Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Surgery with stapedotomy and stapes prosthesis implantation in a case of otosclerosis with avascular necrosis of stapes crura can be success therapy to improve patient's hearing and M. Erdoglija, J. 63letý pacient, hypertonik, celoživotní kuřák, byl na kliniku TRN FN Plzeň přijat pro tumor pravé plíce zjištěný pro bolesti v pravém hemitoraxu. Na základě stážování byla stanovena diagnóza adenokarcinomu pravé plíce s generalizací do obou plic a levé nadledviny, st. IV. Byla zahájena chemoterapie Paclitaxel (200 mg/m2) – Carboplatina (AUC) – Avastin (15 mg/kg) v rámci studie SAIL firmy Roche s velmi dobrou tolerancí. Po 3. cyklu chemoterapie byla potvrzena parciální remise dle CT hrudníku, dále pokračováno ve stejném schématu chemoterapie s Avastinem do 6. cyklu. Kontrolní CT hrudníku prokázalo další regresi tumoru. Vzhledem k velmi dobrému efektu podávané léčby bylo dále pokračováno v monoterapii Avastinem v třítýdenních intervalech zatím do 22 cyklů. Po 22 měsících léčby s Avastinem bylo dosaženo trvající parciální remise nálezu dle PET/CT trupu, pacient je klinicky zcela bez potíží, neomezen v běžném životě, tedy s velmi dobrou kvalitou života (KS 90 bodů), bez vedlejších nežádoucích účinků Avastinu. Chemoterapie již nemá potenciál pro další prodlužování přežití, je tedy potřebné hledání nových léčebných strategií. Avastin jako inhibitor angiogeneze vede poprvé k prodloužení celkového přežití nad 12 měsíců. Významné prodloužení přežití bez progrese onemocnění (PFS) umožňuje delší přežití bez zhoršení symptomů., A 63-year-old hypertensive patient, a life-long smoker, presented to the Department of Tuberculosis and Respiratory Disease of the Plzeň Teaching Hospital with a right-lung tumour detected due to pain in the right hemithorax. Based on staging, a diagnosis of stage IV right lung adenocarcinoma with generalization into both lungs and the left adrenal gland was made. Chemotherapy with Paclitaxel (200 mg/m2) – Carboplatin (AUC) – Avastin (15 mg/kg) was initiated within the SAIL study of the Roche company with very good tolerance. Following the third chemotherapy cycle, partial remission was confirmed based on CT of the chest; the same chemotherapy regimen with Avastin was continued for six cycles. Follow-up CT of the chest revealed further tumour regression. Given the very good effect of the treatment administered, monotherapy with Avastin was continued in three-week intervals for 22 cycles so far. Following 22 months of Avastin treatment, sustained partial remission was achieved according to PET/CT of the trunk; the patient is clinically free of complaints, without limitations in everyday life, thus with a very good quality of life (KS of 90) and free of adverse effects of Avastin. Chemotherapy has no more potential for further extension of survival; therefore, new therapeutic strategies have to be looked for. As an angiogenesis inhibitor, Avastin leads to an extension of overall survival beyond 12 months. Metastatický karcinom prsu je nevyléčitelné onemocnění, ale použití nových léčebných možností může zmírnit obtíže pacientky, prodloužit dobu do progrese a prodloužit celkové přežití. Léčba protinádorovou chemoterapií je často doprovázena řadou závažných nežádoucích účinků a v průběhu léčení časem dojde k rezistenci na chemoterapii. Nové naděje se tedy vkládají do kombinace chemoterapie s cílenou léčbou. Vaskulární endoteliální růstový faktor (VEGF) je jedním z nejdůležitějších regulátorů angiogeneze. Humanizovaná monoklonální protilátka proti VEGF – bevacizumab prokázal účinnost u řady nádorových onemocnění. Karcinom prsu v diseminovaném stadiu je nevyléčitelné onemocnění s definovanou dobou přežití, která v případě viscerálních metastáz dosahuje jen několika měsíců. U pacientek s viscerálním postižením, symptomatických a hormonálně nedependentních je základem léčby chemoterapie. U pacientek HER2 negativních (resp. triple negativních) má své místo antiangiogenní léčba bevacizumabem, která je schválena v I. linii léčby metastatického HER2 negativního karcinomu prsu v kombinaci s paclitaxelem, a to na základě významného prodloužení PFS ve studii III. fáze E2100 (1). Kazuistika: 53letá pacientka onemocněla triple negativním karcinomem prsu v roce 2008. Absolvovala neoadjuvantní chemoterapii, radikální mastektomii s exenterací axilly s pooperačním stagingem ypT1a ypN1. Následovala adjuvantní chemoterapie, ozáření. V květnu 2012 prokázány metastázy v plicích a v mediastinálních uzlinách dle CT. Provedena bronchoskopie a histologicky verifikovány metastázy duktálního karcinomu, triple negativního, dle FISH nebyla nalezena amplifikace genu HER2. Indikována ke kombinované léčbě paclitaxel + bevacizumab. Došlo ke kompletní remisi onemocnění. Léčba samotným bevacizumabem pokračuje dosud. Pacientka přežívá od zjištění generalizace dosud 43 měsíců! Kvalita života je výborná., Metastatic breast cancer is an incurable disease with a defined time of survival. This time is limited to only a few months when visceral metastases are present. Chemotherapy is the crucial treatment for patients with visceral involvement, symptomatic and hormone-independent disease. There is a role as well for an antiangiogenic therapy with bevacizumab in HER2-negative disease. Bevacizumab is approved for the treatment of metastatic HER2-negative breast cancer in combination with paclitaxel. This approval is based on significant improvement in PFS reported in phase III trial E2100. Case report: 53-years old patient, female, was diagnosed with triple-negative breast cancer in 2008. Chemotherapy was delivered in neoadjuvant setting, followed by radical mastectomy with dissection of axilla. Pathological staging was ypT1a ypN1. Adjuvant chemotherapy was applied as well as radiation therapy. In May 2012 the evidence of metastatic disease in lung and mediastinal lymph nodes was proved by CT scan. Triple negative breast cancer was verified from bronchology exam. FISH analysis of HER2 was also negative. Combination treatment with paclitaxel + bevacizumab was indicated. Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta.
