Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Cíl práce: Podat přehled o minimálně invazivní chirurgické léčbě děložních myomů. Typ práce: Souhrnný článek. Metodika: Analýza dostupných literárních zdrojů. Závěr: Děložní myomy jsou nejčastějšími tumory ženského reprodukčního systému. Asi 25 % pacientek je symptomatických. Mezi symptomy při myomatóze patří nepravidelné krvácení, potíže z útlaku okolních orgánů, pánevní bolesti, infertilita a těhotenské komplikace. Problematika se netýká jen symptomatických žen, ale také žen, kterým se nedaří otěhotnět, anebo se léčí s infertilitou. V léčbě pokročilých nálezů dominuje chirurgická léčba. Metodou volby u žen plánujících graviditu je laparoskopická myomektomie. Laparoskopická okluze děložních tepen je alternativní metodou pro ženy s mnohočetnou myomatózou či myomem v nepříznivé lokalizaci. U žen nepřejících si zachovat dělohu je metodou volby hysterektomie. Vaginální i laparoskopický přístup je ve srovnání s abdominální hysterektomií spojen s rychlejší rekonvalescencí. Study design: Review. Methods: Analysis of available literature resources. Conclusion: Uterine fibroids are the most common benign tumour occuring in women. Symptoms occurs by 25 % of women with uterine fibroids, but some may have abnormal uterine bleeding, pelvic pain, problems of oppression adjacent organs infertility and complication during the childbirth and pregnancy. The surgery treatment is prefer in the treatment of large myomas. Problems with treatment of fibroids applies not only to symptomatic patients who wish to preserve the uterus, but also women who have uterine fibroids negatively affect their reproduction. The surgical treatment dominates in the treatment of large myomas. Laparoscopic myomectomy is the best option by symptomatic patients with pregnancy plans. Laparoscopic occlusion of uterine arteries is an alternative method for women with multiple fibroids or myoma in unfavorable location. By women not wishing to preserve the uterus is suitable hysterectomy. Vaginal and laparoscopic approach is associated with faster recovery as compared with abdominal hysterectomy. Pánevní kongesce je zvláštní forma chronického žilního onemocnění postihující ženy. Definovaná je jako klinicko‑anatomický syndrom charakterizovaný chronickou pánevní bolestí, perineálním dyskomfortem, mikčními obtížemi a postkoitální bolestí, které jsou způsobeny ovariálním a/nebo pánevním žilním refluxem. Bývá příčinou varixů vulvy, perinea, hýždí a dolních končetin. Těsný vztah je mezi symptomatickými pánevními varixy a renálním syndromem louskáčku. Duplexní ultrasonografie žil břicha, pánve, dolních končetin a vulvoperineální oblasti je hlavní neinvazivní diagnostickou metodou. Magnetická rezonanční venografie nebo jen MR venografie má vysokou senzitivitu v hodnocení pánevní kongesce a může být užita jako screeningové vyšetření nebo v případě nejasného nálezu na ultrazvuku a klinických pochybností. Možnosti medikamentózní terapie jsou omezené, základními léky jsou venofarmaka. Endovaskulární intervence – terapeutická embolizace ovariálních/pánevních žil a implantace ilikokaválního a renálního endovenózního stentu – se stává léčbou první volby u žen s těžkými příznaky, které nereagují na konzervativní terapii., Pelvic congestion is a special form of chronic venous disease in women. This is defined as s syndrome which includes chronic pelvic pain, perineal discomfort, difficulties of micturition, and post‑coital pain in association with ovarian and/or pelvic vein reflux. It may cause vulvar, perineal, gluteal and lower extremity varices. There is a close relation between symptomatic pelvic varices and renal nutcracker syndrome. Duplex ultrasound of abdominal, pelvic and lower extremity veins and vulvoprineal region is the cardinal method of non‑invasive examination. MRI Venography is a highly sensitive technique for evaluating pelvic congestion. It may be used as a screening method or in cases of ambiguous sonographic findings and doubts. Pharmaceutical treatment options are limited, and therapy is based on venoactive drugs. Endovascular intervention – ovarian/iliac veins embolisation therapy and ilio‑caval and renal venous stenting – becomes the first line of treatment for women with severe symptoms, which are resistant to conservative approaches., and Holý M.
