Rachels je autor zastávající novodobou koncepci utilitaristické morálky. Ve své publikaci The End of Life velmi podrobně probírá všechny důležité momenty filosofické a etické diskuse o eutanazii a vytyčuje tak kontury území, na němž se jak zastánci jeho závěrů, tak i jejich odpůrci, musí pohybovat. Hovoří o posvátnosti lidského života, rozebírá různé formy eutanazie, kriticky hodnotí některé důležité distinkce, široce a někdy nekriticky užívané v lékařské etice: rozdíl mezi usmrcením (killing) a ponecháním zemřít (letting die), mezi řádnými a mimořádnými léčebnými prostředky, mezi aktivní a pasivní eutanazií. Důležitým prvkem Rachelsovy reflexe je i diskuse o důležitosti úmyslu (intence) v etickém vyhodnocování lidských skutků., This contribution is the second of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the problems of euthanasia on the background of Rachel´s famous book The End of Life. Rachels as an author is holding a modern concept of utilitarian morality. In his publication The End of Life he discusses in detail all the important moments of philosophical and ethical debate about euthanasia. In this way he also outlines the contours of the territory for his proponents as well as their opponents for recent discussion on euthanasia and end of life decisions. He talks about the problems of conception of the sanctity of human life, discusses the various forms of euthanasia, critically evaluates some important distinctions between terminology in medical ethics: the difference between killing (killing) and allowing to die (letting die), between ordinary and extraordinary means of treatment, the active and passive euthanasia. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. The important biological role of saliva in maintaining of the homeostasis of the oral cavity environment, preventing infection and tooth decay is widely accepted. Salivary glands insufficiency may violate the balance between health and disease. Clinical data indicated that the destruction, agenesis and aplasia of salivary gland commonly followed with hypo salivation, low salivary flow, which resulted in severe caries and periodontal disease. Salivary gland dysfunction also frequently found in preterm and low birth weight newborns. But the information about structural background of abnormal salivation in early childhood is still limited. A presence of any correlation between salivary glands structural development and intrauterine restrictions of fetal growth (IUGR) is unclear. The aim of present study was to determine morphological and morphometric peculiarities of human parotid gland in case of IUGR at late gestation. Material and methods: Parotid glands of twenty human fetuses 2022 weeks of gestation with diagnosed IUGR from late abortions material were compared with ten fetal glands in cases of induced abortions due to psychological reasons (control group). Tissue samples were immersionfixed in 10% buffered formalin solution, embedded in paraffin wax. Histological slides were stained routinely with hematoxylin & eosin, with Van Gieson's Stain. Microscopical examination was performed on magnification x 40 and x 100. Stereometric study by point count method at magnification x 40 allowed finding out volume fractions (VF) of glands parenchyma and stroma. VF of lobules components (glands wall, glands lumen, ducts wall, ducts lumen, vessels, intralobular connective tissue) were registered at magnification x100. Morphometry of the secretory portion of the parotid gland was conducted on the Zeiss microscope with the help of the AxioVision Rel.4.8 program. The mature (differentiated) end pieces were measured, including their area (in mkm2), width, height, perimeter (all in mkm). Similar measurements were done in the foci of immature secretory ends of a gland. Additionally the mean height of the epithelial cells layer within the mature secretory end pieces was measured. The differences were analyzed by methods of mathematical statistics using the software Microsoft Excel; data was compared with control measurements by Students ttest. Results: The results of present research have shown the delayed differentiation of fetal parotid glands parenchymal components in case of IUGR. The parenchymal VF did not reach control values. Furthermore, VF of lobules components was also decreased. Ducts lumens appeared to be significantly narrower than at physiological gestation. Interlobular and intralobular connective tissue stroma, in contrast to