V tomto článku budou souhrnně rozebrány jednotlivé mechanismy, kterými etanol podporuje vznik a růst karcinomů horních dýchacích a polykacích cest. Etanol v kancerogenezi působí jako kokancerogen tím, že usnadňuje průnik jiných kancerogenů do buněk. Etanol však představuje i přímý kancerogen, neboť z něj vzniká kancerogenní formaldehyd. Podporovat růst nádorů může i skrze oslabení imunity anebo hormonální dysbalanci., This article summarizes mechanisms through which ethanol promotes the genesis and growth of carcinomas of an upper aerodigestive tract. Ethanol plays a role in cancerogenesis as a cocancerogen because it facilitates penetration of other cancerogens to cells. However, ethanol also represents a direct cancerogen, because the cancerogenic acetaldehyde is formed of it. Gastrointestinální stromální tumor je nejčastější mezenchymální nádor trávicího traktu. V současné době je definovaný jako nádor tvořený vřetenitými a/nebo epiteloidními buňkami, jejichž fenotyp odpovídá diferenciaci v Cajalovy interstitiální buňky. Nejčastěji se gastrointestinální stromální tumor vyskytuje v žaludku, méně často v ostatních částech trávicí trubice, vzácně i mimo trávicí trakt, a to v mezenteriu, omentu, retroperitoneu, žlučníku, močovém měchýři, pankreatu a ve vagině. Ve většině případů je možná poměrně spolehlivá diagnóza z rutinního barvení hematoxylinem a eosinem ve světelné mikroskopii. Imunohistochemická vyšetření zahrnují pozitivní průkaz KIT proteinu (CD117), PDGFRA a/nebo DOG-1, stejně jako negativní průkaz markerů jiných diferenciačních linií. Molekulárně genetické vyšetření genů KIT a PDGFRA má význam nejen diferenciálně diagnostický, ale i prediktivní. Negativita těchto vyšetření však diagnózu nevylučuje. Každý gastrointestinální stromální tumor je třeba považovat za potenciálně maligní. Riziko agresivního chování se v současné době stanovuje na základě lokalizace, mitotického indexu a maximálního rozměru nádoru., Gastrointestinal stromal tumor is the most frequent mesenchymal tumor of the alimentary tract, currently being defined as a tumor composed of spindle and/or epithelioid cells presumably differentiating towards interstitial cells of Cajal. The most common location of gastrointestinal stromal tumor is stomach followed by other sites of gastrointestinal tract. Occasional sites of occurrence are mesenterium, omentum, retroperitoneum, gallbladder, urinary bladder, pancreas and vagina. Light microscopic examination of slides stained with haematoxylin and eosin is highly reliable in most cases. Immunohistochemical investigation relies on expression of KIT protein (CD117), PDGFRA and/or DOG-1, as well as absence of markers of other differentiation lineages. Molecular genetic analysis of KIT and PDGFRA genes is useful not only for differential diagnosis, but also for prediction of therapeutic response. Nevertheless, negative results do not exclude histologically proven diagnosis. CT s vysokým rozlíšením (HRCT) sa stalo metódou voľby v zobrazovaní a diagnostike ochorení stredného ucha a prevodovej poruchy sluchu. Pomocou HRCT dokážeme detailne popísať anatómiu spánkovej kosti, patológiu stredného ucha (otitis média chronica, cholesteatóm a ich komplikácie) a traumatické zmeny. Magnetická rezonancia má naopak vysoké rozlíšenie mäkkých tkanív, preto ju využívame k diferenciálnej diagnostike mäkkotkanivovej masy stredného ucha zobrazenej na CT, k odlíšeniu reziduálneho ochorenia alebo recidívy v teréne pooperačných zmien a k zobrazeniu intrakraniálnych komplikácií akútnej a chronickej otitídy CT a MR sú komplementárnymi zobrazovacími metódami, pomáhajú stanoviť správnu diagnózu, ktorá je pre klinika dôležitá z hľadiska stratégie liečby. V publikácii uvádzame spektrum patológií stredného ucha, s ktorými sa pri zobrazení spánkovej kosti môžeme stretnúť a na ktoré treba myslieť., High resolution computed tomography (HRCT) is a modality of choice in diagnostic imaging of middle ear pathology and conductive hearing loss. It enables to image a detail anatomy of temporal bone, pathology of middle ear (e.g. chronic otitis média, cholesteatoma and their complications) and traumatic changes in this area. On the other hand, magnetic resonance imaging (MRI) enables a high resolution of soft tissue structures. It is used in imaging of soft tissue masses in middle ear cavity previously diagnosed on CT, differentiating residual disease or recurrence in postoperative ear, and in imaging of intracranial complications of acute and chronic otitis. CT and MRI are complementary imaging modalities in helping the clinicians to determine the proper strategy for patient treatment.
