Diabetes 1. typu (DM1) vzniká jako následek autoimunitní destrukce beta buněk pankreatu. Vliv diabetu na celkový výsledek těhotenství je poměrně dobře prostudován. Mnohem méně se toho však ví o vlivu diabetu matky na vyvíjející se imunitní systém plodu. Riziko rozvoje DM1 u dětí diabetických matek (pokud onemocněly před těhotenstvím) je nižší než např. u dětí diabetických otců. Tato diskrepance podporuje teorii, že by mohlo docházet k určitému navození imunotolerance u dětí DM1 matek. Z tohoto důvodu jsme si pro náš výzkum vybrali T regulační buňky (Tregs), které hrají důležitou roli v imunoregulaci a patogenezi mnoha autoimunitních onemocnění. Studie Tregs pupečníkové krve DM1 matek jsou navíc ojedinělé. Pomocí průtokové cytometrie jsme analyzovali zastoupení Tregs (definovaných jako CD45+ CD3+ CD4+ CD25+ CD127(low/-)) ve vzorcích pupečníkové krve od 17 matek s DM1, 17 s gestačním diabetem a 42 zdravých kontrol. Signifikantní rozdíl jsme nalezli jen v zastoupení Tregs z Th lymfocytů, kde v pupečníkové krvi zdravých matek činil tento poměr 9,25 % a u matek s DM1 6,73 % (p=0,043). Ostatní rozdíly nedosáhly signifikance. Pokud tedy dochází k navození specifické imunotolerance, je tato patrně podmíněna jinak než změnou počtu Tregs. Pokračujeme proto ve funkční analýze Tregs s cílem nalézt vysvětlení pro nižší výskyt diabetu u dětí DM1 matek ve srovnání s ostatními prvostupňovými příbuznými DM1 pacientů. Pochopení tohoto jevu by mohlo přispět např. i k vytvoření imunointervenční terapie DM1., Type 1 diabetes (T1D) develops due to autoimmune pancreatic beta cells destruction. The effect of diabetes on pregnancy outcome is well documented. However, much less is known about the effect of maternal diabetes on the developing foetal immune system. The risk of T1D development in a child of T1D mother who was diagnosed prior pregnancy is lower than e.g. in a child of T1D father. This discrepancy supports the theory that there is some immunoregulatory influence on her baby. For this reason we decided to study T regulatory cells (Tregs) which are important in immunoregulation and for autoimmune diseases pathogenesis. Moreover Tregs cord blood (CB) studies of T1D mothers are limited. Tregs defined as CD45+ CD3+ CD4+ CD25+ CD127(low/-) in CB samples were investigated in our study by flow cytometry and 17 T1D mothers, 17 gestation diabetes mothers and 42 healthy controls were enrolled. We found just that CB from babies of T1D mothers contained 6,73 % Tregs/total Th-lymfocytes in comparison to 9,25 % in controls (p = 0,043). Other differences were not significant. If an establishment of specific immunological tolerance in foetus of T1D mother really exists, it seems to be rather due to other factors than just to changes in Tregs count (which was moreover found to be decreased). We are currently focused on explanation why children of T1D mothers suffer from diabetes less frequently than other first degree relatives by using functional analysis of Tregs. Cíl práce: Aktuální pohled na těhotné s roztroušenou sklerózou (RS), analýza výsledků těhotenství těchto nemocných ve FN Motol v letech 2003-2011. Typ studie: Retrospektivní analýza Soubor a metodika: Analýza 76 těhotenství pacientek s relaps-remitentní formou roztroušené sklerózy – posouzení vlivu gravidity na aktivitu onemocnění (použitá analgezie a laktace ve vztahu ke vzniku akutní ataky) a naopak vlivu onemocnění na průběh těhotenství (komplikace, vedení porodu a perinatální výsledky). Výsledky: Akutní ataka v graviditě se v souboru 76 žen vyskytla u 4 z nich (5,3 %, RR 0,08), do půl roku po porodu jsme akutní ataku zaznamenali u 17 žen (22,4 %, RR 0,46). Ataku mělo 18,4 % kojících žen (9/49) ve srovnání s 33,3 % žen (8/24), které nekojily, p = 0,2375 (OR = 0,45, 95 % CI 0,15-1,37). Epidurální analgezii k porodu mělo 13,6 % žen (3/22) s akutní atakou, 25,9 % žen (14/54) s atakou po porodu bylo bez analgezie, p = 0,3648 (OR = 0,45,95 % CI 0,12-1,76). Zhoršení choroby jsme neprokázali (EDSS 1,4, resp. EDSS 1,6). 