the controls, occupied vast areas, and their volume fraction was increased. Deficit of the parenchymal components of the gland was enhanced by slower maturation of glands. In cases with IUGR, differentiated glands occupied smaller area, with reduced width, height and perimeter. Epithelium lining the differentiated glands is characterized by significantly lower height compared to the control group. Delayed differentiation resulted in higher proportion of immature glands. Their area, width, height and perimeter increased. IUGR was also accompanied with a variety of pathological changes. Conclusion: Present evidences suggest that IUGR leads to impaired growth and maturation of the parotid gland. Structural immaturity and lack of differentiated parenchymal elements of the organ may form the basis of its secretory functions lesion. Účelem této úvahy je stručně seznámit čtenáře s filosofickými základy pojmu kauzalita a porovnat tyto základy s koncepcí využívanou v deliktním právu. Z tohoto důvodu je nejprve využita deskriptivní metoda spočívající ve stručném shrnutí některých základních filosofických konstrukcí formulovaných v klíčových filosofických dílech. Článek proto poukazuje na historické pozadí vývoje pojmu kauzalita, jenž se v první fázi objevil v antickém Řecku. V druhé části se autor vyrovnává s některými současnými teoriemi, které vytyčily debatu v právní teorii a filosofii. Pojem příčiny a následku, příčinné souvislosti je zde pojímán v právním kontextu jeho využití. Autor odkazuje také na rozličný přístup k faktické a normativní kauzalitě a jejich využití v právní vědě i praxi. Posledním úkolem práce by mělo být vytyčení směrů, kterými by se legislativa při normotvorném procesu a justice při aplikaci práva, měly vydat., The purpose of this essay is to introduce a philosophical foundation into the ideas relating to Causality in Tort Law. This Article uses firstly historical and descriptive method in order to find crucial theories on Causation in famous philosophical works. In brief it deals with historical background to many of the issues and disputes surrounding causation that concern philosophers today. Causation´s role has been recognized since the ancient Greeks. The second part deals with the leading theories that have been the focal points of contemporary debate in the philosophy of law. In this part is central point the notion of “cause” as used in legal context. The distinction between factual and normative causation and its consequences is also mentioned. Fyziologická gravidita je sprevádzaná zmenami v lokálnej a systémovej koagulácii a fibrinolýze. Hemostáza zasahuje do procesu implantácie blastocysty a placentácie. Udržiava optimálne prekrvenie placenty, zabezpečuje rýchle a adekvátne zrážanie počas pôrodu. Mnohé koagulačné proteíny sú exprimované na začiatku embryonálneho vývoja a hrajú úlohu mimo koaguláciu v proliferácii a diferenciácii buniek. Mechanizmus regulácie v placente nie je dostatočne chápaný. Poruchy hemostázy sa podieľajú na peripartálnom krvácaní, gestačnom tromboembolizme a považujú sa za rizikový faktor neúspešnej reprodukcie a tehotenských komplikácií., Physiological pregnancy is accompanied by changes in local and systemic coagulation and fibrinolysis. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Mechanism regulating haemostasis within the placenta remain poorly understood. The results of morphological ivestigation of prostate tissue after vaporization prostatectomy by Laser Dornier Medilas D UroBeam and transurethral prostate resection in patients with benigh prostatic hyperplasia is shown. We found that the successive use laser Dornier Medilas D Urobeam at power 175-250 W and transurethral resectionof prostate provides diminishing of area of coagulative necrosis which prevents formation of massive scab. Cíl: cílem našeho šetření bylo vyhodnotit moţnou přítomnost bolesti u klientů s demencí, kteří byli přeloţeni na gerontopsychiatrické lůţko psychiatrické léčebny v Kroměříţi z jiného zdravotnického zařízení. Metodika: studium překladové ošetřovatelské dokumentace z odesílajících zdravotnických zařízení, škála pro hodnocení bolesti u pacientů s demencí (PAINAD). Soubor: klienti přeloţení do psychiatrické léčebny v Kroměříţi, obojího pohlaví, starší 65 let, pro demenci jakéhokoliv typu. Tíţe demence byla středně těţká a těţká (bylo hodnoceno při přijetí na gerontopsychiatrické lůţko celkovým skóre dosaţeným v Mini Mental State Examination - MMSE - v rozmezí 0 - 