Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. Možnosti léčby jsou závislé na stadiu nemoci, celkovém stavu pacienta a také molekulární charakteristice nádoru. V rámci předoperační péče se léčebné postupy u karcinomu konečníku mírně liší od léčby karcinomu tlustého střeva. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Radikální resekce metastáz je v současné době nejefektivnější léčebnou metodou u nemocných s jaterními metastázami kolorektálního karcinomu. U části nemocných, u kterých nejsou metastázy primárně resekabilní, je účinná tzv. konverzní léčba, kterou se daří provést downsizing ve 25 % s následným provedením jaterní resekce. Velké jaterní resekce a podávání biochemoterapie je vhodné provádět na specializovaných pracovištích, na kterých je garantováno multidisciplinární pojetí léčby. Tento přístup zaručuje nejen nízké operační riziko a podávání adekvátní (neo)adjuvantní léčby, ale především její komplexnost s dosažením více než 50 % pětiletého přežití s dobrou kvalitou života. Další zlepšení osudu těchto nemocných je podmíněno brzkým přechodem na individualizovaný přístup a personalizované pojetí léčby., Radical resection of metastases is currently the most effective treatment modality in patients with liver metastases of colorectal cancer.
