Pseudomyxoma peritonei je vzácný klinický syndrom; nádorový proces, pro který je charakteristické hromadění mucinózního ascitu a peritoneálních implantátů v břišní dutině. Primárním zdrojem je téměř vždy low grade mucinózní tumor apendixu. Jedná se o pomalu progredující, nízce agresivní, ale smrtící onemocnění s častými recidivami. Optimálním léčebným postupem je kompletní chirurgická cytoredukce (CRS) v kombinaci s peroperačně aplikovanou intraperitoneální hypertermickou chemoterapií (HIPEC) – tzv. Sugarbakerova metoda., Pseudomyxoma peritonei (PMP) is an uncommon clinical syndrome characterized by the slow and progressive accumulation of peritoneal implants and mucinous ascites, intraabdominal gelatinous collection (jelly belly). PMP is reported to originate from the low grade appendiceal mucinous neoplasm. Pseudomyxoma peritonei often recurs after treatment and may eventually cause death by abdominal visceral dysfunction via compression with mucinous ascites. Cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy comprising hyperthermic intraperitoneal chemotherapy (HIPEC) with or without postoperative intraperitoneal chemotherapy (EPIC) has been regarded as the standard of care in specialized centers. It is often referred to as the „Sugerbaker procedure“. Conventionally, PMP is considered resistant to systemic chemotherapy. Even though complete cytoreduction is associated with prolonged overall survival, recurrence of disease is common and multiple operations are frequently required. Patients may enjoy sustained period of remission, free of symptoms, but long – term disease free survival is distinctly uncommon. The pathologic subtype remains the dominant factor in survival. Karcinom štítné žlázy patří celosvětově k nádorům s nejrychleji rostoucí incidencí. Velký vzestup počtu nových onemocnění je zaznamenán hlavně u diferencovaných papilárních karcinomů štítné žlázy. V České republice je zdokumentováno, že výskyt rakoviny štítné žlázy neustále roste, od roku 1980 se zvýšil 4krát. Jde o vyšší výskyt než ve většině ostatních evropských zemí, zároveň se řadíme mezi země s průměrnou a klesající úmrtností na toto onemocnění. Česká republika patří celosvětově mezi země s nejvyšším výskytem kolorektálního karcinomu (KRK), jehož incidence však mezi českými kraji není distribuována rovnoměrně. Ve všech epidemiologických ukazatelích (incidence, mortalita i prevalence) tohoto maligního onemocnění dominuje Plzeňský kraj. Vzhledem k prodlužování průměrné délky života vzrostl v posledním desetiletí podíl KRK jako druhého metachronního primárního maligního onemocnění. V roce 2000 byl zahájen Národní program screeningu KRK, který t. č. nabízí asymptomatickým jedincům starším 50 let možnost výběru ze dvou screeningových metod (testy na okultní krvácení a primární screeningová kolonoskopie). Od roku 2006 bylo v rámci screeningu kolonoskopicky vyšetřeno 57 255 osob, u 17 760 (31,0 %) z nich byly diagnostikovány a odstraněny adenomové polypy a u 2 665 (4,7 %) byl nalezen karcinom., The Czech Republic belongs to the countries with highest colorectal cancer (CRC) incidence worldwide which differs in regions of this country. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. Z 30 166 případů leukemií, evidovaných v českém registru nádorů v letech 1976–2005 bylo 3 937 vícečetných případů spojeno s výskytem jiných novotvarů. Z nich se 2 450 (62,2 %) vyskytlo u mužů a 1 487 (37,8 %) u žen, tj. 14,7% případů u mužů a 11 % u žen ze všech evidovaných leukemií. Po 1 715 primárních leukemiích bylo diagnostikováno 2 064 jiných novotvarů a před 2 222 následnými leukemiemi 2 642 jiných primárních nádorů. Ze 14 krajů se ve třech vyskytlo 35,5 % vícečetných leukemií (Jihomoravský a Severomoravský kraj, Praha). V průměru za 4,9 roku u mužů a 5,5 roku u žen následovalo 35,1 % nádorů kůže a melanom, 16,9 % nádorů trávicích, 13 % dýchacích a 8,4 % močových cest. Očekávaný nízký počet pokročilých klinických stadií u následných nádorů se nepotvrdil: u mužů zahrnoval 43,8 % nádorů dutiny ústní, 41,5 % dýchacích, 34,2% trávicích, 10,6 % močových cest a 15,7 % pohlavních orgánů; u žen 39,4 % dýchacích, 30,6 % močových, 30 % trávicích cest a 28,2 % pohlavních orgánů, 25 % prsů, 25 % dutiny ústní. Bohužel mnoho klinických stadií nebylo v registru nahlášeno. Před následnými leukemiemi bylo nejvíce primárních karcinomů kůže a melanomu u 43,6 % případů jako ukazatel maligního stavu. Do října 2007 přežívalo u mužů jen 12,4 % primárních a 10,3 % následných leukemií, respektive 19,7 % a 14 % u žen. Kolik dalších novotvarů se vyskytne u osmi tisíc přežívajících s leukemií, očekávaných v roce 2015? Unfortunatelly a lot of clinical stages were not registered. Before the subsequent leukemias the most frequent were 43.6 % primary cancers of skin and melanoma as a sign of malignant status. Up to October 2007 in males survived only 12.4 % of primary and 10.3 % subsequent leukemias, respective 19.7 % and 14 % in females. How many other neoplasms will be diagnosed among 8,000 surviving with leukemia, estimated in the Czech Republic in 2015? Za posledních 46 let bylo u české populace evidováno 56 828 onemocnění nádorem pankreatu (PC) dg. C25, s incidencí 19,1 na 100 tis. mužů a 18,2 na 100 tis. žen v roce 2007. Prevalence PC vzrostla mezi roky 1989–2005 u mužů o 45,9 % na 629, u žen o 119,1 % na 241. Z 3 632 vícečetných nádorů PC, evidovaných za období 1976–2005 v registru, bylo u mužů 321 (1,5 %) primárních a 1 581 (7,4 %) následných, u žen 241 (1,2 %) primárních a 1 489 (7,7 %) následných, které představovaly 11 % ze všech nově hlášených PC. Duplicitní PC byl početnější u žen, multiplicitní u mužů. Počet primárních PC se zvýšil na 34 případů a následných PC na 258 v r. 2005. Téměř polovina vícečetných PC u mužů a třetina u žen byla evidována ve věku 35–64 let. Z 3 632 vícečetných PC se více vyskytlo v kraji Severomoravském 11,8 %, Jihomoravském 11,1 % a v Praze 8,1 %. Průměrný interval mezi primárním PC a jinými následnými novotvary (dg. C00–D48) byl 3,2 roku u mužů a 2,1 roku u žen, poměr synchronních a metachronních případů byl 1:0,7 u mužů a 1:0,5 u žen. Nejpočetnější po primárních PC byly u mužů nádory trávicích, dýchacích a močových cest a pohlavních orgánů, u žen nádory trávicích cest, pohlavních orgánů, močových cest a prsu. Následným PC nejčastěji předcházely u mužů primární nádory kůže, trávicích, dýchacích a močových cest a pohlavních orgánů, u žen nádory kůže, pohlavních orgánů, trávicích cest a prsu.
