Ramsay-Huntov syndróm je výsledkom reaktivácie varicella-zoster vírusu na úrovni ganglion geniculatum. Syndróm je charakterizovaný kombináciou periférnej tvárovej paralýzy, výsevom herpetiformnej vyrážky v oblasti tváre, najčastejšie v oblasti zvukovodu a ušnice, a možným pridružením kochleo-vestibulárnych symptómov. Zlatým štandardom v liečbe Ramsay-Huntovho syndrómu je aj naďalej dľa dostupných údajov kombinácia antivírusovej terapie s kortikosteroidmi a adekvátnou analgetickou terapiou. V kazuistike je prezentovaný prípad 55-ročnej pacientky., The Ramsay-Hunt syndrome results from reactivation of the varicella-zoster virus at the geniculate ganglion level. The syndrome is characterized by a combination of peripheral facial paralysis and a herpes-like rash occurring on the face, most commonly in the ear canal and the skin of the external ear, and the possible association with the cochlea-vestibular symptoms. According to the available data, combination of an antiviral therapy with corticosteroids and adequate analgesic therapy remains the gold standard of the treatment of the Ramsay-Hunt sydrome. We present a case of a 55-year-old patient. Úvod: Splenóza predstavuje autotransplantáciu tkaniva sleziny do atypických miest v brušnej dutine, hrudníku alebo iných častiach organizmu. Kazuistika: Autori popisujú prípad splenózy u 35 ročnej ženy, ktorá bola hospitalizovaná s bolesťami brucha, a u ktorej bola pred 19 rokmi vykonaná splenektómia pre traumatickú ruptúru s implantáciou časti sleziny do omenta. Pre intenzívne ťažkosti bola indikovaná chirurgická revízia. Histopatológ potvrdil diagnózu splenózy v exstirpovanom tkanive. Záver: Napriek upusteniu od autotransplantácie tkaniva sleziny po splenektómii sa môže chirurg stále stretnúť s diagnózou splenózy u pacientov po traumatických ruptúrach sleziny. Každý náhodný nález splenózy pri inej operácii je vhodné histologizovať., Introduction: Splenosis is the autotransplantation of splenic tissue in atypical locations in the abdomen, chest or other parts of the body. Case report: Authors present a case of splenosis in a 35 years old woman hospitalized with abdominal pain, who underwent splenectomy for traumatic rupture with splenic tissue implantation in the omentum 19 years ago. Surgical revision was indicated for intensive pain. Histopathology confirmed the diagnosis of splenosis in the excised tissue. Conclusion: Despite the abandonment of splenic tissue autotransplantation after splenectomy, surgeons may still encounter the diagnosis of splenosis in patients after traumatic splenic rupture. Úvod: Frekvencia pankreatických metastáz sa pohybuje od 2 % do 5 % všetkých malígnych tumorov pankreasu. Pankreas je elektívnym miestom metastáz renálneho karcinómu a všeobecne sa vyskytujú v siedmej dekáde a sú obyčajne asymptomatické. Kazuistika: V kazuistike prezentujeme prípad 58 ročnej ženy s multifokálnym a izolovaným postihnutím pankreasu metastázami renálneho karcinómu, ktorý sme riešili totálnou duodenopankreatektómiou. Záver: V selektovaných prípadoch považujeme resekčný výkon na pankrease pri metastázach renálneho karcinómu za indikovaný a zlepšujúci celkové prežívanie pacientov. Pancreas is an elective site for metastases from renal carcinoma. Pancreatic metastases from renal cell carcinoma are frequently the only metastatic site and they typically occur in the seventh decade of life. Case report: We report 1 female patient (aged 58 years) with multifocal and isolated pancreatic metastases of renal cell carcinoma. Total duodenopancreatectomy was performed. Conclusion: In selected cases, pancreatic resection due to renal cell cancer metastases is deemed indicated, improving overall patient survival.
