92. Content validation of the diagnosis ineffective breastfeeding and readiness for inhanced breastfeeding

- Východiska: Implementace mezinárodní klasifikace NANDA-I v podmínkách neonatologické péče v ČR. Cíl: Cílem práce bylo validovat diagnostické prvky ošetřovatelských diagnóz 00104 Neefektivní kojení a 0106 Snaha zlepšit kojení vybraným souborem neonatologických sester – expertek a zjistit, které diagnostické prvky považují za hlavní a které za vedlejší. Metody: Pro obsahovou validaci byl využit Fehringův model validity diagnostického obsahu (Diagnostic Content Validity Model). Soubor tvořilo 26 neonatologických sester – expertek. Za expertky byly považovány sestry pracující na neonatologii, které získaly minimálně 4 body dle modifikovaných Fehringových kritérií. Pro posouzení významnosti diagnostických prvků byl použit měřící nástroj obsahující 43 položek (diagnostických prvků) uvedených v klasifikačním systému NANDA-International. Výsledky: U ošetřovatelské diagnózy 00104 Neefektivní kojení za významné určující znaky (vážené skóre vyšší než 0,80) expertky považovaly: kojenec není schopen správně uchopit prs (0,82). Ze souvisejících faktorů dosáhlo váženého skóre vyšší než 0,80 celkem 3 prvky. U ošetřovatelské diagnózy 0106 Snaha zlepšit kojení váženého skóre vyšší než 0,80 nedosáhl žádný prvek. Závěr: Pro posouzení nespokojenosti nebo potíží matky a dítěte při procesu kojení na oddělení šestinedělí považují expertky za významné 4 prvky z 30, týkající se pouze ošetřovatelské diagnózy 00104 Neefektivní kojení., Background: Implementation of the International Classification of NANDA-I in neonatology care in the Czech Republic. Aim: The aim of the study was to validate the diagnostic elements of nursing diagnoses 00104 Ineffective Breastfeeding and 00106 readiness for inhanced breastfeeding selected file by neonatal nurses-experts and determine which diagnostic elements consider as the main, and which the minor are. Methods: For the content of validation was used Fehring diagnostic content of validity model (Diagnostic Content Validity Model). The sample consisted of 26 neonatal nurses-experts. For expert were considered nurses working in neonatology, which received at least 4 points modified by Fehring standard. To assess the significance of diagnostic elements was used measurement instrument containing 43 items (diagnostic elements) listed in the classification system, NANDA-International. Results: At nursing diagnosis 00104 Ineffective Breastfeeding, the main key features (score > 0, 80) experts considered: the infant isn´t unable to correctly to grasp the breast (0, 82). From the related factors achieve score 0, 80 in all 3 elements. At the nursing diagnosis 00106 Effort to improve breastfeeding weighted score of > 0, 80 reached no character. Conclusion: For the assess of dissatisfaction or problems of mother and child at the process of breastfeeding on postpartum department, specialists considered to be significant of 4 members from 30, concerning only to nursing diagnoses 00104 Ineffective breastfeeding., and Jana Chrásková, Mária Boledovičová

- Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Klíčová slova: střevní tuberkulóza − Crohnova choroba − kritické komplikace, Intestinal tuberculosis is a rare disease in developed countries, affecting mainly immigrants and immunocompromised patients. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. - Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma.
