LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Multidisciplinary approach is essential in the treatment., and K. Menclová, F. Bělina, J. Pudil, D. Langer, M. Ryska
Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Limited financial resources and persistent risks of lifestyle do not allow to slow down the increase of primary and subsequent neoplasms in the near future among cancer survivors not only in gynaecology., Edvard Geryk, Radim Štampach, Viera Bajčiová, Teodor Horváth, and Literatura
In view of the high anti-oxidative potential oftocotrienol, the role of the tocotrienol-rich fraction (TRF) of palm oil in preventing pregnancy induced hypertension (PIH) was explored in a randomized double-blind placebo-controlled clinical trial in an urban teaching hospital. Healthy primigravidae were randomized to receive either oral TRF 100 mg daily or placebo, from early second trimester until delivery. Out of 299 women, 151 were randomized into the TRF arm and 148 into the placebo arm. A total of 15 (5.0%) developed PIH. Although there was no statistically significant difference in the incidence of PIH (4/151 or 2.6% in the TRF arm vs. 11/148 or 7.4% in the placebo arm, p = 0.058) between the two arms, there was a tendency towards a lower incidence of PIH in the TRF arm compared to the placebo arm. With TRF supplementation, the relative risk (RR) of PIH was 0.36 (95% CI 0.12-1.09). In conclusion, although TRF from palm oil does not statistically significantly reduce the risk of development of PIH in the population studied, the 64% reduction in incidence of PIH is substantial. The findings warrant further clinical trials, particularly in high risk populations. and ZA. Mahdy, HH. Siraj, H. Khaza'ai, MS. Mutalib, MH. Azwar, MA. Wahab, AZ. Dali, R. Jaafar, NA. Ismail, MA. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. The disease by itself is a poor prognostic factor in a one-year mortality rate. Key words: incidence infective – infective endocarditis, and Rudolf Koubek
Byly změřeny základní antropometrické údaje, tepová frekvence, krevní tlak, zjištěny hodnoty sérového lipidogramu a glykemie. Vyšetřující lékaři zaznamenávali přítomnost nově zjištěných rizik KV onemocnění a stanovili celkové KV riziko dle tabulek SCORE. Aktivně kouřilo 36 % mužů a 22 % žen. 62 % mužů a 65 % žen doporučili participující lékaři nefarmakologickou léčbu. Závěr: Provedené šetření dokumentuje vysokou prevalenci rizikových faktorů kardiovaskulárních onemocnění v populaci 40letých mužů a 50letých žen. Vysoké zastoupení nově zjištěných rizikových faktorů i nadpoloviční podíl osob, u nichž byla v rámci provedeného šetření zahájena farmakoterapie, jasně dokazují nezbytnost časného screeningu rizikových faktorů. Genetická determinace jednotlivých kardiovaskulárních rizikových faktorů odrážející se v rodinné anamnéze je významnou komponentou celkového rizika oběhových onemocnění, a musí být aktivně zjišťována a zohledňována při úvaze o volbě intenzity preventivních strategií., Background: Cardiovascular (CV) disease belongs to the most important mortality causes worldwide. Early identification of risk factors and increased CV risk may help decrease of morbidity and mortality. An optimal age for CV risk factors screening ages of 40 in males and of 50 in females have been identified. CV risk profile of persons in these age categories has been examined by general practitioners and recorded for the purposes of this cross-sectional survey. Methods: 1812 persons, males at average age of 40 years and females at average age of 50 years, were included into the survey. In each of the examined family and personal history including pharmacological were recorded into the study protocol as well as abusus and physical activity patterns. Basic anthropometrical parameters, e.g. heart rate, blood pressure, body mass index and laboratory measures including blood lipids and glycaemia. The participating physicians recorded newly identified risk factors CV disease and determined the global CVD risk according to the SCORE charts. Percentage of those already treated for any of the main CVD risk factors and, among these, also attaining the treatment goals. The percentage of patients with a newly identified CV risk factor was calculated. We also tested the hypothesis of a relationship between positive family history of any of the followed risk factors and its risk in the examined probands using Pearson´s test. Results: 961 males, average age of 42.9 ? 4.7 years, and 851 females , average age of 51.2 ? 3.6 years, were enrolled into the study. 49% of males and 31% of females were overweight and 32% men and 31% of women were obese. There were 36% of smokers among men and 22% among women. The prevalence of type 2 diabetes was 11% in males and also in females. Arterial hypertension was diagnosed in 43% of males and 45% of females while dyslipidemia was present in 39% of males and 41% of females. Pharmacological treatment of any of the above mentioned diseases was used in 48% of the probands, however, only 7% of them attained treatment goals of blood pressure, LDL-cholesterol and glycaemia. Type 2 diabetes was newly identified in 3% of both males and females, arterial hypertension in 8% of males and 5% of females and dyslipidemia was newly detected in 20% of probands of both genders. Non-pharmacological treatment was recommended to 62% of male and to 65% of female participants, respectively. Pharmacological treatment was initiated in 53% of males and 51% of females. In both genders this was mostly antihypertensive treatment with ACE inhibitors (29% of males and 24% of females) and lipid lowering therapy with a statin (29% of males, 27% females). The analysis of relationship between the positive family history of any of the followed diseases and their presence in the probands examined revealed significant increases of the risk for arterial hypertension, type 2 diabetes mellitus and dyslipidemia. Conclusion: The survey following CV risk profile in a cohort of 40 years old men and 50 years old women showed high prevalence of CV risk factors in these age categories in the Czech probands. The high frequency of modifiable risk factors and the need to initiate pharmacological treatment in more than one half of the examined population documents the need of early detection of risk. Genetic determination of individual major risk factors for CVD mirrored in the positive family history represents an important component of the global cardiovascular risk and must be actively detected and taken into account for the risk stratification., and Michal Vrablík, Zdeněk Chmelík, Věra Lánská
