Je zde popsána metoda pro specifické stanovení kyseliny močové v lidském séru. Byla testována účinnost různých deproteinačních činidel. Výsledky získané proměřením 300 vzorků sér pomocí vysoce účinné kapalinové chromatografie byly porovnány s výsledky získanými rutinní enzymatickou metodou. Vzorky sér byly deproteinovány pomocí kyseliny chloristé a supernatanty byly analyzovány vysoce účinnou kapalinovou chromatografií na reverzní fázi. Pro separaci byla užita kolona MAG 1, 4,6 x 150 mm, Biospher PSI 200, 5 µm. Mobilní fází byla směs 5% metanolu v 25 mmol/l dihydrogenfosforečnanu sodného (v/v), pH 4,75. Analytické parametry metody jsou vyhovující: variační koeficienty přesnosti v sérii a mezi sériemi se pohybovaly pod 5 %. Výtěžnost, určená metodou standardních přídavků, se pohybovala v rozmezí 90,0–103,4 %. Mez stavitelnosti byla 10,0 µmol/l (3,3 pmol/nástřik). Výsledky získané chromatografickou metodou dobře korelovaly s výsledky získanými rutinní enzymatickou metodou, avšak chromatografická metoda poskytovala vyšší hodnoty, a to v závislosti na dané skupině pacientů. Zde prezentovaná metoda je vhodná pro analýzu těch vzorků, u nichž klasická enzymatická metoda poskytuje nespolehlivé výsledky., A sensitive method for the specific determination of uric acid in human serum is described. The effectiveness of various protein precipitants was tested. Results from measurements by high-performance liquid chromatography on 300 serum samples were compared with an enzymatic method. Serum samples were deproteinized with perchloric acid and analyzed by reversed-phase high-performance liquid chromatography. For the separation, a reverse phase column MAG 1, 4.6 x 150 mm, Biospher PSI 200 C18, 5 µm, was used. The mobile phase consisted of 5% methanol in 25- mmol/L sodium dihydrogenphosphate (v/v), pH 4.75. The analytical performance of this method is satisfactory: the intra-assay and inter-assay coefficients of variation were below 5%. Quantitative recoveries of spiked serum samples were between 90.0 – 103.4%. The limit of quantification was 10.0 µmol/L (3.3 pmol/inject). Results obtained by chromatographic method correlated well with an enzymatic method, but gave at average higher values depending on a group of patients. Ačkoliv vznik alergické rýmy či alergického bronchiálního astmatu bývá obvykle spojován s dětským věkem, nemusí tomu tak být zcela. Prevalence alergických onemocnění v seniorském věku je udávána kolem 5 %. Alergická onemocnění ve věku nad 65 let nezahrnují pouze alergická onemocnění dýchacích cest. Mezi časté obtíže jsou uváděny kožní alergické projevy a lékové alergie/ intolerance, alergie na hmyzí bodnutí. Pokud byla inhalačními alergeny testována populace nad 80 let, byly pozitivní prick testy ještě v 7 % testované seniorské skupiny. Alergická rýma provázena slizničním eozinofilním zánětem je lokálním projevem systémového alergického onemocnění. Jedná se o imunopatologický stav rizikový z hlediska rozvoje bronchiálního astmatu, proto je důležité seniorského rinitika odeslat k funkčnímu vyšetření plic. Fenotyp bronchiálního astmatu ve stáří není zásadně odlišný, ačkoliv imunosenescence modifikuje obraz bronchiálního astmatu. Negativní vliv na průběh bronchiálního astmatu má i přirozený pokles síly dýchacích svalů, zvýšená celková rigitida hrudního koše a pokles plicní elasticity v seniorském věku. Stupňovitá farmakoterapie bronchiálního astmatu k dosažení kontroly nad chorobou vyžaduje pravidelnou léčbu jak protizánětlivou, tak bronchodilatační. Snažíme se o jednoduchý léčebný režim s dávkováním maximálně 2krát denně. Významná je volba inhalačních léků s častou kontrolou správné techniky inhalace., Even though the onset of allergic rhinitis and allergic bronchial asthma is usually being linked to childhood, this may not always be the case. The prevalence of allergic diseases reported in older age patients is about 5%. Allergic diseases in patients above 65 years of age include more than just the respiratory tract disorders. Allergic skin reactions, allergic drug intolerance and allergic reactions to insect bites are among the most frequent complaints. Inhalation allergy testing in a population of patients above 80 years of age resulted in positive prick tests in as many as 7% of the senior population. Allergic rhinitis associated with mucosal eosinophilic inflammation is a local sign of a systemic allergic disorder. This is a high risk immunopathological state particularly with respect to potential development of bronchial asthma; this is why it is imperative to refer an older patient with rhinitis for pulmonary function assessment. Older age bronchial asthma phenotype is not essentially different even though immunosenescence modifies the picture of bronchial asthma. Natural loss of respiratory muscle tone, increased rigidity of thoracic cage and a reduction of lung elasticity in older age all have a negative effect on the course of bronchial asthma. Stepwise approach to pharmacological asthma management requires continuous anti-inflammatory as well as bronchodilator treatment. We should aim for a simple regimen with no more than twice daily dosing. Important is the choice of inhalation agents and frequent inspections of the inhalation technique., I. Krčmová, B. Background. Nonunion of the lateral humeral condyle are of the complex pathology of the elbow joint, occurring relatively often and resulting in disability of children. The treatment of nonunion of the lateral humeral condyle of humerus with cubitus valgus remains controversial. Purpose of this report was improvement of the results of surgical treatment of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus with use of differential approach to the surgical strategy. Material and methods. We were observing 28 children (17 boys and 11 girls) with nonunion and longstanding nonunion of lateral condyle of humerus, with various degrees of severity. There were used MRI and Xray investigations for differential study of the patients divided into 3 groups in relation to stability and nonstability of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus. Results. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarov’s technique. The mean postoperative humerusulna angle was 6,0 degrees of cubitus valgus. All of reverse Tosteothomies healed uneventfully, and there was no loss of correction postoperatively. The mean duration of followup was 7 years. The overall results were excellent in 15(53,5%) patients, good in 11(39,3%) patients, and fair in 2(7,2%) patients. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarov’s technique. Autoři referují dva případy náhodně zjištěného duplicitního tumoru pankreatu v ponechané části slinivky po parciální resekci pro adenokarcinom hlavy slinivky. Důvodem odstranění ponechaného zbytku slinivky byla vždy pooperační komplikace. Je diskutována možná četnost těchto duplicitních nádorů slinivky vzhledem k známému pohledu na vznik duktálního adenokarcinomu pankreatu z prekanceróz, tedy pankreatických duktálních intraepiteliálních neoplazií. Současně je hodnocen možný význam těchto němých duplicit v klinické praxi., The authors refer two cases of incidentally founded synchronous pancreatic tumors in the remaining pancreas following partial pancreatic resection for adenocarcinoma of the head of the pancreas. The reasons for removing the retained pancreas were postoperative complications. The frequency of these synchronous tumors is discussed with regards to the newly described development of pancreatic adenocarcinoma from pancreatic ductal intraepithelial neoplasia.
