INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Dobře a středně diferencované neuroendokrinní tumory plic představují malou, ale nezanedbatelnou skupinu plicních tumorů. Jsou více známy pod zažitým označením typický a atypický karcinoid. Díky své poměrně příznivé prognóze (pětileté přežití 90 %, resp. 40–60 %) a sporadickému výskytu (1–2 % plicních malignit) stály ve výzkumu a vývoji často ve stínu bronchogenního karcinomu. Pokroky v oblasti molekulární biologie a genetiky recentně popsaly jednak signální dráhy karcinoidových buněk, jednak identifikovaly nové potenciální molekulární cíle a slibná léčiva. Jako nejvýznamnější se jeví PI3K/Alt/mTOR signální kaskáda a s ní spřažené signální cesty, např. kaskáda IGF-1 receptoru nebo receptorů pro vaskulární endoteliální růstové faktory. Ač příslušná léčiva nejsou až na výjimky dosud schválena v terapii karcinoidu plic, výsledky pilotních studií, jež se snažíme shrnout, jsou povzbuzující. Nové léky ze skupiny mTOR inhibitorů nebo anti-VEGF monoklonálních protilátek tak rozšiřují dosud omezené možnost léčby těchto chemorezistentních tumorů. Slibně se jeví též kombinace standardní léčby somatostatinovými analogy s radionuklidovou terapií., Low and intermediate grade neuroendocrine tumors of the lungs, more often known as typical and atypical carcinoid, represent minor, but clinically important part of lung neoplasms. In past years they were somewhat on the edge of interest in both research and therapy because of their favorable prognosis (5-year survival 90 %, respectively 40–60 %) and low incidence (1–2 % of all pulmonary tumors). However, recent advances in molecular biology and genetics helped to understand signal pathways and processes used by carcinoid cells. New molecular targets were identified, opening possibility for development of new therapeutic drugs. Especially blockade of PI3K/Alt/ mTOR signal cascade and associated pathways, like IGF-1 receptor or vascular endotelial growth factor receptors, shows great potential in stopping proliferation of carcinoid cell. Although most of appropriate therapeutics is not yet approved for pulmonary carcinoid therapy, results of summarized pilot studies are encouraging. V databázi Globocan 2008 (IARC) bylo za rok 2008 z celosvětového počtu 12,663 milionů nových nádorů odhadováno 2,445 milionů v EU, ze 7,565 milionů zemřelých téměř 1,235 milionů v EU a z pětileté prevalence 28,8 milionů asi 6,617 milionů v EU. Početnější prevalence se týkala ve světě 18 % nádorů prsů, 11 % kolorekta, 11 % prostaty, 6 % plic, 5,5 % žaludku, 5,5 % cervixu, 4 % měchýře a 4 % dělohy, v EU 20 % ZN prsů, 14 % kolorekta, 18 % prostaty, 5 % plic, 4 % melanomu, 3 % ledvin, 3 % dělohy a 3 % NH lymfomu. V roce 2030 mohou počty nových nádorů dosáhnout ve světě 21 milionů případů, v EU 3 miliony, v ČR 93 tisíc (dg. C00-D09). Z předběžných celkových výdajů 288 572 mil. Kč na zdravotnictví za rok 2011 uhradily zdravotní pojišťovny 217 653 mil., z toho 19 217 mil. na onkologii. V letech 1990–2011 vzrostly celkové náklady na zdravotnictví o 257 534 mil. Kč s podílem 7,58 % na hrubém domácím produktu. Průměrné výdaje VZP na léčbu jednoho nádoru v roce 2010 dosáhly 19 030 Kč s nejvyššími náklady u novotvarů mízní a krvetvorné tkáně, mozku a CNS, trávicího ústrojí, úst a hltanu, dýchacích cest. Výkonnost globální ekonomiky nedosahuje potřebných prostředků na zdravotnictví. Český veřejný dluh překračuje 1 680 miliard Kč v době dokončení tohoto článku. I když celkové výdaje obyvatel na zdravotní péči rostou, zdá se, že jeho pokračující zadlužování zvýší také dluhy nemocnic a zdravotních pojišťoven. Dostupné počty nádorů a výdajů na zdravotnictví a jejich zvyšování potvrzují nutnost důsledné prevence rakoviny, efektivní přidělování a využívání prostředků na zdravotní péči., In 2008 (Globocan 2008; IARC), 12.663 million new cancer cases occurred worldwide and, of them, 2.445 million in the EU. Here preliminary total health care expenditures in 2011 were 288,572 million CZK; of these, health insurance companies covered 217,653 million, with 19,217 million per one cancer treatment. Between 1990 and 2011, the total health expenditures increased by 257,534 million CZK, accounting for 7.58 % of the gross domestic product. In 2010 the General Health Insurance Company paid on average 19,030 CZK for the treatment of one cancer case, with the highest expenditures for neoplasms of the lymphoid and haemotopoietic tissues, brain and central nervous system, digestive organs, oral cavity and pharynx, and respiratory organs. The global economic performance cannot provide enough resources for public health care. The Czech public debt exceeded 1,680 billion CZK at the time this article was written. Although out-of-pocket money paid by cancer patients