Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. The Purpose of the study: define the indications and contraindications to sphincterpreserving operation in lowerampullar cancer of rectum. The Materials and methods: In coloproctological branch of National Oncological Scientific centre (NOSC) in 20052009 years 142 patients were performed radical operations, 64 (45,1%) patients were performed sphincterpreserving operations abdominoanal resection of rectum (AARR), medium age of patients 59,8 years, beside 78 (54,9%) patients were performed abdomeno perineal extirpation of rectum (APER), medium age 54,2 years. The Results: Studied results of the morphological research of removed preparation by histological structure of tumors and by form of the growing, depending on locations of the tumors on toothed line. The remote postoperative results studied beside 51 (79,7%) patient with AARR and beside 63 (80,1%) patients with APER. The Findings: sphincterpreserving operations (SPO) of the rectum can be the operation of the choice in treatment of high differentiated adenocarcinoma (HDA) and medium differentiated adenocarcinoma (MDA) in stages T23 in localization of the lower pole of tumors, not below 1 centimeter of toothed line (TL) and carries combined character, in location of the lower edge of tumors on toothed line in patients with HDA MDA in 4% and 2,2% cases are shown that completing intrasphincternal resections of rectum, in connection with particular aggressiveness LDA question about choice of the method of the treatment under their localizations directly on TL must be solved in favour of complete APER, in exophytic tumor located onto and above TL is indication for sphincterpreserving operation in type of AARR, indices recurrencefree and not metastatic period, oneyear deathrate, threeyear survivability after SPO at inferior ampular cancer of rectum has not an essential distinction from results after APER., A. Aim of investigation: to study immediate and remote results of standard and extended lymphodissection in patients with rectal cancer. Material and methods: 132 patients with rectal cancer were performed radical surgery with lymphodissection in D2 and D3 volume according to a height of tumor location. In immediate postoperated period there were noted complications in 4 patients that made up 3,03%. Observation time was 5 years. Tumor recurrence was reported in 5 patients (3,8%). In remote period metastases were revealed in 8 patients (6,6%). 5year total survival rate was 78,5±4,2% at stage II. 5year total survival rate was 56,4±3,7% at stage III. Comparing the results of standard and extended lymphodissection it was noted statistically authentic increase of 5 year survival rate in patients at stage III. Obtained data allow to come to the conclusion about reasonability of performance of different variants of lymphodissection for patients with rectal cancer at stages II and III., Navruzov S. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet.