Úvod: Vysoká incidence kolorektálního karcinomu představuje v České republice aktuální a populačně závažný zdravotnický problém. Závěr: K rozhodování o způsobu vedení léčby včetně volby operace by mohla přispět klasifikace pomocí komplexního geriatrického vyšetření., Introduction: High incidence of colorectal cancer in the Czech Republic is an actual and demographically significant health issue. Half of all of the patients is older than 70 years. Both surgical and non-surgical treatment options in this group of patients depend on factors that are difficult to measure only by current oncological and anesthesiological classifications (cTcNcM, ASA). The objective of this paper is to measure the impact of age on the use of various treatment modalities within the protocol and their results, and also to suggest alternative options for therapy tolerance assessment. Methods: Analysis of data over a five-year period from the NOR database prepared by the Institute of Biostatistics and Analyses, Masaryk University. Results: In all parameters a difference was demonstrated between patients below the age of 70 and those above the age of 70 years. Older patients were disadvantaged. Only 11.2% of patients younger than 70 years were not treated, whereas 25.2% over the age of 70 years were not treated. Conclusion: A complex geriatric examination could improve the indication process in various treatment modalities, including surgery., and J. Hoch, M. Blaha, D. Malúšková
Z 20 855 nádorů hlavy a krku (HNC), uvedených v registru nádorů ČR v letech 1976–2005 bylo 4 912 vícečetných případů, z toho 3 679 (74,9 %) u mužů a 1 233 (25,1 %) u žen, tj. ze všech hlášených HNC bylo 17,6 % případů u mužů a 19,5 % u žen spojeno s výskytem dalšího nádoru. U mužů bylo 56,3 % primárních a 43,7 % následných HNC, u žen 43,8 % a 56,2 %. Počet primárních HNC rostl během první dekády a pak klesal, počet následných HNC trvale rostl. Z 425 synchronních případů bylo nejvíc nádorů dýchacího systému, kůže a jiných HNC, z 2 660 metachronních případů nádory kůže, dýchacího a trávicího systému. Následným 2 302 HNC předcházelo 2 858 jiných novotarů, nejvíce kůže, dýchacího systému a jiných lokalizací HNC. K říjnu 2007 přežívalo z primárních HNC 14,7 % mužů a 20,6 % žen a zemřelo 85,3 % mužů a 79,4 % žen. V roce 2015 bude v ČR léčeno téměř 8 tisíc přežívajících s HNC, část z nich s vícečetnými nádory. Naplní se jejich očekávané počty?, A total of 20,855 head and neck cancers (HNC), based in the Czech Cancer Registry in 1976–2005, there were notificated 4,912 multiple cancers, of which were 3,679 (74.9 %) in males and 1,233 (25.1 %) in females, i.e. 17.6 % cases in males and 19.5 % in females of total HNC, affected by other neoplasms. There were 56.3 % primary and 43.7 % subsequent HNC in males, 43.8 % and 56.2 % in females. The number of primary HNC incerased during the first decade and then decreased, the number of subsequent HNC increased steadily. A total of 14 Czech regions were distributed 51.6 % multiple HNC in five regions (Northern and Southern Moravia, Prague, Southern and Central Bohemia). The most frequent were 28.8 % multiple cancers of lips, 11.9 % of tonsils, 11.1 % of parotid gands. The primary 2,070 HNC in males were followed by 2,436 other neoplasms in the average interval 6.7 years; the primary 540 HNC in females were followed by 649 other neoplasms in males in the average interval 6.9 years. The most frequent diagnoses of 425 synchronous cases were cancers of respiratory tract, skin and other HNC, of 2,660 metachronous cases were cancers of skin, respiratory and digestive tract. The subsequent 2.302 HNC were preceded by 2.858 other neoplasms with the most frequent cancers of skin, respiratory tract and other HNC. The expected low representation of advanced clinical stages was not confirmed. Of 2,436 subsequent neoplasms in males and 649 in females were registered after early HNC stages in 16.1 % males and 18.6 % females of stages I, II, in 10.7 % males and 7.8 % females of stages III, IV; after advanced HNC stages were in 8.2 % males and 6.5% females of stages I, II, in 8.9 % males and 5.2 % females of stages III, IV; the unknown stages represented 56.1 % in males and 61.9 % in females of subsequent neoplasms. All 729 subsequent advanced cancers (606 males, 123 females) represented 3.5 % of total registered cases of HNC during 30 years and about 1,540 patients with HNC affected by other subsequent cancer of 43,850 HNC based in the Czech Cancer Registry during half-century. Up to Oct. 2007, of the primary HNC 14.7 % males and 20.6 % females survived, and those 85.3 % males and 79.4 % females died. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. In this period the number of primary cases increased twice, of subsequent ones nearly 30 times during five periods. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract. A total of 962 multiple MN CNS there were only 91 survived and 871 death patients up to analysis in October 2007 with predominance of surviving females over males., Edvard Geryk, Jiří Kozel, Radim Štampach, Dalibor Pacík, Petr Dítě, Petr Kubíček, Milan Konečný, and Lit.: 30