for treatment is growing, the continuing public indebtedness is likely to increase debts of hospitals and health insurance companies. Úvod: Takotsubo kardiomyopatie (takotsubo cardiomyopathy – TCM) je vzácné srdeční onemocnění charakterizované v akutní fázi přechodnou dysfunkcí levé komory, které může být komplikováno fibrilací komor nebo srdečním selháním. V diferenciální diagnostice musí být vždy vyloučen akutní infarkt myokardu. Cílem práce je prezentovat charakteristiku pacientů s TCM, jejich jednoletou prognózu a přínos magnetické rezonance k diagnostice těchto pacientů v klinické praxi. Metodika: Vyhodnoceno bylo celkem 47 pacientů s prokázanou TCM. U všech pacientů byla vyloučena významná ateroskleróza koronárních tepen pomocí selektivní koronární angiografie. Celkem u 10 pacientů (21,3 %) bylo provedeno vyšetření magnetickou rezonancí (MRI) srdce. Pacienti byli prospektivně sledováni minimálně 12 měsíců. Výsledky: Soubor pacientů s TCM tvořily z 89,4 % ženy, průměrný věk pacientů byl 62,3 ? 11,1 let. Třicet devět pacientů (83,0 %) mělo apikální a 8 midventrikulární formu TCM. Zjevný provokující stresový faktor jsme identifikovali u 18 pacientů (38,3 %). Časté byly další komorbidity, zejména onemocnění štítnice, psychiatrická, plicní nebo alergická onemocnění. U 42 pacientů (89,4 %) byla přítomna dysfunkce levé komory, 4 pacienti (8,5 %) byli v kardiogenním šoku. Čtyři pacienti (8,5 %) prodělali během hospitalizace fibrilaci komor. Roční mortalita v našem souboru činila 14,9 %, jedna pacientka (2,1 %) prodělala v průběhu sledování recidivu ataky TCM. Závěr: Přestože u pacientů s TCM dochází k úpravě funkce levé komory, akutní fáze může být komplikována rozvojem kardiogenního šoku nebo fibrilací komor. U části pacientů je indikováno vyšetření MRI k vyloučení ischemické nebo zánětlivé etiologie., Introduction: Takotsubo cardiomyopathy (TCM) is a rare cardiac disease. In the acute phase it may imitate acute myocardial infarction of the anterior wall with ST elevations. This can be complicated by ventricular fibrilation or heart failure. The aim of this study is to show the characteristics of patients with takotsubo cardiomyopathy, the one year outcome and the application of magnetic resonance imaging of the heart in the diagnosis of this disease. Methods: Forty seven patients with takotsubo cardiomyopathy were enrolled in this study. Severe stenosis of coronary arteries was excluded by selective coronarography. In twelve patients the magnetic resonance imaging of the myocardium was performed. The patients were observed for at least 12 months. Results: 89.4 % of takotsubo patients were women. Average of age was 62.3 ? 11.1 years. We found obvious stress factors in 18 patients (38.3 %). Many patients suffered from thyroid disorders, psychiatric, lung or allergic diseases. Thirty nine patients (83.0 %) had the apical and eight the midventricular type of takotsubo cardiomyopathy. In 42 patients (89.4 %) developed systolic left ventricular dysfunction and four patients (8.5 %) were in cardiogenic shock. Four patients (8.5 %) manifested ventricle fibrilation. One patient (2.1 %) had recurring takotsubo cardiomyopathy. Conclusion: Left ventricle function is mostly recovered in patients with takotsubo cardiomyopathy, but the acute phase can be complicated by cardiogenic shock or ventricular fibrilation. Během posledních 30 let se transplantace pankreatu stala rutinním postupem, který je zvláště vhodný pro pacienty s diabetem 1. typu, kteří současně podstupují transplantaci ledviny. Zlepšuje kvalitu života a v porovnání s transplantací samotné ledviny zlepšuje dlouhodobé přežívání. Poskytuje nezávislost na inzulinu s téměř normální glukózovou tolerancí, uvolnění diety, klid od hypoglykemií a možnost zastavení postupu nebo dokonce regresi mikroangiopatických komplikací. Samostatně se transplantace pankreatu provádí zejména u vybraných pacientů s rekurentními epizodami těžkých hypoglykemií při syndromu porušeného vnímání hypoglykemie nebo jako následný výkon u příjemců s funkčním štěpem ledviny od zemřelého či žijícího dárce. 