75 % těhotenství bylo ukončeno nekomplikovaným vaginálním porodem. Výskyt závažných těhotenských komplikací nebyl zvýšen a perinatální výsledky byly srovnatelné s běžnou populací. Závěr: V souladu s recentními studiemi prokazujeme v našem souboru pokles aktivity choroby v graviditě s nárůstem počtu akutních atak po porodu. Ke zhoršení či progresi onemocnění však během 12 měsíců po porodu nedochází. Kojení ani použití epidurální analgezie nemá na vznik akutní ataky vliv, těhotenství probíhají ve většině případů fyziologicky, vedení porodu a novorozenecké výsledky se neliší od běžné populace. Gravidita je tedy pro stabilizované pacientky s relaps-remitentní formou RS bezpečná., Objective: Current view on pregnant women with multiple sclerosis (MS), the analysis of pregnancy outcomes of these women with MS in the Motol Hospital in the years 2003-2011. Design: Retrospective analysis Methods: Analysis of 76 pregnant women with MS – to assess the impact of pregnancy on the activity of the disease (breastfeeding and used analgesia in relation on the relapse rate post partum) and the impact of the disease on the course of pregnancy (pregnancy complications, differences in the management of labor, and perinatal outcomes). Results: Relapse during pregnancy occurred in 4 out of the 76 women (5.3% relapse rate – RR 0.08). 17 women experienced a post partum relapse in the first six months after delivery (22.4%, RR 0.46). 18.4% breastfeeding women (9/49, 18.4%) had post partum relapse in comparison with 33.3% women, who did not breastfeed (8/24, 33.3%), p = 0.2375 (OR=0.45, 95% CI 0.15-1.37). 13.6% women (3/22, 13.6%) with post partum relapse used epidural analgesia (EDA), 25.9% women (14/54, 25.9%) with relapse did not used it, p = 0.3648 (OR = 0.45, 95% CI 0.12-1.76). Pregnancy did not influence the progress of disability (EDSS 1.4 or EDSS 1.6). 75% women had uncomplicated vaginal delivery, the incidence of serious pregnancy complications was not increased, and the perinatal outcomes are comparable with the general population. Conclusion: Consistent with recent studies, we evaluated decreasing relapse rate during pregnancy. In the post partum period the relapse rate has increased, however the pregnancy did not influence the disability progress in 12 months postpartum. Neither breastfeeding nor epidural analgesia correlated with presence of post partum relapses. In most cases, the pregnancies in patients with MS were physiological, the method of delivery and the overall perinatal outcomes are comparable with the general population. Cíl studie: Vyhodnotit přínos interleukinu-6 u těžké akutní pankreatitidy pro odlišení sterilní nekrózy (SN) a nekrózy infikované (IN). Typ studie: Retrospektivní deskriptivní observační studie série případů. Metody: Ve vlastní práci bylo zahrnuto 59 pacientů hospitalizovaných s těžkou akutní pankreatitidou (Atlantská klasifikace 1992) na chirurgickém oddělení Nemocnice Písek v průběhu sedmi let. Pravidelně byly vyšetřovány C-reaktivní protein (CRP) a interleukin-6 (IL-6), přičemž dostatečná data byla u 42 pacientů: 14 s IN (průkaz mikrobiologicky či dle CT), 28 se SN. Statistické vyhodnocení pomocí Fisherova testu a chí-kvadrát testu. Výsledky: Byla nalezena statisticky významná diskriminační schopnost denních průměrů IL-6 5. a 6., resp. 7. a 8. den hospitalizace mezi IN a SN (p=0,0014 pro den 5 a 6, resp. p=0,0009 pro den 7 a 8). Rozvoj IN reálně nastával nejčastěji mezi 5. a 9. dnem (mikrobiologický průkaz agens či nepochybné CT známky infekce). CRP tuto schopnost diskriminace neprokázalo, jeho hladiny byly od 2. do 8. dne setrvale výrazně zvýšeny. Charakteristický byl průběh koncentrací IL-6, kdy vstupně byla přítomna výrazná elevace nezávislá na pozdějším rozvoji IN a již od druhého dne hospitalizace došlo k výraznému poklesu. K dalšímu vzestupu IL-6 došlo pouze u pacientů s rozvojem IN. Pro cut-off (diskriminace IN a SN) IL-6 (platný od 5. dne hospitalizace) 100 ng/l bylo p<0,005, pro 150 ng/l p<0,01. Závěr: Interleukin-6 v naší retrospektivní studii prokázal statisticky významnou schopnost diskriminovat infikovanou a sterilní nekrózu u těžké akutní pankreatitidy. Překročení cut-off IL-6 od 5. dne hospitalizace svědčilo pro přítomnost infikované nekrózy. Cut-off 100 ng/l diskriminoval obdobně (p<0,005, senzitivita 100 %, specificita 92 %) jako cut-off 150 ng/l (p<0,01, senzitivita 87 %, specificita 100 %)., Objective: Our retrospective study aimed to evaluate the contribution of interleukin-6 in severe acute pancreatitis. Mortality of severe acute pancreatitis ranges from 10-20% (for sterile necrosis, SN) to 20-85% (infected necrosis, IN). Resolution of SN and IN is clinically very important, but often difficult and therefore they are still searching for new laboratory markers for their differentiation. Design: