, PDGFRA-alfa a bcr-abl, znamenala výrazný průlom v terapii tohoto nádoru. V další části proto článek shrnuje současné poznatky o indikacích imatinibu a popisuje determinaci rizika rekurence – pro GIST dlouho nebyla vytvořena TNM klasifikace, míra rizika recidivy byla tedy stanovována na základě několika kritérií, jako je velikost tumoru, počet mitóz na 50 patologických polí při histologickém vyšetření nádoru, lokalizace a ruptura tumoru, z nichž také nová TNM klasifikace vychází. Současně článek přináší přehled zásadních klinických studií pro adjuvantní podávání imatinibu a zmiňuje se i o možnostech terapie pro pacienty s metastatickým či pokročilým onemocněním, resp. o další linii biologické léčby, sunitinibu (Sutent®)., The paper describes the spectrum of therapeutic options and its specific features in gastrointestinal stromal tumour (GIST) with a focus on adjuvant therapy. The introduction deals with general information related to GIST, i. e. epidemiology, histopathology and symptomatology. Given the fact that GIST is among radioresistant and chemoresistant tumours, the efficacy of imatinib mesylate (Glivec®), a selective tyrosine kinase inhibitor of c-kit, PDGFRA-alpha and bcr-abl, has been a significant breakthrough in the treatment of this tumour. Therefore, the paper also summarizes the current knowledge of the indications for imatinib treatment and describes the assessment of the risk of recurrence – there had been no TNM classification for GIST for a long time; therefore, the risk of recurrence was established based on several criteria, including the tumour size, the number of mitoses per 50 high-power fields on the histological examination of the tumour, and the tumour site and rupture, criteria which are also used in the new TNM classification. Additionally, the paper presents an overview of clinical trials essential for adjuvant imatinib treatment and deals with the treatment options for patients with metastatic or advanced disease as well as with another line of biological therapy, sunitinib (Sutent®)., Michaela Zezulová, Bohuslav Melichar, and Lit.: 13

- U onkologických pacientů, léčených chemoterapií, je velmi častý výskyt anémie. U těchto pacientů jsou při léčbě anémie účinné preparáty stimulující erytropoézu (ESA), avšak existuje možnost vyššího rizika trombovaskulárních příhod. V posledních randomizovaných klinických studiích byly dosaženy odlišné výsledky týkající se dopadu ESA na celkové přežití a mortalitu. V tomto přehledu je provedeno porovnání studií, uvádějících vyšší mortalitu spojenou s aplikací preparátů stimulujících erytropoézu (ESA) a studií, které vyšší mortalitu spojenou s aplikací preparátů stimulujících erytropoézu (ESA) neuvádějí. Tento přehled pojednává o aktuálním pohledu na léčbu anémie u pacientů s nádorovým onemocněním. Předběžné výsledky současné metaanalýzy nezávislých údajů o pacientech zaměřené na úmrtí během studie a celkové přežití u pacientů léčených chemoterapii (jediná onkologická populace, u které je v současné době indikována terapie preparáty stimulujícími erytropoézu (ESA)) neukazují statisticky významný rozdíl mezi skupinami pacientů léčených preparáty stimulujícími erytropoézu (ESA) a kontrolními skupinami (riziko úmrtí během studie (HR): 1,10; 95% interval spolehlivosti (CI), 0,98–1,24; celková pravděpodobnost přežití: 1,04; 95% CI: 0,97–1,11, ve srovnání s kontrolní skupinou). V tomto článku jsou diskutovány možné příčiny, které by výsledky studie mohly ovlivnit. Neexistují přesvědčivé důkazy o tom, že by preparáty stimulující erytropoézu (ESA) stimulovaly růst tumorů. Preparáty stimulující erytropoézu (ESA) snižují potřebu transfuzí RBC, dosahují zvýšení hladiny hemoglobinu na cílové hodnoty a tento léčebný efekt preparátů stimulujících erytropoézu (ESA) nemá průkazný dopad na celkové přežívání nebo mortalitu, pokud jsou preparáty stimulující erytropoézu (ESA) užívány podle aktuálních indikačních doporučení. Nicméně je na základě aktuálně dostupných údajů a metaanalýz nutno použití preparátů stimulujících erytropoézu (ESA) pečlivě posuzovat z hlediska možného nebezpečí vyšší mortality., Anemia commonly occurs among cancer patients receiving chemotherapy. In these patients, erythropoiesisstimulating agents (ESAs) are effective in managing anemia but there is an increased risk for thrombovascular events. In more recent randomized clinical trials, there have been differing results regarding the impact of ESAs on overall survival and mortality. The balance between studies that show higher ESA-associated mortality and those that don’t show ESA-associated mortality is examined in this review. This review discusses where we stand today on anemia management in cancer patients. Preliminary results from a recent independent patient data meta-analysis for on-study deaths and overall survival in patients receiving chemotherapy (the only oncology population for which ESA treatment is currently indicated) showed no statistically significant difference between the ESA and control groups (onstudy deaths hazard ratio [HR], 1.10; 95% confidence interval [CI], 0.98 –1.24; overall survival HR, 1.04; 95% CI, 0.97–1.11, compared with controls). Possible factors that could influence study results are discussed in this review. There are no convincing data to support ESAinduced tumor stimulation in patients. ESAs decrease RBC transfusion needs and sustain targeted hemoglobin levels, and this ESA response does not significantly impact overall survival or mortality when ESAs are used within guidelines and labeling.