5leté přežívání funkce štěpu pankreatu činí při kombinované transplantaci, transplantaci samotného pankreatu či transplantaci pankreatu následně po ledvině více než 75, 50 a 62 %. V rozporu se zlepšujícími se výsledky je určitá stagnace počtu výkonů v některých evropských zemích a v USA. Ačkoliv se na tom podílí také horší kvalita dárců, zavádění ostrůvkové transplantace jako méně invazivní procedury a naděje v technologická řešení, hlavní příčina spočívá v nedostatečném povědomí o současné úspěšnosti a menším počtu pacientů na čekací listině. ČR se v oblasti transplantační léčby diabetu tradičně řadí k nejaktivnějším zemím. Pro pacienty jsou s ohledem na klinickou potřebu k dispozici různé varianty včetně transplantace Langerhansových ostrůvků., During the past 30 years pancreas transplantation evolved into a routine procedure especially suitable for type 1 diabetic recipients undergoing simultaneously kidney transplantation significantly improving quality of life and life expectancy as compared with kidney only recipients. It provides insulin independence with near-normal glucose control without special dietary restriction, freedom from hypoglycemia and chance for halting or regression of microangiopathic diabetes complications. As a separate procedure, pancreas transplantation is carried out mainly in selected subjects suffering from severe hypoglycemic episodes and impaired hypoglycemia awareness or as a subsequent procedure in type 1 diabetic kidney recipients from both cadaveric or living donors. Five-year insulin independence rate following combined pancreas and kidney, pancreas only and pancreas after kidney procedures currently exceed 75, 50 and 62 %, respectively. Though the outcomes still continue to improve, the rate of pancreas transplants has reached a plateau in several European countries or even declines in the United States. Main reasons for that include fewer referrals from diabetes specialist, decreased donor quality, introduction of islet transplantation as a less invasive procedure but probably most of all probably insufficient information on the latest progress and trends achieved in this area. Životní prognóza pacientů s diabetes mellitus 1. typu (DM1T) se dramaticky změnila po objevu inzulinu v roce 1922, ale stále byla o 25 let kratší ve srovnání s nediabetickou populací. Někteří lidé s DM1T však žili stejně dlouho jako nediabetická populace a neměli pozdní komplikace diabetu. Začali být oceňováni medailemi za dlouhý život s diabetem. Stali se však i předmětem výzkumu zabývajícího se otázkou, proč žijí tak dlouho, čím se odlišují od těch pacientů s DM1T, kteří žili krátce. Práce se zabývá odlišnostmi medailistů a diskutuje různé hypotézy, které by mohly tyto odlišnosti vysvětlit. Zdá se, že velice důležitá je kvalitní kontrola diabetu v prvních 20 letech od stanovení diagnózy, protože existuje tzv. glykemická paměť, která může v dalších letech životní prognózu významně ovlivnit. Život lidí se lineárně prodlužuje od počátku 19. století, prodlužuje se však i život osob s DM1T. Je to způsobeno jednak kvalitnější kontrolou glykemie, jednak kvalitnější prevencí a léčbou komplikací. Ukazuje se, že klesá výskyt terminálních stadií diabetické nefropatie, zlepšuje se primární i sekundární prevence kardiovaskulárních komplikací i kardiologická a kardiochirurgická léčba. Jako nejvýznamnější prognostické faktory zvýšené mortality se jeví manifestní proteinurie, diabetická neuropatie a hypertenze. Pokud nejsou přítomny tyto ukazatele, neliší se životní prognóza pacientů s DM1T významně od životní prognózy nediabetické populace. Klíčová slova: diabetická nefropatie – diabetes mellitus 1. typu – glykemie – glykemická paměť – ICHS – mortalita – pozdní komplikace diabetu – prognóza, The life expectancy of Type 1 diabetes mellitus (T1DM) dramatically improved after the discovery of insulin in 1922, but was still 25 years shorter than that of non-diabetic population. Some people with T1DM, however, lived to the same age as a non-diabetic population and had no late complications of diabetes. They began to be awarded medals in appreciation of their long life with diabetes. They also became the subject of a research examining why they lived so long and what was the difference between them and those patients with T1DM, whose lives were much shorter. The paper deals with the differences observed in the ´medallists´ and discusses various hypotheses that might account for them. It seems that reliable control of diabetes within the first 20 years following the diagnosis is very important, in relation to the existence of “glycemic memory“ which may significantly affect life expectancy in the following years. Human lifespan in general has been linearly extended since the early 19th century and the same holds for lifespans of people with T1DM. This is due to the higher quality control of glycemia on the one hand, and a better prevention and treatment of complications. It is observed that the incidence of terminal stages of diabetic nephropathy has been dropping, the primary as well as secondary prevention of cardiovascular complications, cardiological treatment and heart surgery have been improving. Manifest proteinuria, diabetic neuropathy and hypertension appear to be major prognostic factors of increased mortality. If these indicators are not present, the life expectancy of patients with T1DM does not significantly differ from that of the non-diabetic population.