Retrospective descriptive observational study of cases series. Methods: 59 patients hospitalized with severe acute pancreatitis (Atlanta classification 1992) in the surgery department of Hospital Písek in the 2000-2006 were included. Regularly they were investigated by C-reactive protein (CRP) and interleukin- 6 (IL-6), sufficient data was in 42 patients: 14 with IN (identified microbiologically or according to computed tomography, CT), 28 with SN. Statistical analysis was performed using Fisher‘s test and chi-square test. Results: There was found statistically significant discriminatory ability of daily averages of IL-6 5th and 6th + 7th and 8th day of hospitalization between the IN and SN (p = 0.0014 for day 5 and 6, respectively. p = 0.0009 for day 7 and 8). Developing IN realistically set in the most between 5th and 9th day (needle biopsy with identification of microbiological agents or CT signs of infection). The ability of CRP to discriminate SN and IN was not found, the levels were between 2nd to 8th days increased steadily. The run of the concentrations of IL-6 was characteristic, where at the admission there was present significant elevation irrespective of subsequent development of IN and from the second day of hospitalization it decreased significantly. Further increase in IL-6 was present only in patients with the development of IN. For the cut-off (discrimination of SN and IN) for IL-6 (valid from 5th of hospitalization) 100 ng/L was P<0.005, for 150 ng/L P<0.01. Conclusion: Interleukin-6 in our retrospective study showed a statistically significant ability to discriminate between infected and sterile necrosis in severe acute pancreatitis. Autoři ve svém příspěvku uvádějí první výsledky šetření, které bylo zaměřeno na zjištění hmotností školních tašek chlapců a dívek 1. a 2. tříd základních škol v olomouckém regionu. Celkem byla zjištěna hmotnost u 502 školních tašek, změřeno 502 dětí (263 dívek a 239 chlapců). Z výsledků měření je zřejmé, že hmotnost tašky se zvyšuje s postupem žáků do vyšší třídy. Největší rozdíl mezi minimální a maximální hodnotou hmotnosti tašky byl zjištěn u 8letých dívek, činil 8,9 kg. Výsledky ukázaly, že procentuální podíl hmotnosti tašky doporučeným 10 % tělesné hmotnosti dítěte odpovídal pouze u 17,5 % dětí. 82,5 % dětí pak nosí aktovky těžké (10–20 %), přičemž 18,3 % dětí nosí aktovky o hmotnosti nad 20 % své tělesné hmotnosti, což už lze považovat za extrémně těžké. Nepřiměřená hmotnost školní tašky pak může ovlivňovat držení těla a výskyt bolesti zad., Effects of the weights of school bags carried by pupils from the first two classes of primary schools in the Olomouc region (502 schoolbags belonging to 263 girls and 239 boys) on the pupils’ posture were examined. The weight of the schoolbags of pupils from the 2nd year class was found to be higher than of pupils in the first year. The greatest difference between minimum and maximum weights (8.9 kilograms) was detected in the schoolbags of 8 year old girls. Only 17.5% of children carried bags that were within the recommended 10% of the child’s body weight. AIMS: Authors studied potential side effects of fetal calf serum (FCS) in cultivation media on human dental pulp stem cells (DPSC) during long-term cultivation. METHODS: Two lines of DPSC obtained healthy donors (male 22 years, female 23 years) were used. Both lines were cultivated under standard cultivation conditions in four different media containing 10% or 2% FCS and substituted with growth factors. During long-term cultivation proliferation ability, karyotype and phenotype of DPSC were measured. RESULTS: Both lines of DPSC cultivated in a media containing 2% FCS and ITS supplement showed the highest number of population doublings. On the other hand the proliferation rate of DPSC cultivated in a media with 2% FCS without ITS supplement was slowest. Proliferation rate of DPSC cultivated in 10% FCS media with or without FGF-2 was comparable. DPSC cultivated in a media with 10% FCS showed a significantly higher amount of chromosomal aberrations. These chromosomal aberrations do not seem to be clonal but surprisingly we found large amounts of tetraploid cells in the 9th passage in both media containing 10% FCS. CONCLUSIONS: Our study proved that cultivation of DPSC in media containing higher concentration of FCS has critical side effects on cell chromosomal stability. and J. Suchánek, TS. Kleplová, M. Competencies develop and changes throughout a persons life, they can gain or lose, going through various age stages. Their development does not end in youth, but continue on through the adult life. The ability to thing and reflect those thoughts specifically come forward in the center of structure of competency, which grows at the same time as the individual matures. One of the competency types is health competency. Health competency is a relatively new concept; it is not sufficiently researched. Aim of the study was to determine the factors of an adult individual health competency. 