17). Výsledky: pomocí škály PAINAD byla zachycena bolest u 26,7 % přeloţených klientů se středně těţkou nebo těţkou demencí. Bolest však byla řádně zadokumentována v překladových ošetřovatelských zprávách jen u 8,96 % klientů. Téměř třetina ošetřovatelské překladové dokumentace (29,9 %) měla zcela nevyplněný, nebo nedostatečně vyplněný formulář pro hodnocení bolesti. Téměř pětina (19,4 %) ošetřovatelských překladových zpráv fyzicky vůbec neobsahovala ţádný takový formulář. Závěr: přítomnost nepodchycené nebo nedostatečně vyhodnocené bolesti u klientů s kognitivními poruchami je závaţným problémem moderního ošetřovatelství. Nutná je potřeba neustálé edukace zdravotnického personálu, kvalitního vedení ošetřovatelské dokumentace, ale také potřeba důslednějšího systému kontrol z řad vedoucích pracovníků., Aim: to evaluate presence of pain in clients of both sexes, older than 65, who have been admitted to Mental hospital Kroměříţ due to dementia of any type. Methods: study of nursing documentation from dispatching health care facilities, PAINAD scale (Pain assessment in advanced dementia). Severity of dementia was moderate of severe (this was evaluated by Mini-Mental State Examination total score during admission process to our hospital, scores ranged from 0 to 17). Results: pain was detected in 26.7% of clients with moderate or severe dementia by means of PAINAD scale, compared to 8.96% of clients with pain which has been reported in nursing documentation. Almost in one third of nursing documentation (29.9%) no data or incomplete data about pain were found. Almost one fifth (19.4%) of nursing documentation have not contained any tool for pain screening. Concusion: undiagnosed pain in cognitively impaired clients is a severe problem of nursing care. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cíl: Cílem studie bylo zjistit spokojenost pacientů s poskytovanou ošetřovatelskou péčí na vybraných odděleních dvou nemocnic Moravskoslezského kraje. Porovnat spokojenost pacientů podle zdravotnického zařízení a oddělení, kde byli hospitalizováni. Zjistit, zda existuje souvislost mezi demografi ckými a klinickými charakteristikami pacientů a jejich spokojeností s ošetřovatelskou péčí. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time. Jaká je hodnota života, či poněkud specifičtěji, jaká je hodnota lidského života? Podle nauky o posvátnosti lidského života má každý lidský život svou vnitřní hodnotu, tato teorie však v současné bioetice rozhodně nepředstavuje ortodoxii. John Harris je jedním ze zastánců nové teorie hodnoty lidského života (především ve své publikaci The Value of Life), podle níž jsou vnitřně hodnotné pouze osoby. Osoby se definují jako bytosti - lidského či jiného druhu - schopné vyšších kognitivních funkcí, které díky tomu mohou hodnotit svůj vlastní život (vědomí sebe sama je nutnou podmínkou osoby) a mohou si přát dále žít. Hlavním cílem této kritické studie je zhodnocení Harrisových argumentů. V první části se odhaluje obecná logická struktura jeho argumentace a zdůrazňují se její důsledky pro některá témata současné bioetiky (interrupce, infanticida, eutanazie). V druhé části práce se odhalují nedostatky Harrisova myšlení. Ukazuje se, že jeho argumentace je kruhová, předpokládá teorii, kterou se snaží odvodit. Studie uzavírá, že Harrisova teorie hodnoty lidského života je nesprávná a nevhodná pro současnou bioetickou diskusi., What is the value of life? Or more specifically, what is the value of human life? According to the doctrine of the sanctity of human life, all human beings are intrinsically valuable, but this theory is far from being orthodoxy in current bioethics. John Harris is one of the advocates of the new theory of the value of human life (mainly in his book The Value of Life), according to which intrinsically valuable are only persons. Persons are beings -human or not--capable of higher mental functions that consequently can value their own life (self-consciousness is a necessary condition for personhood) and can have the desire to go on living.The overall aim of this critical study is to go into Harris's arguments in some depth. In the first part the general logical structure of Harris's argumentation is put forward and its consequences for current bioethical issues (abortion, infanticide, euthanasia) are pointed up. In the second part two fatal flaws of Harris's thoughts are exposed. It is showed that his argumentation is circular, it presupposes theory it aims to develop.