827 respondents participated in the study, in the processing of data was used SPSS. Was used factor analysis, analysis of variance with ANOVA and KruskalWallis test and Pearsons correlation. It was found that health competency is affected by several factors. These are: health education, health behavior, and the value of the environment. Each of the sets was distributed to key factors. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). Cíl: Cílem studie je stanovit prevalenci chronického selhání ledvin u 101 pacientů s monoklonální gamapatií neurčeného významu (MGUS). Typ studie: retrospektivní studie. Materiál a metody: Porovnali jsme glomerulární filtraci odhadovanou ze sérového kreatininu a cystatinu C a hodnotili jsme korelaci mezi koncentrací volných lehkých řetězců v séru a funkcí ledvin. U všech pacientů jsme provedli elektroforézu pro - teinů séra s denzitometrickou kvantifikací monoklonálního imunoglobulinu a změřili jsme koncentraci kreatininu, cystatinu C, volných lehkých řetězců kappa a lambda, β 2 mikroglobulinu. Výsledky: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů na základě odhadu ze sérového cystatinu C a u 34,6 % pacientů na základě odhadu ze sérového kreatininu. B2mikroglobulin má největší počet významných korelací; cystatin C (0,90), CysC GFR (-0,69), Kreatinin (0,70), MDRD GFR (-0,60), MIg koncentrace (0,24), κ FLC (0,22) λ FLC (0,34) a věk (0,40). Tento velký počet korelací lze vysvětlit tím, že β 2mikroglobulin odráží jak zátěž organismu maligními buňkami, tak pokles glomerulární filtrace. Závěr: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů při odhadu glomerulární filtrace z cystatinu C a u 34,6 % při odhadu ze sérového kreatininu. Třída monoklonálního imunoglobulinu může mít vliv na výsledky odhadu glomerulární filtrace ze sérového cystatinu C, odhad glomerulární filtrace ze sérového kreatininu není ovlivněn třídou imunoglobulinu. Diabetici neměli statisticky signifikantně nižší glomerulární filtraci než pacienti bez diabetu mellitu., Background: Renal function is more frequently decreased in patients with monoclonal gammopathy. 10 % of patients with multiple myeloma present with acute renal failure and renal function is included in CRAB criteria for the diagnosis of multiple myeloma. Aim: The aim of this study is to determine the prevalence of chronic renal failure (CRF), defined as a glomerular filtration rate (GFR) below 1.0 ml/s/1.73m 2 , in 101 patients with monoclonal gammopathy of undetermined significance (MGUS). Design: retrospective study Material and methods: We compared GFR estimates from serum creatinine (MDRD GFR) and serum cystatin C (CysC GFR) and looked at the correlation between free light chains (FLC) concentration and renal markers. We performed serum protein electrophoresis (SPE) with quantification of monoclonal intact immunoglobulin (MIg) and measured serum creatinine, cysta - tin C, κ and λ FLC and β 2microglobulin in all patients. Results: We found CRF in 38.6% patients using estimation from serum cystatin C and 34.6 % using estimation from serum creatinine. B2microglobulin has the highest number of significant correlations; cystatin C (0.90), CysC GFR (-0.69), crea - tinine (0.70), MDRD GFR (-0.60), MIg concentration (0.24), κ FLC (0.22) λ FLC (0.34) and age (0.40). The best correlation results may be explained by the fact that β 2microglobulin reflects both malignant cells burden and renal function. Conclusions: We found CRF in 38.6 % MGUS patients using estimation from serum cystatin C and in 34.6 % using estima - tion from serum creatinine. The class of monoclonal immunoglobulin may influence results of glomerular filtration estimated from cystatin C but it does not influence GFR estimated from creatinine. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation.
